수술(개흉·개복술) 부위로의 지속적 국소마취제 투여법

Abstract

평가배경 수술 부위로의 지속적 국소마취제 투여법(Continuous wound infiltration, CWI)은 개흉·개복술 환자 중 기존 통증 조절 방법으로 통증 조절이 충분하지 않은 환자, 기존 통증 조절 방법을 사용할 수 없는 환자를 대상으로 통증 조절을 목적으로 사용하는 기술이다. 2010년 신의료기술로 평가된 후 2011년 비급여로 등재되었고, 2016년 선별급여로 변경된 의료기술이다. 해당 의료기술은 보건복지부 급여화 추진 계획 분야를 중심으로 선별급여 항목 검토를 통해 내부 모니터링을 통해 발굴된 주제로, 2022년 제6차 의료기술재평가위원회(2022.6.10.)에서 평가계획서를 심의받아 수술 부위로의 지속적 국소마취제 투여법의 안전성과 효과성을 확인하기 위해 재평가를 수행하였다.

평가방법 ‘수술 부위로의 지속적 국소마취제 투여법’의 임상적 안전성 및 효과성을 평가하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “수술 부위로의 지속적 국소마취제 투여법 소위원회(이하 ‘소위원회’)”의 심의를 거쳐 확정하였다. 소위원회는 의료기술재평가자문단에서 무작위로 선정된 마취통증학과 2인, 일반외과1인, 흉부외과 1인, 산부인과 1인, 근거기반의 1인, 총 6인으로 구성하였다. 평가의 핵심질문은 “흉부수술(개흉·흉강경) 환자에서 수술 부위로의 지속적 국소마취제 투여법은 임상적으로 안전하고 효과적인가?”, “복부수술(복부 빛 복강경) 환자에서 수술 부위로의 지속적 국소마취제 투여법은 임상적으로 안전하고 효과적인가?”였다. 안전성 지표는 시술 관련 합병증, 약물 부작용 발생이고, 효과성 지표는 통증 점수, 수술 후 추가 진통제 소비량이었다. 또한, 관련 문헌의 양이 풍부하고 엄격하게 수행한 질 높은 문헌을 통해 근거를 도출하기 위해 연구유형은 무작위배정 비교임상시험(Randomized controlled trial, RCT)로 제한하였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 수행하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하여 두 명의 검토자가 독립적으로 수행하고 의견 합의를 이루었다. 자료추출은 미리 정해 놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 평가자 간 합의를 통하여 확정하였다. 자료분석은 기본적으로 대조군 유형에 따라 나누어 수행하였으며, 양적 분석이 가능할 경우 메타분석을 수행하고 불가능한 경우 질적 검토를 하였다. 체계적 문헌고찰 결과는 GRADE 방법론을 이용하여 근거수준을 평가하였고, 본 평가는 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가결과 체계적 문헌고찰을 수행한 결과, 총 90편(흉부수술 15편, 복부수술 75편)의 무작위배정 비교임상시험 연구를 선정하였다. 대조군은 정맥내 주입, 정맥내 자가통증 조절법, 경막외 주입, 경막외 자가통증 조절법, 지속적 경막외 주입, 신경차단, 척수강내 주입, 위약군 및 무치료가 있었고, 대상환자는 흉부수술(개흉·흉강경)와 복부수술(개복·복강경)을 구분하고, 중재군 분류를 직접비교(단독사용)와 병합비교(병합사용)으로 구분하여 안전성 및 효과성 결과를 확인하였다. 비뚤림위험 평가 결과, ‘연구참여자, 연구자에 대한 눈가림’, ‘선택적 보고’ 및 ‘민간연구비 지원’의 비뚤림위험이 다소 높았고, 그 외 영역에서는 비뚤림위험이 낮은 것으로 판단하였다. 1) 흉부수술(개흉·흉강경) 안전성 수술 부위로의 지속적 국소마취제 투여법의 임상적 안전성을 보고한 대부분 연구에서 감염 시술 관련 합병증(시술 관련 감염 및 이상반응, 카테터 관련 문제 등) 및 약물 부작용(오심 및 구토, 가려움, 신경학적 이상반응, 국소마취제 독성 관련 등) 발생이 대조군과 유의한 차이가 없거나, 일부 연구에서는 중재군에서의 관련 합병증 및 부작용 발생이 더 적은 것으로 보고하였다. 효과성 직접비교 수술 부위로의 지속적 국소마취제 투여법과 기존 통증 조절법을 직접 비교한 대부분 연구에서 통증 점수와 수술 후 추가 진통제 소비량의 군간 유의한 차이가 없었고, 일부 연구(신경차단법과 비교)에서만 수술 부위로의 지속적 국소마취제 투여법의 통증 점수가 더 높았고 추가 진통제 소비량이 더 많았다. 병합비교 수술 부위로의 지속적 국소마취제 투여법에 기존 통증 조절법을 추가한 군(중재군)과 위약/무치료에 기존 통증 조절법을 추가한 군(대조군)의 통증 점수는 통계적으로 유의한 차이가 없었다. 2) 복부수술(개복·복강경) 안전성 수술 부위로의 지속적 국소마취제 투여법의 임상적 안전성을 보고한 대부분 연구에서 시술 관련 합병증(시술 관련 감염 및 이상반응, 카테터 관련 문제 등) 및 약물 부작용(오심 및 구토, 가려움, 신경학적 이상반응, 국소마취제 독성 관련 등) 발생이 대조군과 유의한 차이가 없거나 중재군에서 관련 합병증 및 부작용 발생이 더 적었다. 일부 연구(지표별 각 1편)에서만 대조군에서 합병증 및 부작용 발생이 더 적었다. 효과성 직접비교 수술 부위로의 지속적 국소마취제 투여법과 기존 통증 조절법을 직접 비교한 대부분의 연구에서 통증 점수와 수술 후 추가 진통제 소비량의 군간 차이에 대한 연구결과가 일관되지 않았다. 병합비교 수술 부위로의 지속적 국소마취제 투여법에 기존 통증 조절법을 추가한 군(중재군)과 위약/무치료에 기존 통증 조절법을 추가한 군(대조군)의 통증 점수는 통계적으로 유의한 차이가 없거나 중재군에서 더 낮았다. 메타분석 결과는 수술 후 1일 시점에 중재군의 통증 점수가 유의하게 더 낮았다. 수술 후 1, 2일 시점의 추가 진통제 소비량에 대한 메타분석 결과, 수술부위로의 지속적 국소마취제 투여법에 기존 통증 조절법을 추가한 병합요법군이 위약/무치료에 기존 통증 조절법을 추가한 병합요법군에 비해 추가 진통제 소비량이 통계적으로 유의하게 적었으나, 연구간 이질성이 높아 해석에 주의가 필요하다.

결론 및 제언 소위원회는 평가결과에 근거하여 수술 부위로의 지속적 국소마취제 투여법은 시술 관련 합병증 및 약물 부작용이 덜 발생하거나 다른 통증 조절법과 비교해 차이가 없어 임상적으로 안전한 의료기술로 판단하였다. 효과성에 대한 판단은 소위원회 위원 간 이견이 있었다. 직접비교 결과, 흉부수술(개흉·흉강경)에서는 관련 근거의 양이 많지 않아 해석에 주의가 필요하나 신경차단법보다는 통증 조절 효과가 떨어지고 그 외 다른 통증 조절법과는 통증 조절 효과가 유사한 것으로 판단하였고, 복부수술(개복·복강경)에서는 통증 점수 및 수술 후 추가 진통제 소비량에 있어 상반된 연구결과들이 혼재되어 있었으나 이는 이미 효과가 입증된 다른 통증 조절법들과의 우열성을 판단하기 어려운 것으로 해석할 수 있어 다른 통증 조절법과 유사한 통증 조절 효과를 가지는 의료기술로 판단하였다. 수술 부위로의 지속적 국소마취제 투여법과 기존 통증 조절법을 추가 사용한 병합비교의 효과성에 대한 판단은 소위원회 위원간 이견이 있었다. 흉부수술(개흉·흉강경)에서는 (i) 전체 연구결과에서는 최소 한 시점에서라도 기존 통증 조절법과 수술 부위로의 지속적 국소마취제투여법을 함께 사용할 때 메타분석 결과, 위약/무치료군에 비해 통증 점수와 수술 후 추가 진통제 사용량에서 수술 부위로의 지속적 국소마취제 투여법의 추가 사용으로 인한 유의한 차이가 없어 기존 통증조절법과 함께 사용하는 경우 효과성을 입증하기 어렵다는 4인의 의견과 (ii) 전체 연구결과에서는 기존 통증조절법과 수술 부위로의 지속적 국소마취제 투여법을 함께 사용할 때 위약/무치료군을 함께 사용한 군에 비해 수술 후 추가 진통제 소비량이 낮다는 연구가 더 많고, 메타분석 통합 추정치가 통계적 유의성은 없으나 효과가 좋은 경향성이 있어 수술 부위로의 지속적 국소마취제 투여법을 기존 통증조절법과 함께 사용하는 것이 효과적이라는 2인의 의견이 있었다. 복부수술(개복·복강경)에서는 (i) 메타분석에 일부 연구만 포함되어 선택 비뚤림의 가능성이 있기에 결과 해석에 주의가 필요하고, 제한적이지만 메타분석 결과에 따라 위약/무치료군에 비해 동 기술을 기존 통증조절법과 함께 사용할 때 통증 점수는 수술 후 1일 시점에서 더 낮았고, 수술 후 1일과 2일 시점의 추가 진통제 소비량이 더 낮아 개복·복강경 수술 환자에서 수술 부위로의 지속적 국소마취제 투여법을 다른 기존 통증조절법과 함께 사용하는 것이 효과적이라는 4인의 의견과 (ii) 메타분석이 아닌 전체 연구결과에서 위약/무치료군과 비교시, 유의한 차이가 없거나 수술 부위로의 지속적 국소마취제 투여법을 추가 사용하는 것이 효과적이라는 결과를 보인 연구가 혼재되어 있어 수술 부위로의 지속적 국소마취제 투여법을 다른 기존 통증조절법과 함께 사용하는 것의 효과성을 입증하기 어렵다는 2인의 의견이 있었다. 2023년 제6차 의료기술재평가위원회(2023.6.2.)에서는 소위원회 결론을 검토한 결과, ‘수술(개흉·개복술) 부위로의 지속적 국소마취제 투여법’에 대해 다음과 같이 심의 의결하고 권고등급을 결정하였다. 의료기술재평가위원회에서는 개흉·흉강경(흉부)와 개복·복강경(복부)에서 수술 부위로의 지속적 국소마취제 투여법의 효과성 차이가 크지 않은 것으로 판단하여 수술부위 구분없이 동일하게 판단하되, 단독사용과 병합사용을 구분하여 권고등급을 결정하기로 하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 흉부(개흉·흉강경) 및 복부(개복·복강경) 수술 환자에서 통증 조절을 위해 수술 부위로의 지속적 국소마취제 투여법의 단독사용을 ‘조건부 권고함’으로 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 흉부(개흉·흉강경) 및 복부(개복·복강경) 수술 환자에서 통증 조절을 위해 기존 다른 통증 조절법과 수술 부위로의 지속적 국소마취제 투여법의 병합사용을 ‘권고하지 않음’으로 심의하였다.

Background Continuous local anesthetic wound infusion (CLAWI) has emerged as an innovative pain management technique for patients who have undergone thoracotomy or laparotomy. This method is especially relevant for those who cannot utilize standard pain control methods or for whom these methods are insufficiently effective. Initially assessed as a new health technology in 2010, CLAWI was designated as a non-benefit item in 2011, a decision that was revised in 2016 when it was listed as a selective benefit item. This technology came under scrutiny through internal monitoring as part of the Ministry of Health and Welfare's review of non-benefit and selective benefit items, part of the coverage implementation plan. Subsequently, a reassessment was initiated following the agenda set during the sixth session of the Committee of Health Technology Reassessment in 2022 (on June 10, 2022).

Methods To evaluate the clinical safety and effectiveness of CLAWI, a systematic review was conducted. This review was meticulously planned and executed, with the methods and criteria being finalized after careful deliberation by the “Continuous local anesthetic wound infusion subcommittee (hereafter referred to as the “Subcommittee”) ”. The primary question guiding this assessment was “Is CLAWI clinically safe and effective for patients who have undergone thoracotomy or laparotomy?” The safety of this technology was examined by looking into the procedure-related complications and adverse drug reactions (ADRs). Effectiveness was assessed by analyzing pain scores and the quantity of additional postoperative analgesics required by patients. Moreover, considering the purpose of this assessment, the review was restricted to randomized controlled trials (RCTs).

An extensive search was carried out across three international and five domestic databases. The process of article selection involved two independent reviewers who screened and chose articles based on predefined inclusion and exclusion criteria. These reviewers also independently performed the risk of bias (RoB) assessment using the Cochrane RoB tool and achieved consensus on their findings. Data extraction was executed using a predetermined format, and any disagreements among reviewers were resolved through discussion to reach a consensus. Data analysis was primarily based on the type of control group used in the studies. A meta-analysis was carried out when quantitative analysis was feasible; otherwise, a qualitative review was performed. The results of the systematic review were then evaluated for their level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method. Finally, the Committee of Health Technology Reassessment determined the grade of recommendation based on a thorough review of the findings from the Subcommittee.

Results The systematic review included a total of 90 RCTs, which were categorized into 15 RCTs focusing on thoracotomy and thoracoscopy (chest) and 75 RCTs on laparotomy and laparoscopy (abdomen). The outcomes of safety and effectiveness for these two categories were reported separately:

1) Thoracotomy/thoracoscopy (chest)

- Safety: The majority of studies reported no significant differences in procedure-related complications and ADRs between the intervention (CLAWI) and control groups. However, some studies indicated lower rates of these complications and ADRs in the intervention group.

- Effectiveness: - Direct comparison: Most studies comparing CLAWI directly with conventional pain control methods found no significant differences in pain scores and the amount of additional postoperative analgesic use. However, some studies, particularly those comparing CLAWI to nerve block, reported higher pain scores and greater analgesic use in the intervention group. - Combined comparison: In studies comparing the intervention group (CLAWI plus conventional pain control) with the control group (conventional pain control plus placebo/no treatment), no significant differences in pain scores were generally observed. Some studies reported less additional postoperative analgesic use in the intervention group compared to the control group, but the meta-analysis did not show a significant overall difference between the two groups.

2) Laparotomy/laparoscopy (abdomen)

- Safety: Most studies indicated fewer procedure-related complications and ADRs with the intervention or found no significant difference compared to the control group. Only a minority of studies (one study for each indicator) reported fewer complications and ADRs in the control group.

- Effectiveness: - Direct comparison: The majority of studies directly comparing CLAWI with conventional pain control methods reported inconsistent results regarding differences in pain scores and the amount of additional postoperative analgesic use. - Combined comparison: In these comparisons, either no significant difference in pain scores was noted between the groups, or the intervention group had lower pain scores. Meta-analysis results showed that pain scores were significantly lower in the intervention group on postoperative day 1 (POD1). While there were more articles reporting no significant difference in the amount of additional postoperative analgesic use between the groups, meta-analysis results indicated that additional analgesic use was significantly lower in the intervention group at POD1 and POD2. However, it is important to note that there was high heterogeneity among the studies.

Conclusions and recommendations Upon evaluating the results of the assessment, the Subcommittee concluded that CLAWI qualifies as a clinically safe health technology for pain management, particularly in situations where it has shown to result in fewer or equivalent procedure-related complications and ADRs compared to other pain control methods. However, there was a division in opinion among the Subcommittee members regarding the effectiveness of CLAWI.

1) In the case of thoracotomy/thoracoscopy (chest): - The Subcommittee observed that when directly compared with other pain control methods, CLAWI was less effective than nerve block techniques. Yet, it appeared to offer a similar level of effectiveness in pain control as other standard pain control methods. However, the Subcommittee advised caution in interpreting these findings due to a lack of substantial evidence. - When assessing the effectiveness of combining CLAWI with conventional pain control methods (combined comparison), the results were inconclusive: - (i) Across the studies, a trend was noted where using CLAWI in conjunction with conventional pain control methods (at least at one time point) showed a tendency towards reduced additional postoperative analgesic use compared to placebo/no treatment combined with conventional methods. However, this observation was not consistently supported by meta-analysis results. As a result, four members of the Subcommittee believed that it was challenging to definitively prove the effectiveness of CLAWI in combination with conventional methods. - (ii) Conversely, several studies indicated that using CLAWI in combination with conventional pain control methods resulted in less additional postoperative analgesic use(at at least at one time point), but the pooled estimates in the meta-analysis did not reveal statistical significance. Two members, therefore, held the view that CLAWI could be effective when used alongside conventional methods.

2) In the case of laparotomy/laparoscopy (abdomen): - The effectiveness of CLAWI, when directly compared with other pain control methods, showed mixed results in terms of pain scores and the amount of additional postoperative analgesic use. However, the Subcommittee deemed CLAWI's effectiveness as similar to other established pain control methods since its superiority or inferiority was not definitively ascertainable. - For combined comparisons, opinions among the experts differed: - (i) Some caution and acknowledgment of limitations were advised in interpreting the results, partly due to potential selection bias in the studies included in the meta-analysis. Nonetheless, the meta-analysis indicated that the combined use of CLAWI with conventional methods resulted in lower pain scores at POD1 and less additional postoperative analgesic use at POD1 and POD2, compared to using placebo/no treatment with conventional methods. Consequently, four members of the Subcommittee opined that CLAWI, when used in combination with conventional methods, can be effective for patients undergoing laparotomy or laparoscopy. - (ii) Considering all study results, including those not part of the meta-analysis, the findings were inconsistent, showing either no significant difference or a trend toward effectiveness with the additional use of CLAWI. Two Subcommittee members therefore found it difficult to conclusively determine the effectiveness of CLAWI in conjunction with conventional pain control methods.

In the sixth session of the Committee of Health Technology Reassessment in 2023 (on June 2, 2023), the Committee evaluated these findings and arrived at specific determinations and grades of recommendation for the use of CLAWI at thoracotomy/laparotomy sites.

- The Committee decided not to distinguish between thoracotomy/thoracoscopy (chest) and laparotomy/laparoscopy (abdomen) in their grading of recommendations, based on the observation that there was no significant difference in the effectiveness of CLAWI between these procedures. However, the Committee chose to issue distinct recommendations for the use of CLAWI alone versus its combination with other methods.

- The Committee “conditionally recommends” the use of CLAWI alone for pain control in patients who have undergone thoracotomy/thoracoscopy or laparotomy/laparoscopy, tailored to the clinical situation in Korea. This recommendation was made after a comprehensive consideration of the evidence regarding CLAWI's clinical safety and effectiveness, along with other assessment items.

- On the other hand, the Committee “does not recommend” the use of CLAWI in combination with other pain control methods for controlling pain in patients who have undergone thoracotomy/thoracoscopy or laparotomy/laparoscopy. This decision was based on a thorough assessment of the evidence of CLAWI's clinical safety and effectiveness, in addition to other relevant factors.
