정신질환 및 알코올 중독질환을 가진 보호관찰대상자에 대한 지역사회 연계 치료개입에 대한 평가 연구

Abstract

정신질환과 알코올 중독은 범죄와 밀접한 관련이 있다고 보고되고 있으나 이러한 문제를 가진 보호관찰대상자에 대한 지속적이며 체계적인 치료·관리가 미흡하여 재범으로 이어지고 있는 실정이다. 이에 본 연구는 정신질환 및 알코올 중독 증상이 있는 보호관찰대상자의 범죄원인을 치료적 개념으로 접근함으로써 이들의 재범을 방지하고 건전한 사회복귀를 도모하기 위하여 지역사회와 연계한 체계적인 치료개입을 시도하였으며, 이를 통하여 보호관찰대상자의 치료에 대한 경험, 효과 및 치료지속요인을 질적인 방법으로 탐색하였다. 본 연구는 1차적으로 보호관찰대상자에 대한 심층면담 및 각종 사전검사 등을 통해 대상자를 선정하여 대상자의 동의하에 지역사회 내 치료 기관에 의뢰하였고, 의뢰받은 기관에서는 대상자에 대한 평가를 실시한 결과에 따라 치료 계획을 수립하여 외래진료, 입원치료, 사회복귀를 위한 재활프로그램 참여 등을 실시하였다. 총 29명의 보호관찰대상자가 치료에 동의하고 참여하였으며, 이 중 치료를 지속하고 있는 11명의 동의를 얻어 ‘치료의 경험’과 ‘치료의 효과’ 및 ‘치료의 지속요인’에 대하여 인터뷰 후 내용분석법(content analysis)을 이용하여 분석하였다. 연구 결과로, 알코올 중독 치료에서의 ‘치료의 경험’은 ‘새로운 삶을 살아보기로 결심 함’의 범주로 진행되며, ‘힘든 치료에 적응 되어감’ 단계로 발전한다. ‘치료의 효과’는 알코올 중독이 질환임을 알게 되는 ‘지식의 변화’, 절주/단주를 하게 되는 ‘행위의 변화’, 정신병적 증상이 감소되고, 긍정적인 변화를 느끼는 ‘심리사회적 변화’ 단계로 발전한다. ‘치료 지속요인’은 가정에 대한 책임 등과 같은 ‘긍정적 요인’과 음주에 관한 잘못된 신념 등과 같은 ‘부정적 요인’등이 나타났다. 정신질환 치료에서의 ‘치료의 경험’은 상담을 받으면 편안해짐을 느끼는 ‘치료가 필요한 나에게 마음이 편안한 곳’의 범주로 진행되며, 약물부작용을 겪으며 현실적인 도움이 더 절실해지는 ‘여전히 힘이 듬’ 단계로 발전한다. ‘치료의 효과’는 정보와 자원을 접하게 되는 ‘정보 및 지지의 획득’과 정신증상이 완화되고 살아가는데 의욕이 생기는 ‘심리사회적 변화’단계로 발전한다. ‘치료 지속요인’은 주변의 지지, 경제적 치료지원 등과 같은 ‘긍정적 요인’과 약 복용관리의 미흡 등과 같은 ‘부정적 요인’등이 나타났다. 본 연구는 정신질환 및 알코올 중독문제를 가진 보호관찰대상자에 대한 체계적인 치료 및 관리를 수행하여 그 경험과 효과성을 파악하였으며, 또한 치료 지속요인을 밝혀 추후 발전된 치료모델을 개발하는 데에 있어 그 근거를 마련하였다는 것에 그 의의가 있다.

It has been well-known fact that mental illnesses and alcohol misuse problems are highly related with crime. Indeed, the offenders with the problems of mental illnesses or alcohol addicts often involve in a recidivism. Thus, the purpose of this study was to evaluate the community-network treatment for offenders on probation who were mentally-ill or alcohol addicted. Total 29 offenders on probation participated in the treatment; however, the data were collected from 11 subjects among them through semi-structured interviews. All interviews were audio-recorded and transcribed. The interview data and the memos from the treatment centers were qualitatively analyzed by using content analytic method. The results were qualitatively described as per three categories – experience of the treatment, effect of the treatment and the factors of treatment continuity – by using the voice of the offenders with alcohol addiction as well as those with mental illnesses. In conclusion, this study was meaningful in terms of suggesting a realistic treatment model for the offenders on probation who needed a professional medical treatments. Also, this study explored not only the experiences and effectiveness of the treatment, but also examined the factors of treatment continuity which would be a strong evidence in developing further treatment model for the offenders on probation.
