- Abstract
- 목적: 상공막염이 유일한 임상소견인 신경매독 1예를 보고하고자 한다. 증례요약: 40세 여자가 한 달 전부터 우안 상공막염에 대한 치료를 하였으나 호전되지 않아 전원되었다. 안통이 지속되었고 이학적검사상 안구의 상이측 부위에 국한된 결막 부종과 충혈된 소견을 보였으며, 안저검사 및 영상학적 검사상 특이소견 관찰되지 않았다. 혈청학적 검사상 VDRL, TPHA, FTA-ABS에 양성반응을 보여 매독에 의해 발현된 상공막염 의증 하에 뇌척수액 검사를 시행하였으며신경매독으로 확진되었다. 2주간 페니실린(Penicillin G Potassium 500만 IU q 4 hours)을 정주하였고, 치료 1주일 내에 우안의 안구증상은 소실되었으며 재발은 없었다. 결론: 상공막염에 대한 적절한 치료에도 잘 반응을 보이지 않고 지속되는 경우 혈청 및 뇌척수액의 매독검사를 실시해 보아야 하며,이에 적합한 향균제의 사용 및 지속적인 경과관찰이 필요한 예를 경험하였기에 이를 보고하는 바이다.
