In countries that have adopted a, such as the UK and Australia, medicalinformation is obtained through the national public health record platform. Singapore also utilizes a similar national healthrecord platform. In Japan, which has adopted a National Health Insurance system, each patient has a personal medicinenote. Pharmacists and other medical staff can obtain patient information through this handbook. Through this study, it wasfound that community care and medication management services have been established according to social and geographicconditions in each country.
