Abstract
저자들은 수술 후 발열과 함께 복막염이 의심되는 환자에서 시행한 복막액 배양에서 세 건의 드문 Bacteroides 균종을 분리하였다. 세 균주들은 Rapid ID 32A (bioMérieux, France) 검사로 각각 B. fragilis/B. stercoris와 B. ovatus로 동정되었으나, MALDI-TOF MS와 16S rRNA 염기서열 분석을 통해 B. nordii 1예와 B. salyersiae 2예로 확인되었다. 세 균주는 항혐기성 약제인 piperacillin/tazobactam, imipenem, meropenem, chloramphenicol 및 metronidazole에 대해서 모두 감수성이었으며, 환자들은 piperacillin/tazobactam 치료 후 호전되었다. 본 증례를 통해 드문 B. fragilis군균종 동정시에 MALDI-TOF MS가 16S rRNA 염기서열 분석 결과와 일치도가 높고, 간편하고 신속하게 결과를 확인할 수 있다는 점에서 유용함을 알 수 있었다. In this study, we report three cases in which two species of the Bacteroides fragilis group, ‘Bacteroides nordii’ and ‘Bacteroides salyersiae’, were isolated from peritoneal fluid cultures from post-operative peritonitis patients. The two species of the B. fragilis group were initially misidentified as B. fragilis/Bacteroides stercoris and Bacteroides ovatus by Rapid ID 32A (bioMérieux, France), and finally confirmed as B. nordii and B. salyersiae using matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) and 16s rRNA sequencing. For the identification of anaerobes, particularly B. fragilis group organisms, MALDI-TOF MS is a useful method not only because of its concordance with 16S rRNA sequencing results, but also because of its rapidity and simple procedure.
