- Abstract
- 목적: 접안렌즈를 이용한 범망막광응고술의 합병증으로 눈꺼풀처짐이 발생할 수 있는지 알아보고자 한다.
대상과 방법: 당뇨망막병증으로 범망막광응고술 시행 예정인 환자들을 대상으로 전향적으로 시행당일, 시술 3개월 후의 눈꺼풀각막반사간거리(MRD1), 눈꺼풀올림근의 기능, 눈꺼풀틈새의 높이 및 너비, 위눈꺼풀판의 높이를 측정하였다.
결과: 전체 31안 중 시술 후 MRD1이 감소한 안은 8안(25.8%), 눈꺼풀올림근의 기능이 감소한 안은 5안(16.1%)이었고 눈꺼풀틈새의높이가 감소한 안은 6안(19.4%)이었다. 범망막광응고술 후 MRD1은 평균 0.2 ± 0.4 mm의 감소를 보였고(p=0.008) 눈꺼풀틈새의 높이는 평균 0.1 ± 0.3 mm 감소를 보였다(p=0.031). 범망막광응고술 후 MRD1의 감소량은 위눈꺼풀판의 높이가 낮은 것과 유의한 관련성을 보였다(p=0.045).
결론: 접안렌즈를 이용한 범망막광응고술의 합병증으로 눈꺼풀처짐이 발생할 수 있으며 위눈꺼풀판의 높이가 낮은 것과 관련이 있었다.
Purpose: To investigate the possibility of blepharoptosis as a complication after panretinal photocoagulation using ophthalmoscopic contact lens.
Methods: We prospectively evaluated patients who were diagnosed with diabetic retinopathy and scheduled to be treated with panretinal photocoagulation. Margin reflex distance 1 (MRD1), levator function, palpebral fissure height and width,and tarsal plate height were measured at the day of photocoagulation and 3 months after treatment.
Results: MRD1 was decreased in 8 eyes (25.8%), levator function was decreased in 5 eyes (16.1%), and palpebral fissure height was decreased in 6 eyes (19.4%). The decrement of MRD1 and palpebral fissure height after photocoagulation were significant (p = 0.008, p = 0.031, respectively). There was a significant negative correlation between MRD1 decrement and tarsal plate height (p = 0.045).
Conclusions: We identified blepharoptosis after panretinal photocoagulation using contact ophthalmoscopic lens, which was related with low tarsal plate height.
