- Abstract
- 종양괴사인자 차단제는 류마티스관절염, 강직성척추염,불응성 사르코이드증과 같은 면역 매개성 질환의 치료에효과적인 약제로 알려져 있다. 그러나 최근 류마티스 질환에서 종양괴사인자 차단제를 사용하면서 오히려 사르코이드증이 발생하는 증례가 외국에서 종종 보고되고 있으나 국내에서는 아직 보고된 적이 없었다. 이에 저자들은 강직성척추염으로 etanercept를 5년 동안 사용한 후에 우측 폐문부 림프절 종대를 포함한 사르코이드증이 새롭게 발생한 증례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
Etanercept is a soluble receptor fusion protein that inhibits tumor necrosis factor-alpha(TNF-α). The receptor is used widely in the treatment of ankylosing spondylitis,rheumatoid arthritis, sarcoidosis and other indications.
For sarcoidosis, it potentially suppresses granuloma formation with TNF-α blocking. On the other hand, recent studies have suggested that paradoxical sarcoidosis can be induced by TNF-α antagonists in some cases. A 42-yearold woman, who was treated with etanercept due to ankylosing spondylitis for 5 years, was admitted because of right suprahilar lymphadenopathy on chest radiography.
Chest computed tomography revealed an enlargement of supraclavicular, paratracheal, mediastinal lymph nodes.
She was diagnosed with sarcoidosis on the supraclavicular lymph node biopsy, which was non-caseating epithelioid cell granuloma and excluded from similar diseases. She was treated for sarcoidosis with prednisolone instead of etanercept.
