노인의 주관적 건강 인식에 따른 의료비에 관한 연구

Abstract

연구 목적: 본 연구는 노인의 주관적 건강 평가에 따른 의료비 지출 요인을 탐색하는데 있다. 연구 방법: 연구를 위해 한국의료패널 2015-2018년 자료를 활용했으며, 65세 이상 노인의 입원과 외래진료비를 이용하여 분석을 수행했다. 연구 내용: 앤더슨 모형을 기반으로, 필요요인을 자기 건강 평가지표인 삶의 질(EQ-5D)과 주관적 건강 상태로 대체하여 노인 의료비에 영향을 미치는 요인에 대해 분석했다. 결론 및 제언: 입원진료비에 영향을 미치는 요인에는 의료수급여부와 함께 자기관리, 통증 및 불편, 주관적 건강 상태였으며, 외래진료비에 영향을 미치는 요인은 노인의 연령과, 교육수준, 의료수급여부, 경제활동, 가구소득, 운동능력, 자기관리, 통증 및 불편, 주관적 건강 상태가 유의하게 영향으로 나타났다. 그러므로 노인 보건의료서비스 수립을 위해 먼저 주관적 건강 인식에 따른 의료 욕구가 반영되어야 하며, 노인의 연령차와 소득격차로 인해 차이가 발생하는 의료 필요도 함께 반영되어야 할 것이다.

This study aimed to analyze the elderly medical expenditure according to the perceived health status of the elderly. For this purpose, the Korea Health Panel 2015-2018 data was used. This study was based on the Anderson model, but the need factors were replaced with self-health evaluation indicators: quality of life (EQ-5D) and perceived health status. As a result of the study, factors affecting inpatient treatment cost were self-care, pain/discomfort, and perceived health status, along with the availability of medical care. Factors influencing outpatient medical expenses included the elderly’s age, education level, medical care availability, economic activity, household income, mobility, self-care, pain/discomfort, and perceived health status. Therefore, in order to establish health care services for the elderly, medical needs according to perceived health status should be considered, and medical needs that differ due to the age difference and income gap of the elderly should also be reflected.
