앤더슨 모형을 이용한 노인 의료비 지출 분석

Abstract

연구 목적: 본 연구는 성장혼합모형을 이용하여 노인 의료비 지출 궤적을 탐색하고자 하였다. 연구 방법: 연구를 위해 한국의료패널 2015-2015년 자료를 활용했으며, 연구의 대상인 65세 이상 노인은 총 3,810명이었다. 연구를 위해 R 4.2.1과 R Studio, Mplus 7.11을 활용하였다. 연구 내용: 성장혼합모형을 통해 잠재계층 탐색했고, 구분된 잠재계층에 영향을 미치는 요인을 확인하기 위해 앤더슨 모형을 적용하여 다항로지스틱 분석을 실시하였다. 결론 및 제언: 분석을 통해 4개의 하위집단을 확인할 수 있었다. 대부분의 노인들은 저수준 의료비 집단에 속해 있었다. 다항로지스틱 분석을 실시한 결과, 의료수급여부와 경제활동여부가 저수준 의료비 집단에 속할 확률을 높이는 반면, 만성질환은 상대적으로 의료비를 더 많이 지출하는 집단에 속할 확률이 높게 나타났다. 그러므로, 경제활동이 거의 없는 노인들의 의료비 지출 수준에 대한 고민이 필요하며, 노인의 소득수준과 만성질환 외에 노인의 의료비 지출 수준을 결정하는 요인을 더 탐색하여 적정한 노인 의료서비스를 수립해야 할 것이다.

This study, using growth mixture model, explored the latent classes of elderly medical expenses using the 2015-2018 data from the Korea Health Panel (KHP). Multi-nominal logistic analysis as well as descriptive statistics were utilized to explore influencing factors on latent classes. Influencing factors were classified into three factors of Andersen model. As a result of the analysis, the multi-year trajectories of elderly medical expenses were classified into four subgroups. Most of the elderly belonged to Low group. The medical care recipients were more likely to belong to Low group, and the chronic patients were more likely to belong to the group that spent more medical expenses than the Low group. Based on the results, it is necessary to consider the level of medical expenses of the elderly, who have little economic activity. In addition to the income level and chronic diseases of the elderly, should be further explored to establish appropriate medical services for the elderly.
