Abstract
목적: 무증상안의 형광안저촬영에서 발견되는 주변부 형광누출을 조사하고 영향을 줄 수 있는 관련인자를 분석해 보았다. 대상과 방법: 47명 94안을 대상으로 후향적 연구를 시행하였다. 형광안저촬영에서 주변부의 형광누출여부와 발생 위치를 조사하고,팔망막순환시간, 정맥만충시간, 굴절이상, 연령, 고혈압과 당뇨의 관련성을 분석하였다. 결과: 47명 중 10명(11안)에서 주변부형광누출이 관찰되었다. 비누출군의 평균연령은 34.7 ± 7.86세이고 누출군의 평균연령은 44.3± 9.63세였으며 통계적으로 유의한 차이가 있었다(p=0.001). 평균 구면렌즈 대응치는 비누출군 -2.85 ± 2.71디옵터, 누출군 -3.46± 3.62디옵터로 유의하지 않았다(p=0.471). 평균 팔망막순환시간은 비누출군에서 10.50 ± 2.06초, 누출군에서 11.76 ± 2.47초로통계적으로 유의한 차이가 있었다(p=0.041). 평균 정맥만충시간은 비누출군에서 9.70 ± 1.91초, 누출군에서 10.75 ± 1.40초로 통계적으로 유의한 차이가 있었다(p=0.048). 결론: 무증상안에서도 다른 안과적 이상 없이 주변부 형광누출이 관찰될 수 있음을 확인하였다. 연령과 정맥만충시간, 팔망막순환시간이 형광누출과 밀접한 관련성이 있으며 이는 주변부망막의 조직학적인 특징과 생리학적 변화와 관련이 있을 것으로 생각한다.
Purpose: To report peripheral vascular retinal leakage findings of asymptomatic eyes based on fluorescein angiography, and investigatethe associated factors. Methods: Data were collected retrospectively from 47 subjects (94 eyes) and the peripheral leakage results based on fluoresceinangiography were analyzed. The relationship between peripheral leakage findings and other factors including- arm-retinal circulationtime (ARCT) and venous filling time (VFT), refractive error, age, hypertension, and diabetes- was evaluated. Results: Ten eyes had peripheral leakage (21.3%). The mean age was 34.7 ± 7.86 years in the non-leakage group and 44.3 ±9.63 years in the leakage group; the difference between the groups was statistically significant (p = 0.001). The mean sphericalequivalent was -2.85 ± 2.71 diopter in the non-leakage group and -3.46 ± 3.62 diopter in the leakage group; the difference betweenthe groups were not significant (p = 0.471). The mean ARCT was 10.50 ± 2.06 seconds in the non-leakage group and11.76 ± 2.47 seconds in the leakage group; the difference between the groups was statistically significant (p = 0.041). Conclusions: Peripheral leakage can be found in asymptomatic eyes. Age, VFT, and ARCT were correlated to peripheral leakagefindings based on angiography. These leakage findings were thought to be related with histological properties and physiologicalchanges in peripheral retina.
