경피적 고주파 열치료술-신장암

Abstract

평가 배경 신장암의 고주파 열치료술은 신장암 환자에서 고주파를 이용하여 조직을 절개 및 응고시킴으로써 신장기능의 저하를 최소화하면서 환자의 생존율 및 삶의 질을 향상시키기 위한 목적으로 실시하는 기술이다. 동 기술은 접근방법별로 개복술하, 복강경하, 경피적 3가지 방법이 신의료기술평가제도가 확립되기 전 비급여로 등재된 후 2011년 행위급여로 전환되어 현재까지 이용되고 있다. 동 기술은 2019년 유관기관 수요조사 접수 및 2022년 내부 모니터링을 통해 재평가 안건으로 발굴되었으며, 2022년 제8차 의료기술재평가위원회(2022.8.12.)에서 전문적·심층적 검토를 통해 동 기술의 임상적 안전성 및 효과성 평가가 필요한 안건으로 심의하였다. 이에 본 평가는 체계적 문헌고찰을 통해 신장암의 고주파 열치료술의 임상적 안전성 및 효과성의 근거를 확인하여 합리적 의료자원 활용을 지원하고자 수행하였다.

평가 방법 동 기술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “신장암의 고주파 열치료술 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 비뇨의학과 2인, 영상의학과 2인, 신장내과 1인, 종양내과 1인, 근거기반의학 1인의 전문가 7인으로 구성하였다. 평가의 핵심질문은 “신장암 환자에서 경피적 고주파 열치료술은 안전하고 효과적인가?”이며 안전성은 합병증 및 이상반응 지표로, 효과성은 일차지표로 종양학적 결과, 신기능 변화, 환자만족도 또는 삶의 질, 이차지표로 소요시간, 혈액소실 정도, 재원기간을 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 선택된 문헌은 1편의 무작위배정비교임상시험과 39편의 후향적 코호트연구로 비뚤림 위험 평가는 각각 Cochrane의 Risk of Bias (RoB)와 Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS) 2.0을 이용하여 평가하였다. 자료분석은 양적분석이 가능한 부분에 대하여 메타분석을 수행하였고, 불가능한 영역에 대하여 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 신장암의 경피적 고주파 열치료술 평가에 포함된 문헌은 무작위배정비교임상시험연구 1편과 후향적 코호트연구 39편, 비용-효용 분석 2편이었다. 비교기술은 극초단파 열치료술, 냉동절제술, 신장 부분절제술, 신장 전절제술이 포함되었다. 안전성 안전성은 총 31편의 연구에서 제시된 합병증 및 이상반응 여부를 통해 평가하였고, 극초단파 열치료술과 비교한 연구는 6편, 냉동절제술과 비교한 연구는 16편, 신장 부분절제술과 비교한 연구는 14편, 전절제술과 비교한 연구는 4편이었다. 보고된 합병증의 내용으로는 양 군 모두에서 혈종이 가장 많았고, 요로상피손상, 혈뇨, 통증, 출혈 등이 포함되었다. 각 비교기술 별 메타분석 결과, 극초단파 열치료술과 비교했을 때 전체 합병증의 발생 위험비(Risk Ratio, RR)는 1.50 (95% CI 1.03~2.18, I2=0%)으로 중재군의 발생률이 유의하게 더 높았고, 전절제술과 비교한 1편의 무작위배정임상시험연구에서는 전체 합병증의 RR이 0.34(95% CI 0.12~0.99)로 중재군의 위험도가 유의하게 낮은 것으로 보고하였다. 나머지 영역들에서는 모두 군간 유의한 차이가 없는 것으로 제시되었다. 효과성 효과성은 일차지표로 종양학적 결과, 신기능 변화 여부, 환자만족도(또는 삶의 질), 이차지표로 시술/수술 소요시간, 혈액소실량, 재원기간, 재입원율을 살펴보기로 하였다. 비교기술 별 주요 결과는 다음과 같다. 극초단파 열치료술과 비교한 연구들 중 첫 번째 지표인 종양학적 결과를 보고한 4편에서 잔존, 재발, 전이에서 군간 유의한 차이가 없었다. 생존 여부에서는 1편에서 추적관찰기간 동안 전체 생존율이 중재군 84.7%, 비교군 60.0%로 중재군에서 더 높았고, 질병특이 생존율, 암특이 생존율, 무진행 생존율, 무병 생존율 등에서는 군간 유사한 수준의 결과를 나타냈다. 신기능 변화와 관련하여 3편 중 2편에서 중재군의 사구체여과율 감소정도가 더 작았고, 다른 1편은 군간 차이를 보이지 않았다. 1편에서 제시한 크레아티닌 상승률은 중재군에서 더 낮았으며, 모든 군에서 만성 신장질환 악화사례는 없었던 것으로 보고하였다. 이차지표인 시술 소요시간은 1편은 군간 유사하였고, 다른 1편은 비교군에서 더 단축됨을 보고하였다. 냉동제거술과 비교한 연구들 중 종양학적 결과를 보고한 16편의 연구에서 잔존을 다루지 않았고, 재발 및 전이, 생존에서 군간 유사한 추이를 보였으나, 1편에서 단일신장 환자를 대상으로 2년 시점 무병 생존율은 중재군 33.2%, 비교군 69.6%의 결과를 제시하였다. 신기능 변화를 다룬 11편 중 1편에서 1년 후 만성 신질환 발생률이 중재군 66.7%, 비교군 15.4%인 것으로 나타났으나 동일 연구에서 단계 악화율은 군간 유의한 차이가 없었다. 그 외 다른 지표들에서는 모두 군간 유사한 수준을 보고하였다. 전체 연구 중 1편에서 환자만족도 및 타인에 시술 권유 의사 여부에 대해 중재군과 비교군 모두 유사한 수준의 높은 점수를 부여하였다. 이차지표 중 시술 소요시간은 1편에서 중재군에서 단축 효과를 보였고, 다른 3편에서는 군간 유사하였다. 혈액소실 정도와 재원기간을 보고한 1편도 모두 군간 유사한 수준을 보고하였다. 신장 부분절제술과의 비교한 종양학적 결과는 19편에서 보고하였고, 잔존, 재발, 전이에서 군간 유의한 차이를 보이지 않았다. 전체생존율, 암특이 생존율, 무재발 생존율은 비교군에서 더 높은 경향성을 확인할 수 있었고, 무전이 생존율 및 무병 생존율은 유사한 수준이었다. 신기능 변화를 다룬 연구들 중 사구체여과율 변화 정도는 7편에서 중재군에 더 양호한 결과를 보고하였고, 다른 6편에서는 군간 유의한 차이가 없었다. 나머지 1편에서는 CKD 2단계에 해당하는 대상자들에서는 군간 유의한 차이가 없었고, CKD 3단계 이상 대상자들의 경우, 중재군의 기저값이 더 낮았고, 3년간 이 추이가 지속되었다가 3년 시점에서 중재군의 악화정도가 유의하게 더 큰 것으로 나타났다. 시술 또는 수술 소요시간은 3편에서 모두 중재군에서 단축됨을 보고하였고, 혈액소실 정도, 재원기간도 중재군에 더 유의한 효과를 보고하거나 유사한 수준이었다. 신장 전절제술과 비교한 연구는 무작위배정비교임상시험연구(RCT) 1편과 후향적 코호트연구 4편이 포함되었다. RCT 연구에서는 전체 생존율에서 군간 유의한 차이가 없음을 보고하였고, 수술 또는 시술 소요시간, 혈액소실 정도, 재원기간에서는 중재군에서 유의한 효과를 제시하였다. 후향적 코호트연구에서 종양학적 결과는 재발 및 전이에서 유의한 차이가 없었고, 1편에서 보고한 전체 생존율은 비교군에서 더 높았으나, 암특이 생존율은 모두 100%였고, 무병 생존율은 중재군에서 더 높은 경향성을 보였다. 신기능 변화에서 사구체여과율은 3편 중 2편에서 중재군에 더 유의한 결과를 보고하였고, 나머지 1편은 군 내 그리고 군 간 차이가 없는 것으로 나타났다. 혈청 크레아티닌 변화를 제시한 1편에서 군 내, 군 간 차이가 없었다. 1편에서 보고한 만성 신장질환 악화율은 3년 시점 중재군 2.0%, 비교군 58.3%로 중재군에 양호한 결과를 나타냈다. 전절제술을 다룬 연구 중 1편에서 8개 영역에 대한 삶의 질 설문을 수행하였고, 중재군은 4개 영역에서 기저값이 기준치보다 낮았다가 시술 후 1주 시점부터 상승하여 이후 비교군과 통계적으로 유의한 차이가 없는 수준까지 향상된 것으로 보고하였다. 수술 또는 시술 소요시간 및 혈액소실 정도는 1편에서 군간 유사한 것으로 나타났다. 경제성 총 4편의 연구에서 해당 의료기술의 경제성과 관련된 결과가 보고되었다. 마콥 모형을 이용하여 비용-효용 분석을 수행한 연구는 2편으로, 캐나다 보건의료시스템에서 신장암의 경피적 고주파 열치료술보다 능동감시 및 냉동절제술이 더 비용 효과적인 대안일 수 있다는 연구와 미국 보건의료시스템에서 수술적 치료가 비용-효과적이지 않아 작은 신세포암의 치료에서 경피적 고주파 열치료술을 우선 고려할 것을 권고하는 결과가 각각 제시되었다. 진료데이터를 기반으로 총 소요비용을 제시한 2편의 후향적 코호트연구에서는 경피적 고주파 열치료술이 수술적 치료에 비해 더 저렴한 비용으로 수행할 수 있는 효과적인 기술이라고 제시하였다.

결론 및 제언 신장암의 경피적 고주파 열치료술은 1편의 무작위배정비교임상시험연구(근거의 신뢰수준, Mode- rate)에서 신장 전절제술 대비 유의하게 낮은 합병증 발생률을 보고하였고, 종양학적 결과는 유사하였으며, 이차지표인 수술 소요시간, 혈액소실 정도, 재원기간에서 유의한 감소를 보였다. 극초단파 열치료술, 냉동제거술, 부분절제술과의 비교는 후향적 코호트연구 39편(근거의 신뢰수준, Very Low)을 통해 평가하였다. 신장암의 경피적 고주파 열치료술은 국소 치료법(극초단파 열치료술, 냉동제거술)과 비교 시 종양학적 결과, 신기능 결과, 이차지표(수술 소요시간, 혈액소실 정도, 재원기간)에서 전체적으로 군간 유사한 수준이었고, 부분절제술과는 잔존, 재발, 전이에서는 유의한 차이가 없었으나 생존율이 부분절제술에서 더 높은 경향성을 보였고, 자료의 비뚤림을 고려할 때 열등할 가능성이 있었으며, 신기능 결과와 이차지표에서는 중재군이 좀 더 높거나 군간 유의한 차이가 없었다. 전절제술과 비교 시 종양학적 결과 및 신기능 결과 등 효과성은 군간 유사한 수준이었다. 이에 소위원회에서는 신장암의 국소치료로 사용되는 신장암의 경피적 고주파 열치료술은 표준 수술치료인 전절제술과 비교한 RCT에서 종양학적 결과가 유사하였고, 후향적 코호트 연구들에서 부분절제술보다는 종양학적 생존율이 떨어지는 경향성을 보였으나 다른 국소 치료법과는 효과가 유사하다는 근거를 바탕으로 선택문헌의 주 연구대상자인 ‘4cm 미만의 작은 신장암’ 환자에서 동 기술을 효과적인 의료기술로 평가하였다. 2023년 제8차 의료기술재평가위원회(2023.08.11.)에서는 소위원회 검토결과에 대해 논의하였고, 의료기술재평가사업 관리지침 제4조제10항에 의거 “신장암의 경피적 고주파 열치료술”에 대해 다음과 같이 심의하였다. 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 수술적 절제술의 적응증이 되지 않는 환자에서 신장암의 경피적 고주파 열치료술을 ‘조건부 권고함’으로 심의하였다(권고등급: 조건부 권고함).

Percutaneous Radiofrequency Ablation for Renal Cancer

Background Radiofrequency ablation (RFA) for renal cancer is used to excise and coagulate tissue using radiofrequency to increase patient survival rates and quality of life while minimizing kidney function deterioration. This technology, applied using three approaches (open, laparoscopic, and percutaneous surgeries), had been listed as a non-reimbursable practice prior to the implementation of the new health technology assessment system and was subsequently converted to a reimbursable procedure in 2011.

The technology was identified as a candidate for re-assessment through a 2019 needs assessment survey by relevant agencies and internal monitoring in 2022. Subsequently, the eighth 2022 Health Technology Reassessment Committee (August 12, 2022) determined the need for an in-depth review of its clinical safety and effectiveness. Therefore, the aim of this assessment was to review the evidence on the clinical safety and effectiveness of RFA for renal cancer to support rational healthcare resource utilization.

Method The safety and effectiveness of the technique were assessed through a systematic review. All assessment methods were finalized after discussions with the “Subcommittee for RFA for renal cancer” (hereinafter “Subcommittee”) considering the assessment objective. The subcommittee consisted of seven experts, including two urologists, two radiologists, one nephrologist, one oncologist, and one evidence-based medicine expert.

The key question for the assessment was “Is Percutaneous RFA safe and effective for patients with renal cancer?” Safety was assessed based on complications and adverse events and effectiveness was assessed based on oncological outcomes, changes in renal function, and patient satisfaction or quality of life as primary outcomes and procedure duration, blood loss, and length of stay (LOS) as secondary outcomes.

Literature searches were conducted using three international and five Korean databases based on the key question. Two reviewers independently screened and selected the literature according to predetermined inclusion and exclusion criteria. Risk of bias was assessed using Cochrane's Risk of Bias tool for randomized controlled trials (RCTs) and the Risk of Bias Assessment Tool for Nonrandomized Studies 2.0 for cohort studies. Quantitative analyses were performed when meta-analysis was feasible and qualitative review was used otherwise. The level of evidence of the systematic review results was assessed using the Grading of Recommendations Assessment, Development, and Evaluation approach.

Results One RCT, 39 retrospective cohort studies, and two cost-effectiveness analyses were included in the assessment of percutaneous RFA for renal cancer. The comparative technologies included microwave ablation, cryoablation, partial nephrectomy, and radical nephrectomy.

Safety Safety was assessed based on the presence of complications and adverse events reported in 31 studies. Comparisons were made with microwave ablation (six studies), cryoablation (16 studies), partial nephrectomy (14 studies), and radical nephrectomy (four studies). The most common complication in both groups was hematomas, and other complications included urothelial damage, hematuria, pain, and bleeding.

Meta-analyses showed that the risk ratio (RR) for overall complications was 1.50 (95% CI 1.03–2.18, I²=0%) compared with that of microwave ablation, indicating a significantly higher incidence in the RFA group. In contrast, one RCT comparing RFA to radical nephrectomy reported an RR of 0.34 (95% CI 0.12–0.99) for overall complications, indicating a significantly lower risk in the RFA group. No significant differences were observed between the RFA group and other comparison groups.

Effectiveness Effectiveness was assessed based on oncological outcomes, changes in renal function, and patient satisfaction (or quality of life) as primary outcomes and procedural/surgical duration, blood loss, LOS, and readmission rates as secondary outcomes. The key findings for each comparative technology are as follows.

Among studies comparing percutaneous RFA with microwave ablation, four reported oncological outcomes, including residual tumors, recurrence, and metastasis. These studies showed no significant differences between the groups. One study reported an overall survival rate of 84.7% in the intervention group compared with 60.0% in the comparison group, with higher overall survival in the intervention group during the follow-up. Disease-specific survival, cancer-specific survival, progression-free survival, and disease-free survival were similar between the groups. Regarding changes in renal function, two of three studies reported a smaller decrease in the glomerular filtration rate (GFR) in the intervention group than in the comparison group, whereas the other study showed no difference between the groups. The rate of creatinine level increase (one study) was lower in the intervention group than in the comparison group, with no cases of exacerbation of chronic kidney disease reported in any group. Procedure duration, a secondary outcome, was similar in one study and shorter in the comparison group than in the RFA group in another study.

Of 16 studies comparing RFA with cryoablation, none addressed residual tumors. The studies generally showed similar trends in recurrence, metastasis, and survival between the groups. However, one study reported a 2-year disease-free survival rate of 33.2% in the intervention group and 69.6% in the comparison group for patients with a solitary kidney. Of 11 studies addressing changes in renal function, one study reported that the incidence of CKD was 66.7% in the intervention group and 15.4% in the comparison group 1 year after treatment, but the same study reported no significant difference in the rate of progression. Other outcomes were similar between the groups. One study reported comparable levels (high scores) of patient satisfaction and willingness to recommend the procedure to others in both groups. Among secondary outcomes, procedure duration was shorter in the intervention group than in the comparison group in one study but similar between the groups in three other studies. One study reported comparable blood loss and LOS between the groups.

Nineteen studies compared RFA with partial nephrectomy and reported oncological outcomes. There were no significant differences in residual tumors, recurrence, and metastasis between the groups. Overall survival, cancer-specific survival, and recurrence-free survival tended to be higher in the comparison group than in the RFA group, whereas progression-free survival and disease-free survival were similar. Seven studies examining changes in renal function showed better GFR preservation in the intervention group than in the comparison group, whereas six reported no significant differences between the groups. One study reported that there was no significant difference between the groups among patients with stage 2 chronic kidney disease (CKD). However, among patients with stage 3 or higher CKD, the intervention group had a lower baseline GFR than the comparison group; this continued for 3 years, with significantly greater exacerbation at 3 years. Procedure or surgery duration was shorter in the intervention group than in the comparison group, and blood loss and LOS were significantly better in the intervention group than in the comparison group or similar between the groups in three studies.

One RCT and four retrospective cohort studies compared RFA with radical nephrectomy. The RCT reported no significant difference in overall survival between the groups, with significant benefits for the intervention group in terms of procedure or surgery duration, blood loss, and LOS. Retrospective cohort studies showed no significant differences in recurrence and metastasis between the groups. One study reported higher overall survival in the comparison group than in the intervention group, but cancer-specific survival was 100% in all groups. Furthermore, disease-free survival tended to be higher in the intervention group than in the comparison group. Two of three studies reported better GFR preservation in the intervention group than in the comparison group, whereas one showed no significant difference within and between the groups. One study reported no significant differences in serum creatinine level changes within and between the groups. A study that examined the rate of CKD exacerbation showed a 3-year incidence of 2.0% in the intervention group and 58.3% in the comparison group, showing better outcomes in the intervention group than in the comparison group. One study that compared RFA with total nephrectomy to assess quality of life across eight domains reported that the scores in the intervention group improved from below baseline in four domains to levels comparable to those in the comparison group starting 1-week post-treatment. Procedure or surgery duration and blood loss (one study) were similar between the groups.

Cost-effectiveness Four studies reported the cost-effectiveness of percutaneous RFA for renal cancer.

Two studies conducted cost-utility analyses using a Markov model. One study suggested that in the Canadian healthcare system, active surveillance and cryoablation were more cost-effective alternatives than RFA for renal cancer. Another study in the United States recommended considering RFA as a primary treatment option for small renal cell carcinomas as surgical treatments are not cost-effective for this condition in the US healthcare system. Additionally, two retrospective cohort studies presented total cost based on healthcare utilization data. These studies suggested that percutaneous RFA was an effective and economical technology that was more cost-effective than surgical treatment.

Conclusion and Recommendations One RCT (level of evidence: moderate) reported that percutaneous RFA for renal cancer has a significantly lower complication rate than radical nephrectomy, similar oncological outcomes, and significantly better secondary outcomes (surgical duration, blood loss, and LOS).

The technology was compared with microwave ablation, cryoablation, and partial nephrectomy based on 39 retrospective cohort studies (level of evidence: very low). Overall, the oncological outcomes, renal function results, and secondary indicators were comparable between percutaneous RFA and other local treatments (microwave ablation and cryoablation). However, on comparing RFA with partial nephrectomy, no significant differences were found in residual tumor presence, recurrence, or metastasis, although survival rates tended to be higher in the partial nephrectomy group than in the RFA group. Considering the potential bias in the data, RFA might be inferior to partial nephrectomy. In terms of renal function and secondary outcomes, percutaneous RFA showed slightly better or comparable results. However, effectiveness outcomes, such as oncological outcomes and renal functional changes, were similar between percutaneous RFA and total nephrectomy groups.

The subcommittee concluded that percutaneous RFA is effective in “patients with small renal tumors (less than 4 cm)” (the primary patient population of the selected studies) based on evidence that showed oncological outcomes similar to those of radical nephrectomy in RCTs and effectiveness comparable to that of other local treatments in retrospective cohort studies, although with a tendency for lower survival rates than those associated with partial nephrectomy.

Based on the subcommittee results and Article 4–10 of the Health Technology Re-Assessment Project Management Guidelines, the eighth 2023 Health Technology Re-Assessment Committee drew the following conclusions for “Percutaneous RFA for renal cancer” (August 11, 2023):

Based on the evidence on clinical safety and effectiveness and other evaluation criteria, percutaneous RFA for renal cancer is conditionally recommended in patients who do not meet the indications for surgical resection in the Korean clinical setting (Grade of recommendation: Conditional).
