요통 환자에서의 경피적 전기신경자극치료

Abstract

평가배경 경피적 전기신경자극치료(Transcutaneous electrical nerve stimulation, TENS)는 신경 및 근골격계의 급성과 만성통증 완화를 위해 전극을 통증부위 등에 부착하고 경피적으로 신경계를 자극하여 통증을 감소시키는 기술이다. 신의료기술평가제도 도입 이전에 급여로 등재된 의료기술로, 내부 모니터링을 통해 재평가 안건으로 발굴되어 의료기술재평가위원회 우선순위 심의를 거쳐 재평가 대상으로 선정되었다. 2023년 제4차 의료기술재평가위원회(2023.4.14.)에서 평가계획서를 심의 받았으며, 요통 환자에서 경피적 전기신경자극치료의 임상적 안전성 및 효과성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하고자 하였다.

평가방법 요통 환자에서 경피적 전기신경자극치료에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “요통 환자에서의 경피적 전기신경자극치료, 간섭파 전류치료 통합소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 재활의학과 2인, 마취통증의학과 2인, 정형외과 1인, 신경외과 1인, 근거기반의학과 1인, 총 7인으로 구성하였다. 평가의 핵심질문은 “요통 환자에서 경피적 전기신경자극치료는 임상적으로 안전하고 효과적인가?”였으며 안전성은 시술 관련 부작용 및 이상반응을 결과지표로, 효과성은 통증점수, 기능장애, 삶의 질을 결과지표로 확인하였다. 평가결과는 급성과 만성요통 환자를 구분하여 확인하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 수행하고, 의견 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 이용하여 평가하였다. 자료추출은 미리 정해놓은 양식을 활용하여 두 명의 검토자가 독립적으로 수행하고 의견 불일치가 있을 경우 평가자간 합의를 통하여 확정하였다. 자료분석은 양적 분석이 가능한 부분에 대하여 메타분석을 수행하였고, 불가능한 경우 질적 검토결과를 제시하였다. 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하였고, 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가결과 체계적 문헌고찰 결과, 총 14편(만성요통 환자 12편, 급성요통 환자 2편)의 무작위배정 비교임상시험연구(Randomized Controlled Trial, RCT)를 최종 선정하였다. 중재군인 경피적 전기신경자극치료는 고주파, 저주파, 혼합형 등 문헌에서 다양한 유형으로 사용되고 있었으며, 비교기술은 거짓치료 및 무치료, 보존적 치료(운동, 한랭치료, 초음파치료)로 구분하여 경피적 전기신경자극치료의 안전성 및 효과성 결과를 확인하였다. 비뚤림위험은 ‘무작위 배정순서 생성’, ‘배정순서 은폐’, ‘불충분한 결과자료’, ‘선택적 보고’ 영역에서 대체로 낮았으나, ‘연구참여자 및 연구자에 대한 눈가림’, ’평가자의 눈가림‘ 및 ’결과평가 눈가림’에서 비뚤림위험이 다소 높거나 불확실한 수준이었다.

안전성 만성요통 환자에서 경피적 전기신경자극치료의 안전성 결과를 보고한 문헌은 12편 중 2편이었다. 경피적 전기신경자극치료는 거짓치료 및 무치료와 비교 시 두 군 모두에서 시술 후 피부자극이 보고되었으나 일시적이고 경미한 수준이었고, 운동과 비교 시 관련 부작용 및 이상반응이 발생하지 않았다. 급성요통 환자를 대상으로 경피적 전기신경자극치료와 관련된 부작용 및 이상반응을 보고한 문헌은 없었다.

효과성 만성요통 환자에서 경피적 전기신경자극치료의 효과성은 12편 모든 연구에서 보고하였다. 효과성 결과지표는 통증점수, 기능장애, 삶의 질로 확인하였다. 경피적 전기신경자극치료의 통증점수를 보고한 연구는 총 12편이었고, 이 중 거짓치료 및 무치료군과 비교한 연구는 9편, 보존적 치료와 비교한 연구는 4편(다중비교 연구 1편 포함)이었다. 메타분석이 가능한 연구를 합성한 결과, 통증점수에 있어 경피적 전기신경자극치료와 거짓치료 및 무치료군, 보존적 치료 모두 통계적으로 유의한 차이가 없었으나 이질성이 높아 해석에 주의가 필요하다(표준화된 평균차(Standardized Mean Difference, SMD) –0.34, 95% 신뢰구간(Confidence interval, CI) –0.70~0.02, I²=57%; SMD 0.19, 95% CI –0.87~1.25, I²=88%). 또한 문헌마다 중재의 특성이 상이하여 측정시점별, 단독 및 병용 여부, 시술 횟수를 기준으로 하위군 분석결과, 거짓치료 및 무치료군과 비교한 일부 문헌에서 시술 직후 및 1~2주 시점에서 통증점수가 낮은 경향을 보였으나 통계적으로 유의한 차이는 없었다. 기능장애를 보고한 연구는 총 9편이었고, 이 중 거짓치료 및 무치료군과 비교한 연구는 7편, 보존적 치료와 비교한 연구는 3편(다중비교 연구 1편 포함)이었다. 메타분석 결과, 기능장애에 있어 거짓치료 및 무치료군, 보존적 치료 모두 통계적으로 유의한 차이는 없었으나 이질성이 높아 해석에 주의가 필요하고(SMD –0.00, 95% CI –0.23~0.23, I²=0%); SMD 0.68, 95% CI –0.98~2.35,I²= 94%), 하위군 분석결과 역시 모두 통계적으로 유의한 차이가 없었다. 삶의 질은 3편에서 SF-36 지표로 결과를 보고하였다. 거짓치료 및 무치료군과 비교한 연구는 2편이었으며, 두 군 모두 시술 전 대비 시술 후 삶의 질이 유의하게 개선되었으나, 군간 유의한 차이는 나타나지 않았고, 보존적 치료(한랭치료)와 비교한 연구 1편에서도 군간 유의한 차이가 없었다. 급성요통 환자에서 경피적 전기신경자극치료의 효과성을 보고한 문헌은 2편이었다. 통증점수는 2편 모두 거짓치료 및 무치료군과 비교하였고, 1편에서는 시술 직후 시점에서 경피적 전기신경자극치료군이 비교군보다 통증점수가 유의하게 낮았고, 다른 1편에서는 두 군의 시술 전 대비 시술 후 통증점수가 모두 유의하게 낮았으나 군간 유의한 차이는 없었다. 기능장애도 경피적 전기신경자극치료는 거짓치료 및 무치료군과 비교한 1편에서 군간 유의한 차이는 없었다.

결론 및 제언 소위원회는 현재가지 확인된 근거를 바탕으로 요통 환자를 대상으로 하는 경피적 전기신경자극치료에 대해 다음과 같이 제언하였다. 만성요통 환자에서 경피적 전기신경자극치료는 피부에 전극을 부착하여 전기자극을 전달하는 비침습적인 기술로, 문헌상 보고된 부작용 및 이상반응도 일시적이고 경미하여 안전한 기술로 판단하였다. 효과성은 전체 분석 및 하위군 분석 결과에서 경피적 전기신경자극치료가 거짓치료 및 무치료 또는 보존적 치료와 통증점수, 기능장애, 삶의 질에 있어 유의한 차이가 없었다. 다만 동 기술은 통증완화를 목적으로 오래전부터 보편적으로 사용되고 있는 의료기술이며, 거짓치료 및 무치료군과 비교한 일부 연구(3편)에서 중재 직후 및 1~2주 시점의 통증점수가 유의하게 낮은 것으로 보아, 만성요통 환자에서 단기적 통증 경감이 필요할 때 임상적으로 고려해 볼 수 있는 기술이라는 의견이었다. 급성요통 환자에서 경피적 전기신경자극치료 관련 부작용 및 이상반응을 보고한 문헌은 없었으나, 만성요통 환자에서 안전성 결과와 달라질 개연성은 크지 않을 것으로 생각되어 임상적으로 안전한 기술로 판단하였다. 효과성은 통증점수에 대한 연구가 2편으로, 연구결과가 일관적이지 않고 효과성을 판단하기에 근거가 충분하지 않아 추가 연구가 필요하다는 의견이었다. 2024년 제1차 의료기술재평가위원회(2024.1.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “경피적 전기신경자극치료”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목을 종합적으로 고려하였을 때, 만성요통 환자를 대상으로 통증 완화 목적으로 사용하는 경피적 전기신경자극치료는 안전한 기술로 판단되며 오래전부터 보편적으로 사용되어 온 기술이긴 하나, 현재 근거기반 측면에서 낮은 수준임을 고려하여 ‘조건부 권고함’으로 심의하였고, 급성요통 환자 대상으로는 만성요통 환자에서 동 기술의 안전성과 달라질 개연성은 크지 않을 것으로 생각되어 안전한 기술로 판단하였으나 근거가 불충분하고, 최근 해외 가이드라인에서도 권고하지 않는 경향 등을 고려하여 ‘권고하지 않음’으로 심의하였다.

Transcutaneous Electrical Nerve Stimulation for Low Back Pain

Background Transcutaneous electrical nerve stimulation (TENS) is a technique used to alleviate both acute and chronic pain in the nervous and musculoskeletal systems by stimulating the nervous system through electrodes placed at the pain site. This health technology was covered by insurance prior to the implementation of the new Health Technology Assessment system. Subsequently, internal monitoring identified TENS as a candidate for reassessment, and it was selected for review after priority evaluation by the Health Technology Reassessment Committee. The assessment plan was reviewed and approved at the 4th Health Technology Reassessment Committee meeting on April 14, 2023. This assessment aimed to provide scientific evidence on the clinical safety and effectiveness of TENS for low back pain (LBP) to support policy decisions regarding its appropriate use.

Methods A systematic review was conducted to assess the safety and effectiveness of TENS for LBP. Assessment methods were finalized based on discussions with the “Subcommittee for ICT for LBP” (hereafter referred to as the Subcommittee), which included two rehabilitation medicine specialists, two anesthesiologists, one orthopedic surgeon, one neurosurgeon, and one evidence-based medicine expert. The key question was: “Is TENS clinically safe and effective for patients with LBP?” Safety was evaluated based on relevant adverse events (AEs) and complications, whereas effectiveness was assessed based on pain score, functional impairment, and quality of life. Results were presented separately for acute and chronic LBP. Literature searches were conducted in three international and three Korean databases. Study selection was carried out independently by two reviewers using predefined inclusion and exclusion criteria, with disagreements resolved through discussion. The risk of bias was assessed using the Cochrane Risk of Bias (RoB) tool. Data extraction was performed by two independent reviewers using a predefined data extraction form, with any discrepancies resolved through discussion. Meta-analyses were performed where quantitative analysis was feasible; otherwise, a qualitative review was performed. The level of evidence for the systematic review results was graded using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method, and recommendations were based on these results.

Results A total of 14 randomized controlled trials (RCTs) were selected for the assessment, including 12 on chronic low back pain (LBP) and 2 on acute LBP. The studies employed various forms of the TENS intervention, such as high-frequency, low-frequency, and mixed forms. Comparators included sham, no treatment, and conservative treatments (exercise, cold therapy, and ultrasound therapy). The risk of bias was low for the following items: random sequence generation, allocation concealment, incomplete outcome data, and selective reporting. However, the risk of bias was high or uncertain regarding blinding, including blinding of participants and personnel, blinding of outcome assessment, and blinding of assessors.

Safety Two of the 12 studies reported data on the safety of TENS in patients with chronic LBP. When compared with sham or no treatment, skin irritation was reported in both the intervention and control groups; however, all cases were transient and mild. No relevant AEs or complications were reported when TENS was compared with an exercise group. None of the studies reported AEs or complications associated with TENS in patients with acute LBP.

Effectiveness All 12 studies reported data on the effectiveness of TENS in patients with chronic LBP, assessing pain score, functional impairment, and quality of life. All studies provided post-treatment pain scores. Nine studies compared TENS with sham/no treatment, and four compared TENS with conservative treatments (including one multiple comparison study). A meta-analysis of the post-treatment pain scores revealed no significant differences between the TENS and sham/no treatment groups. However, results should be interpreted with caution due to high heterogeneity (sham treatment: standardized mean difference [SMD] –0.34, 95% confidence interval [CI] –0.70 to 0.02, I²=57%; no treatment: SMD 0.19, 95% CI –0.87 to 1.25, I²=88%). Subgroup analyses, considering timing of measurement, single or combined interventions, and number of sessions, were performed due to varying intervention characteristics across studies. Although some studies reported lower pain scores in the TENS group immediately after treatment and at 1–2 weeks after treatment than in sham/no treatment, these differences were not statistically significant. Nine studies reported on functional impairment, with seven comparing TENS with sham/no treatment and three comparing with conservative treatments (including one multiple comparison study). A meta-analysis of the functional impairment scores found no significant differences between the TENS and sham/no treatment groups. However, results should be interpreted with caution due to the high heterogeneity (sham treatment: SMD –0.00, 95% CI –0.23 to 0.23, I²=0%; no treatment: SMD 0.68, 95% CI –0.98 to 2.35, I²=94%). Subgroup analyses also showed no significant differences between the groups. Quality of life was reported in three studies using SF-36 scores. Two studies compared TENS with sham or no treatment, with both groups showing significant improvements in quality of life post-treatment compared with pre-treatment. However, no significant differences were observed between the groups. In one study comparing TENS with conservative treatment (cold therapy), there were also no significant differences between the groups. Two studies reported data for the effectiveness of TENS in patients with acute LBP, both comparing TENS with sham or no treatment. In one study, pain scores immediately after the procedure were significantly lower in the TENS group than in the control group. In the other study, both groups showed significantly lower pain scores post-treatment than during pre-treatment, but there were no significant differences between the groups. Additionally, one study found no significant differences in functional impairment between the TENS and sham/no treatment groups.

Conclusions and Recommendations The Subcommittee presented the following recommendations for TENS in patients with LBP based on the current evidence. TENS for chronic LBP is a noninvasive technique that delivers electrostimulation via electrodes affixed to the skin. The Subcommittee determined the technique to be safe, as evidenced by reports of only transient and mild AEs and complications. Regarding its effectiveness, overall and subgroup analyses revealed that the pain scores, functional impairment, and quality of life did not significantly differ between the TENS and control groups (sham/no treatment or conservative treatment). However, as TENS has long been used for pain relief and three studies comparing TENS with sham/no treatment found significantly lower pain scores in the TENS group immediately after the intervention and at 1–2 weeks post-treatment, the Subcommittee concluded that TENS can be clinically considered for short-term pain relief in patients with chronic LBP. Although no studies reported TENS-related AEs and complications in patients with acute LBP, the Subcommittee also deemed the technique to be clinically safe for this population, as it is unlikely that the safety of the technique would differ for chronic and acute LBP. However, the Subcommittee noted that further research is needed to determine the effectiveness of the technique for acute LBP, as only two relevant studies were identified, with inconsistent and insufficient results. Based on the Subcommittee’s assessment results and Article 4-10 of the Health Technology Reassessment Project Management Guidelines, the following conclusions were drawn for TENS in LBP at the 1st 2024 Health Technology Reassessment Committee meeting (January 12, 2024). The Health Technology Reassessment Committee concluded that TENS is conditionally recommended for pain relief in patients with chronic LBP, based on a comprehensive review of the current evidence on clinical safety and effectiveness, despite the weak evidence and the technique's long-standing use. However, the Committee concluded that TENS is not recommended for patients with acute LBP due to insufficient evidence and the lack of recommendation in recent international guidelines, even though no safety concerns were identified.
