동맥경유 방사선색전술

Abstract

평가배경 동맥경유 방사선색전술(Transarterial Radioembolization, TARE)은 외과적 절제 및 국소치료가 불가능하거나 항암화학요법에 실패한 원발성 또는 전이성 간암 환자를 대상으로 방사성동위원소가 함유된 미세구(microshperes)를 간종양에 주입하여 병변을 괴사시키는 시술이다. 국내에서는 2010년 신의료기술평가를 통해 신의료기술로 인정받아 같은 해 비급여 항목(조-753, 경동맥방사선색전술)으로 등재되었으며, 이후 2019년 의료기술재평가를 거쳐 2020년 선별급여 50% 항목(자-664-2, 동맥경유 방사선색전술)으로 변경되어 현재까지 유지되고 있다. 해당 의료기술은 선별급여 항목의 적합성평가 주기를 고려하여 내부 모니터링으로 발굴되었으며 2025년 제1차 의료기술재평가위원회(2025.1.7.)에서 평가계획서를 심의받아 체계적 문헌고찰을 수행하였다.

평가목적 본 평가의 목적은 동맥경유 방사선색전술이 원발성 및 전이성 간암 환자에서 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 동맥경유 방사선색전술에 대한 안전성, 효과성 및 경제성을 평가하고자 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “동맥경유 방사선색전술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 영상의학과 2인, 소화기내과 2인, 근거기반의학 1인의 전문가 5인으로 구성하였다. 본 평가는 원발성 및 전이성 간암 환자를 대상으로 동맥경유 방사선색전술의 안전성, 효과성, 경제성을 확인하였다. 결과변수로 안전성은 시술에 따른 부작용 및 합병증을, 효과성은 생존율, 국소 치료효과 및 질병진행, 삶의 질을, 경제성으로는 비용효과성을 설정하였다. 대조군은 제한하지 않았으며, 연구유형은 무작위배정 비교임상시험, 전향적 비교연구, 매칭/보정된 후향적 비교연구로 제한하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국교육학술정보원(RISS), 사이언스온-한국과학기술정보연구원(ScienceON)) 데이터베이스에서 검색하였다(최종검색일 2025.3.4.). 최종 선택된 연구의 비뚤림위험 평가는 연구설계에 따라 무작위배정 비교임상시험(Randomized Controlled Trials, RCT)의 경우 Cochrane의 Risk of Bias (RoB)를, 그 외 연구는 비무작위연구 비뚤림위험 평가도구 Risk of Bias for Nonrandomized Studies (RoBANS ver. 2.0)를 사용하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 사전에 정한 서식을 활용해 자료를 추출하고 정량적 합성이 가능한 경우 메타분석을 수행하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과 핵심질문을 충족하는 총 68개의 연구(73편 문헌)가 확인되었으며, 결과유형에 따라 안전성 및 효과성을 보고한 연구 52개(57편 문헌), 경제성 결과를 보고한 연구가 16개였다. 안전성, 효과성을 보고한 52개의 연구는 연구대상자 유형에 따라 원발성 간암 40개, 전이성 간암이 12개였으며, 연구설계에 따라 무작위배정 비교임상시험 13개, 비무작위 비교연구가 39개였다. 중재법은 모두 이트륨(Yttrium)-90을 이용한 동맥경유 방사선색전술로 레진(resin) 또는 유리 미세구(microspheres)를 사용하였다. 비뚤림위험 평가결과, 무작위배정 비교임상시험은 open-label 연구가 많아 연구참여자, 연구자에 대한 눈가림 비뚤림위험을 ‘높음’으로 평가하였고(60% 이상), 배정순서 은폐 및 결과평가에 대한 눈가림에 대한 구체적인 언급이 없어 ‘불확실’로 평가하였다(약 40%). 비무작위 비교연구에서는 교란변수의 매칭/보정을 통해 대상군 비교가능성 비뚤림위험이 대부분(약 50%) ‘낮음’이었으나, 나머지는 매칭/보정 후에도 군간 유의한 차이가 있는 변수가 일부 확인되어 ‘불확실’로 평가하였다. 경제성을 보고한 16개의 연구는 원발성 간암 15개, 전이성 간암 연구가 1개였으며, 연구수행국가에 따라 유럽 9개, 아메리카 6개, 아시아(일본) 1개에서 수행되었다.

안전성 동맥경유 방사선색전술의 안전성을 보고한 연구는 총 38개로(무작위배정 비교임상시험 13개, 비무작위 비교연구 25개) ‘모든 이상반응’과 ‘중대한 이상반응’으로 구분하여 확인하였다. 원발성 간암에서 동맥경유 방사선색전술은 전체 연구(무작위배정 비교임상시험 8개, 비무작위 비교연구 19개)에서 동맥경유 화학색전술, 전신치료 및 그 외 다른 치료법과 비교하여 모든 이상반응 및 중대한 이상반응 발생 위험에 전반적으로 유의한 차이가 없었고, 전신치료 또는 수술과 비교한 각 1개의 비무작위 비교연구에서 동맥경유 방사선색전술의 모든 이상반응 발생 위험이 유의하게 낮은 것으로 나타났다. 동맥경유 방사선색전술과 전신치료의 병용요법은 전신치료 단독과 비교 시 전반적으로 안전성에 유의한 차이가 없었다. 전이성 간암에서 동맥경유 방사선색전술은 전체 연구(무작위배정 비교임상시험 5개, 비무작위 비교연구 6개)에서 동맥경유 화학색전술, 간동맥색전술과 비교하여 모든 이상반응 및 중대한 이상반응 발생 위험에 유의한 차이가 없었으며, 동맥경유 방사선색전술과 전신치료의 병용요법 역시, 전신치료 단독과 비교하여 안전성에 유의한 차이가 없었다.

효과성 동맥경유 방사선색전술의 효과성을 보고한 연구는 총 52개로(무작위배정 비교임상시험 13개, 비무작위 비교연구 39개) ‘생존(전체생존, 무진행생존)’, ‘국소치료효과 및 질병의 진행(객관적 치료반응률, 질병조절률, 질병진행)’, ‘삶의 질’로 크게 구분하여 확인하였다.

원발성 간암 원발성 간암에서 동맥경유 방사선색전술의 효과성을 보고한 연구는 40개(무작위배정 비교임상시험 8개, 비무작위 비교연구 32개)로 비교군별로 분석하였다. 원발성 간암에서 동맥경유 방사선색전술과 동맥경유 화학색전술의 효과성을 비교한 연구는 18개(무작위배정 비교임상시험 4개, 비무작위 비교연구 14개)였다. 전체생존(Overall Survival, OS)은 1개의 무작위배정 비교임상시험에서 동맥경유 방사선색전술이 유의하게 좋았으나(Hazard Ratio (HR) 0.48, 95% Confidence Interval (CI) 0.28, 0.82) 5개의 비무작위 비교연구의 메타분석에서 유의한 차이가 없었고(HR 0.88, 95% CI 0.55, 1.39), 무진행생존(Progression Free Survival, PFS)은 1개의 무작위배정 비교임상시험(HR 0.40, 95% CI 0.24, 0.67)과 2개의 비무작위 비교연구(HR 0.44, 95% CI 0.24, 0.81)의 메타분석결과에서 모두 유의하게 좋았다. 객관적 치료반응률(Objective Response Rate, ORR)과 질병조절률(Disease Control Rate, DCR)은 전반적으로 군간 유의한 차이가 없었던 반면, 질병진행(Progression)은 다수의 개별연구에서 동맥경유 방사선색전술에서 유의하게 낮았으며 2개의 무작위배정 비교임상시험(HR 0.26, 95% CI 0.10, 0.69)과 3개의 비무작위 비교연구(HR 0.23, 95% CI 0.10, 0.55)의 메타분석결과에서도 확인되었다. 삶의 질을 보고한 3개 연구에서는 전반적으로 종합적인 삶의 질 점수에서 두 군간 유의한 차이가 없었으나, 일부 하위지표인 피로, 사회적 웰빙, 기능적 웰빙에서 동맥경유 화학색전술의 삶의 질이 유의하게 높은 것으로 나타났다. 원발성 간암에서 동맥경유 방사선색전술과 전신치료의 효과성을 비교한 연구는 10개(무작위배정 비교임상시험 2개, 비무작위 비교연구 8개)였다. 전체생존은 2개의 무작위배정 비교임상시험에서 유의한 차이가 없었으나(HR 1.14, 95% CI 0.98, 1.32) 5개의 비무작위 비교연구의 메타분석에서 유의하게 좋았고(HR 0.54, 95% CI 0.40, 0.72), 무진행생존은 2개의 무작위배정 비교임상시험(HR 0.97, 95% CI 0.84, 1.12)과 1개의 비무작위 비교연구(HR 0.74, 95% CI 0.50, 1.09) 모두에서 유의한 차이가 없었다. 객관적 치료반응률과 질병조절률은 전반적으로 군간 유의한 차이가 없었던 반면, 질병진행은 2개의 무작위배정 비교임상시험에서 동맥경유 방사선색전술이 유의하게 더 낮았다(HR 0.80, 95% CI 0.66, 0.98). 삶의 질을 보고한 3개의 무작위배정 비교임상시험 중 2개의 연구에서는 전신치료보다 동맥경유 방사선색전술의 삶의 질이 유의하게 높은 것으로 나타났으며, 1개의 비무작위 비교연구에서는 3개월 시점의 종합적 삶의 질 점수와 일부 하위지표(기능적 웰빙)에서 유의하게 좋은 것으로 나타났다. 원발성 간암에서 동맥경유 방사선색전술과 방사선 치료의 효과성을 비교한 연구는 비무작위 비교연구 2개였다. 전체생존은 모든 연구에서 유의한 차이가 없었으며, 이는 메타분석 결과에서도 확인되었다(HR 0.90, 95% 0.65, 1.23). 원발성 간암에서 동맥경유 방사선색전술과 지지요법의 효과성을 비교한 연구는 비무작위 비교연구 2개였다. 모든 연구에서 동맥경유 방사선색전술의 전체생존기간이 지지요법보다 유의하게 더 길었다 (p<0.001). 원발성 간암에서 동맥경유 방사선색전술과 열치료술(percutaneous microwave ablation)의 효과성을 비교한 연구는 비무작위 비교연구 1개였다. 전체생존기간은 군간 유의한 차이가 없었으나 (p=0.203) 동맥경유 방사선색전술의 무진행생존기간이 유의하게 길었으며(p=0.005), 객관적 치료반응률은 유의한 차이가 없었다(p=0.548). 원발성 간암에서 동맥경유 방사선색전술과 수술치료의 효과성을 비교한 연구는 비무작위 비교연구 1개였다. 전체생존(HR 0.98, 95% CI 0.40, 2.43) 및 질병진행시간(HR 1.10, 95% CI 0.55, 2.20)에 있어 모두 군간 유의한 차이가 없었다. 원발성 간암에서 동맥경유 방사선색전술과 두 가지 이상의 병용치료의 효과성을 비교한 연구는 비무작위 비교연구 2개였다. 전체생존, 무진행생존(SHR 0.61, 95% CI 0.21, 1.71) 및 객관적 치료반응률, 질병조절률 모두에서 군간 유의한 차이가 없었다. 원발성 간암에서 선량계산법(dosimetry)에 따른 효과를 확인하기 위해 personalized dosimetry 동맥경유 방사선색전술과 standard dosimetry 동맥경유 방사선색전술을 비교한 1개의 무작위배정 비교임상시험이 확인되었다. 전체생존은 personalized dosimetry 동맥경유 방사선색전술이 standard dosimetry 동맥경유 방사선색전술보다 유의하게 높았으며(HR 0.42, 95% CI 0.22, 0.83), 무진행생존은 군간 유의한 차이가 없었다(HR 0.71, 95% CI 0.39, 1.30). 객관적 치료반응률은 각 71%, 36%로 personalized dosimetry 동맥경유 방사선색전술에서 유의하게 높았다(p=0.007). 원발성 간암에서 동맥경유 방사선색전술과 전신치료의 병용요법과 전신치료 단독을 비교한 3개의 연구에서는 전반적으로 병용요법의 효과가 유의하게 좋은 것으로 나타났다. 전체생존은 1개의 무작위배정 비교임상시험에서 유의한 차이가 없었으나(HR 1.01, 95% CI 0.81, 1.25), 2개의 비무작위 비교연구의 메타분석결과 유의하게 좋았으며(HR 0.68, 95% CI 0.57, 0.81), 무진행생존은 각 1개의 무작위배정 비교임상시험 및 비무작위 비교연구에서 유의하게 좋았다(p<0.05). 객관적 치료반응률, 질병조정률 및 질병진행 모두 동맥경유 방사선색전술과 전신치료의 병용요법이 전신치료 단독보다 유의하게 좋은 것으로 나타났다(p<0.05)

전이성 간암 전이성 간암에서 동맥경유 방사선색전술의 효과성을 보고한 연구는 12개(무작위배정 비교임상시험 5개, 비무작위 비교연구 7개)로 비교군별로 분석하였다. 전이성 간암에서 동맥경유 방사선색전술과 동맥경유 화학색전술의 효과성을 비교한 연구는 비무작위 비교연구 4개였다. 메타분석결과, 전체생존(HR 1.40, 95% CI 0.95, 2.05), 무진행생존(HR 1.27, 95% CI 0.85, 1.89), 객관적 치료반응률(Odds Ratio (OR) 0.63, 95% CI 0.08, 5.20), 질병조절률(OR 0.51, 95% CI 0.01, 28.46), 모든 효과성 지표에서 군간 유의한 차이가 없었다. 전이성 간암에서 동맥경유 방사선색전술과 지지요법의 효과성을 비교한 연구는 비무작위 비교연구 2개였다. 모든 연구에서 동맥경유 방사선색전술의 전체생존기간이 지지요법보다 유의하게 더 길었다(p=0.001). 전이성 간암에서 동맥경유 방사선색전술과 간동맥색전술(Hepatic Arterial Embolization, HAE)의 효과성을 비교한 연구는 무작위배정 비교임상시험 1개였다. 동맥경유 방사선색전술의 3개월 시점, 객관적 치료반응률이 간동맥색전술보다 낮았으나(p=0.002), 6개월 시점에서 유의한 차이가 없었다(p=0.24). 전이성 간암에서 동맥경유 방사선색전술과 전신치료의 병용요법과 전신치료 단독을 비교한 5개의 연구(무작위배정 비교임상시험 4개, 비무작위 비교연구 1개)에서는 전반적으로 효과성에 유의한 차이가 없었다. 각 4개, 2개의 무작위배정 비교임상시험의 메타분석에서 전체생존(HR 1.00, 95% CI 0.82, 1.22)과 무진행생존(HR 0.80, 95% CI 0.62, 1.04)은 유의한 차이가 없었으며, 객관적 치료반응률(RR 1.42, 95% CI 0.64, 3.19), 질병조절률(RR 1.32, 95% CI 0.72, 2.44)에서도 유의한 차이가 없었다. 그러나 질병진행은 2개의 무작위배정 비교임상시험의 메타분석결과, 동맥경유 방사선색전술과 전신치료의 병용요법이 전신치료 단독보다 유의하게 좋은 것으로 나타났다(HR 0.49, 95% CI 0.38, 0.64).

경제성 동맥경유 방사선색전술의 비용효과성(Cost Effectiveness)을 보고한 연구는 총 16개였으며, 연구대상자의 구분에 따라 원발성 간암 15개, 전이성 간암 연구가 1개였다. 원발성 간암 환자에서 동맥경유 방사선색전술과 동맥경유 화학색전술의 비용효과성을 비교한 5개의 연구는 모두 동맥경유 방사선색전술이 비용효과적이거나 비용효과적일 가능성이 높은 치료법으로 보았다. 특히 Shirota 등(2024)은 방사성 미세구의 보험급여 가격이 2.8% 낮아진다면 비용효과적일 수 있을 것으로, Rostambeigi 등(2014)은 초기 단계(Barcelona Clinic Liver Cancer (BCLC) stage A) 환자에서는 비용효과적이지 않을 수 있으나 진행 단계(BCLC-C) 환자에서 비용이 정당화될 수 있다고 보았다. 원발성 간암 환자에서 동맥경유 방사선색전술과 전신치료의 비용효과성을 비교한 7개의 연구 중 대부분 동맥경유 방사선색전술이 비용효과적이거나 비용효과적일 가능성이 높은 치료법으로 보았으나, 1개 프랑스 연구(Zarca 등, 2021)는 두 치료법 간 우세할 확률 50%로 동일하고 동맥경유 방사선색전술이 지불의사비용(Willingness-to-pay (WTP), €450,000/QALY)을 초과하였기 때문에 비용효과적인 옵션이 아니라고 결론내렸다. 특히, Schwander 등 (2024)의 연구는 2개의 임상연구 자료를 기반으로 personalized dosimetry 동맥경유 방사선색전술이 아테졸리주맙과 베바시주맙의 병용-전신치료보다 비용효과적인 치료법이라고 보았다. 그 외 Wu 등(2023)은 주요 국소치료법 3개(동맥경유 방사선색전술, 동맥경유 화학색전술, 경피적 절제술(percutaneous ablation)) 중에서 경피적 절제술이 가장 비용효과적인 가교 전략인 것으로 보고하였다. Rognoni 등(2022)은 personalized dosimetry 동맥경유 방사선색전술이 standard dosimetry 동맥경유 방사선색전술에 비해 비용효과적인 치료법으로 보았으며, Pennington 등(2015)은 동맥경유 방사선색전술이 지지요법보다 비용효과적인 치료법으로 보았다. 대장암에서 기원한 전이성 간암 환자에서 동맥경유 방사선색전술과 최적의 지지요법의 비용효과성을 비교한 1개 연구(Brennan 등, 2020)는 동맥경유 방사선색전술이 비용효과적인 치료법으로 보았다.

결론 및 권고결정 의료기술재평가 소위원회에서는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 원발성 간암 환자에서 동맥경유 방사선색전술은 동맥경유 화학색전술과 비교하여 전체 및 중대한 이상반응의 발생에 유의한 차이가 없었으며, 일부 연구에서는 복통, 피로와 같은 경미한 이상반응이 유의하게 덜 발생하여 안전한 의료기술로 판단하였다(근거수준: 낮음 또는 매우 낮음). 효과성은 무진행생존 및 질병진행률에 있어 유의하게 더 좋았으나 그 외 전체생존율, 객관적 치료반응률, 질병조절률, 삶의 질 지표에서는 유의한 차이가 없었다(근거수준: 낮음 또는 매우 낮음). 그 외 다른 치료법(전신치료, 방사선 치료, 병용치료 등)과 비교 시 안전성 및 효과성 결과도 유사하였으며(근거수준: 낮음 또는 매우 낮음), 동맥경유 방사선색전술과 전신치료의 병용요법은 전신치료 단독과 비교하여 전반적으로 안전성에 유의한 차이가 없었으나 효과성은 유의하게 더 좋았다. 추가적으로, 동맥경유 방사선색전술의 선량계산법에 따라 personalized dosimetry와 standard dosimetry를 비교한 1개의 무작위배정 비교임상시험에서 안전성에는 유의한 차이가 없었으나, 효과성에서 personalized dosimetry의 전체생존 및 객관적 치료반응률이 유의하게 더 좋았으며, 그 외 무진행생존에서 유의한 차이가 없었다(근거수준: 낮음). 원발성 간암 환자에서 동맥경유 방사선색전술과 동맥경유 화학색전술, 전신치료 등의 비용효과성을 확인한 15개의 연구에서는 대체로 비교치료법에 비해 비용효과적이거나 비용효과적일 가능성이 높은 치료법으로 결론내리고 있었다. 전이성 간암 환자에서 동맥경유 방사선색전술은 동맥경유 화학색전술과 비교하여 전체 및 중대한 이상반응의 발생에 유의한 차이가 없는 안전한 의료기술로 판단하였으며(근거수준: 매우 낮음), 효과성은 생존율, 국소치료효과 및 질병의 진행, 삶의 질 모든 지표에서 유의한 차이가 없었다(근거수준: 매우 낮음). 그 외 다른 치료법(간동맥색전술, 지지요법 등)과 비교 시 안전성 및 효과성 결과도 유사하였으며, 동맥경유 방사선색전술과 전신치료의 병용요법은 전신치료 단독과 비교하여 전반적으로 안전성 및 효과성에 유의한 차이가 없었으나 질병진행 지표에서 유의하게 더 효과적이었다. 전이성 간암 환자에서 동맥경유 방사선색전술과 최적의 지지요법의 비용효과성을 확인한 1개 연구에서는 동맥경유 방사선색전술이 비용효과적인 치료법인 것으로 결론내렸다. 소위원회에서는 본 평가의 몇 가지 제한점을 다음과 같이 언급하였다.

첫째, 최근 동맥경유 방사선색전술은 치료 효과를 결정하는 선량계산법이 기존 standard dosimetry에서 보다 고선량(high dose)의 방사선을 투여하는 personalized dosimetry로 큰 변화(발전)가 있어 치료성적이 향상되고 있다. 이는 본 평가에 포함된 1개 무작위배정 비교임상시험(Garin 등, 2021, 2024)에서도 personalized dosimetry의 전체생존 및 객관적 치료반응률이 standard dosimetry보다 유의하게 더 좋은 것으로 확인되었다. 그러나 본 평가에 선택된 나머지 대부분의 연구들은 standard dosimetry를 이용한 동맥경유 방사선색전술에 대한 것으로 이에 근거한 본 평과결과는 현재의 임상상황을 제대로 반영하지 못할 수 있어, 추후 personalized dosimetry를 적용한 비교연구 결과가 충분히 축적된 뒤에 재평가가 필요하다고 판단된다.

둘째, 본 평가에 포함된 비교군, 전신치료에는 소라페닙, 렌바티닙 등의 약물이 일차적으로 사용되었으나, 최근 전신치료 약물의 발전에 따라 아테졸리주맙과 베바시주맙의 병용요법 등이 일차치료제로 추천되고 있어, 전신치료와의 비교결과 역시 personalized dosimetry와 새로운 전신치료 일차치료제와의 비교연구 결과가 충분히 축적된 뒤에 재평가가 필요하다고 판단된다. 마지막으로, 비용효과성을 평가한 다수의 경제성 문헌에서 동맥경유 방사선색전술이 동맥경유 화학색전술 또는 전신치료에 비해 비용효과적이거나 비용효과적일 가능성이 높은 것으로 보고되었다. 다만 이는 다른 국가의 임상환경과 비용을 반영한 결과이므로 국내 상황에 직접적으로 적용하기에는 한계가 있어 해석에 신중함이 필요하겠다. 2025년 제8차 의료기술재평가위원회(2025.8.8.)에서는 소위원회 결론 및 분과 의견을 검토하여 의료기술재평가사업 관리지침 제4조10항에 의거 ‘동맥경유 방사선색전술’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 원발성 간암 환자를 대상으로 동맥경유 방사선색전술을 ‘약하게 권고함’으로 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 전이성 간암 환자를 대상으로 동맥경유 방사선색전술을 ‘약하게 권고함’으로 심의하였다.

Background Transarterial radioembolization (TARE) is a procedure for patients with primary or metastatic liver cancer who are ineligible for surgical resection or local treatment, or who have failed chemotherapy. The procedure involves the injection of radioactive isotope-containing microspheres into hepatic tumors to induce tumor necrosis. In Korea, TARE was recognized as a new health technology through the New Health Technology Assessment in 2010. It was listed in the same year as a non-reimbursed item, "Transarterial Radioembolization" (조-753). Subsequently, following a health technology reassessment in 2019, it was reclassified in 2020 as a selective benefit item with 50% patient co-payment, "Transarterial Radioembolization" (자-664-2), and this status has been maintained to date. This health technology was identified through internal monitoring, in line with the reassessment cycle for selective benefit items. A systematic review was conducted following the assessment protocol's review by the 1st Health Technology Reassessment Committee on January 7, 2025.

Objective The objective of this assessment was to provide evidence on whether transarterial radioembolization is clinically safe and effective in patients with primary or metastatic liver cancer, and to determine the recommendation grade for health technology reassessment.

Methods A systematic review was conducted to evaluate the safety, effectiveness, and economic feasibility of TARE. All assessment methods were finalized through discussion by the Subcommittee on Transarterial Radioembolization (hereinafter, the "Subcommittee") in consideration of the assessment objective. The Subcommittee consisted of five experts: two radiologists, two gastroenterologists, and one expert in evidence-based medicine. This assessment examined the safety, effectiveness, and economic feasibility of TARE in patients with primary or metastatic liver cancer. Safety outcomes included procedure-related adverse events and complications. Effectiveness outcomes included survival, local treatment response, disease progression, and quality of life. Economic outcomes included cost-effectiveness. No restriction was placed on the comparator. Eligible study designs were limited to randomized controlled trials, prospective comparative studies, and matched or adjusted retrospective comparative studies. Based on the key questions, literature searches were conducted in three international databases, Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews–Cochrane Central Register of Controlled Trials, and three Korean databases, KoreaMed, Research Information Sharing Service, and ScienceON. The final search date was March 4, 2025. The risk of bias in the final selected studies was assessed according to study design. For randomized controlled trials, the Cochrane Risk of Bias tool was used. For other studies, the Risk of Bias for Non-randomized Studies, version 2.0, was used. All processes were performed independently by two reviewers. Data were extracted using a predefined form, and meta-analysis was conducted where quantitative synthesis was possible. The certainty of evidence for the systematic review findings was assessed using the Grading of Recommendations, Assessment, Development, and Evaluation approach. Based on the Subcommittee's review findings, the Health Technology Reassessment Committee conducted the final deliberation and determined the recommendation grade for health technology reassessment.

Results The systematic review identified 68 studies, reported in 73 publications, that addressed the key questions. By outcome type, 52 studies, reported in 57 publications, assessed safety and effectiveness, and 16 studies reported economic outcomes. Among the 52 studies reporting safety and effectiveness, 40 studies were conducted in patients with primary liver cancer and 12 in patients with metastatic liver cancer. By study design, 13 were randomized controlled trials, and 39 were non-randomized comparative studies. All interventions involved Yttrium-90-based TARE using resin or glass microspheres. In the risk of bias assessment, many randomized controlled trials were open-label studies, and the risk of bias for blinding of participants and personnel was rated as high in more than 60% of studies. Allocation concealment and blinding of outcome assessment were rated as unclear in approximately 40% of studies because specific information was not reported. In non-randomized comparative studies, the risk of bias for comparability of study groups was rated as low in most studies, approximately 50%, due to matching or adjustment for confounding variables. However, the remaining studies were rated as unclear because some variables still showed significant between-group differences even after matching or adjustment. Among the 16 studies reporting economic outcomes, 15 studies were conducted in patients with primary liver cancer and one in patients with metastatic liver cancer. By country or region, nine studies were conducted in Europe, six in the Americas, and one in Asia, specifically Japan.

Safety A total of 38 studies reported the safety of TARE, including 13 randomized controlled trials and 25 non-randomized comparative studies. Safety outcomes were assessed by categorizing them into all adverse events and serious adverse events. In primary liver cancer, TARE showed no overall significant difference in the risk of all adverse events or serious adverse events compared with transarterial chemoembolization, systemic therapy, or other treatments across all studies, including eight randomized controlled trials and 19 non-randomized comparative studies. In one non-randomized comparative study, each comparing TARE with systemic therapy or surgery, the risk of all adverse events was significantly lower in the TARE group. Combination therapy with TARE and systemic therapy showed no overall significant safety difference compared with systemic therapy alone. In metastatic liver cancer, TARE showed no significant difference in the risk of all adverse events or serious adverse events compared with transarterial chemoembolization or hepatic arterial embolization across all studies, including five randomized controlled trials and six non-randomized comparative studies. Combination therapy with TARE and systemic therapy also showed no significant safety difference compared with systemic therapy alone.

Effectiveness A total of 52 studies reported the effectiveness of TARE, including 13 randomized controlled trials and 39 non-randomized comparative studies. Effectiveness outcomes were broadly categorized into survival outcomes (including overall survival and progression-free survival), local treatment response and disease progression (including objective response rate, disease control rate, and disease progression), and quality of life.

Primary liver cancer A total of 40 studies reported the effectiveness of TARE in patients with primary liver cancer, including eight randomized controlled trials and 32 non-randomized comparative studies. The comparator analyzed the results. Eighteen studies compared the effectiveness of TARE with transarterial chemoembolization in patients with primary liver cancer, including four randomized controlled trials and 14 non-randomized comparative studies. Overall survival was significantly better with TARE in one randomized controlled trial, with a hazard ratio of 0.48 and 95% confidence interval of 0.28 to 0.82. However, a meta-analysis of five non-randomized comparative studies showed no significant difference, with a hazard ratio of 0.88 and 95% confidence interval of 0.55 to 1.39. Progression-free survival was significantly better with TARE in both one randomized controlled trial, with a hazard ratio of 0.40 and 95% confidence interval of 0.24 to 0.67, and in a meta-analysis of two non-randomized comparative studies, with a hazard ratio of 0.44 and 95% confidence interval of 0.24 to 0.81. Objective response rate and disease control rate generally showed no significant between-group differences. By contrast, disease progression was significantly lower in the TARE group in several individual studies, and this was also confirmed in meta-analyses of two randomized controlled trials, with a hazard ratio of 0.26 and 95% confidence interval of 0.10 to 0.69, and three non-randomized comparative studies, with a hazard ratio of 0.23 and 95% confidence interval of 0.10 to 0.55. In three studies reporting quality of life, no overall significant difference in global quality-of-life scores was observed between the two groups. However, some subdomains, including fatigue, social well-being, and functional well-being, were significantly better in the transarterial chemoembolization group. Ten studies compared the effectiveness of TARE with systemic therapy in patients with primary liver cancer, including two randomized controlled trials and eight non-randomized comparative studies. Overall survival showed no significant difference in two randomized controlled trials, with a hazard ratio of 1.14 and 95% confidence interval of 0.98 to 1.32. However, a meta-analysis of five non-randomized comparative studies showed significantly better overall survival with TARE, with a hazard ratio of 0.54 and 95% confidence interval of 0.40 to 0.72. Progression-free survival showed no significant difference in either of two randomized controlled trials (hazard ratio = 0.97; 95% confidence interval = 0.84 to 1.12) or in one non-randomized comparative study (hazard ratio = 0.74; 95% confidence interval = 0.50 to 1.09). Objective response rate and disease control rate generally showed no significant between-group differences. However, disease progression was significantly lower with TARE in two randomized controlled trials, with a hazard ratio of 0.80 and 95% confidence interval of 0.66 to 0.98. Among three randomized controlled trials reporting quality of life, two studies showed significantly better quality of life with TARE than with systemic therapy. In one non-randomized comparative study, the global quality-of-life score and some subdomains, including functional well-being, were significantly better at three months. Two non-randomized comparative studies compared the effectiveness of TARE with radiation therapy in patients with primary liver cancer. Overall survival showed no significant difference across studies, as confirmed by the meta-analysis, with a hazard ratio of 0.90 and a 95% confidence interval of 0.65 to 1.23. Two non-randomized comparative studies compared the effectiveness of TARE with supportive care in patients with primary liver cancer. In all studies, overall survival was significantly longer with TARE than with supportive care (p-values < 0.001). One non-randomized comparative study compared the effectiveness of TARE with percutaneous microwave ablation in patients with primary liver cancer. Overall survival showed no significant between-group difference (p=0.203). Progression-free survival was significantly longer with TARE (p=0.005), whereas objective response rate did not differ significantly (p=0.548). One non-randomized comparative study compared the effectiveness of TARE with surgical treatment in patients with primary liver cancer. No significant between-group differences were observed in either overall survival (hazard ratio = 0.98; 95% confidence interval = 0.40 to 2.43) or time to progression (hazard ratio = 1.10; 95% confidence interval = 0.55 to 2.20). Two non-randomized comparative studies compared the effectiveness of TARE with two or more combination therapies in patients with primary liver cancer. No significant between-group differences were observed in overall survival or progression-free survival, with a subdistribution hazard ratio of 0.61 (95% confidence interval: 0.21 to 1.71), objective response rate, or disease control rate. One randomized controlled trial compared personalized dosimetry TARE with standard dosimetry TARE to assess the effect of dosimetry methods in patients with primary liver cancer. Overall survival was significantly better with personalized dosimetry TARE than with standard dosimetry TARE, with a hazard ratio of 0.42 and 95% confidence interval of 0.22 to 0.83. Progression-free survival showed no significant between-group difference, with a hazard ratio of 0.71 and 95% confidence interval of 0.39 to 1.30. The objective response rate was significantly higher with personalized dosimetry TARE than with standard dosimetry TARE, at 71% versus 36%, respectively (p=0.007). Three studies compared combination therapy with TARE and systemic therapy versus systemic therapy alone in patients with primary liver cancer. Overall, the combination therapy showed significantly better effectiveness. Overall survival showed no significant difference in one randomized controlled trial, with a hazard ratio of 1.01 and 95% confidence interval of 0.81 to 1.25. However, a meta-analysis of two non-randomized comparative studies showed significantly better overall survival with combination therapy, with a hazard ratio of 0.68 and 95% confidence interval of 0.57 to 0.81. Progression-free survival was significantly better in one randomized controlled trial and one non-randomized comparative study, both with p-values < 0.05. Objective response rate, disease control rate, and disease progression were all significantly better with combination therapy (TARE and systemic therapy) than with systemic therapy alone (p values < 0.05).

Metastatic liver cancer A total of 12 studies reported the effectiveness of TARE in patients with metastatic liver cancer, including five randomized controlled trials and seven non-randomized comparative studies. The findings were analyzed by comparator type. Four non-randomized comparative studies compared the effectiveness of TARE with transarterial chemoembolization in patients with metastatic liver cancer. Meta-analysis showed no significant between-group differences in any effectiveness outcome: overall survival, with a hazard ratio of 1.40 and 95% confidence interval of 0.95 to 2.05; progression-free survival, with a hazard ratio of 1.27 and 95% confidence interval of 0.85 to 1.89; objective response rate, with an odds ratio of 0.63 and 95% confidence interval of 0.08 to 5.20; and disease control rate, with an odds ratio of 0.51 and 95% confidence interval of 0.01 to 28.46. Two non-randomized comparative studies compared the effectiveness of TARE with supportive care in patients with metastatic liver cancer. In all studies, overall survival was significantly longer with TARE than with supportive care (p=0.001). One randomized controlled trial compared the effectiveness of TARE with hepatic arterial embolization in patients with metastatic liver cancer. The objective response rate at 3 months was lower with TARE than with hepatic arterial embolization (p=0.002). However, no significant difference was observed at six months (p=0.24). Five studies compared combination therapy with TARE and systemic therapy versus systemic therapy alone in patients with metastatic liver cancer, including four randomized controlled trials and one non-randomized comparative study. Overall, no significant difference in effectiveness was observed. Meta-analyses of four and two randomized controlled trials showed no significant differences in overall survival(hazard ratio = 1.00; 95% confidence interval = 0.82 to 1.22) or progression-free survival (hazard ratio = 0.80; 95% confidence interval = 0.62 to 1.04). No significant differences were observed in objective response rate(risk ratio = 1.42; 95% confidence interval = 0.64 to 3.19) or disease control rate(risk ratio = 1.32; 95% confidence interval = 0.72 to 2.44). However, disease progression was significantly better with combination therapy (TARE and systemic therapy) than with systemic therapy alone in a meta-analysis of two randomized controlled trials, with a hazard ratio of 0.49 and a 95% confidence interval of 0.38 to 0.64.

Economic Evaluation A total of 16 studies reported the cost-effectiveness of TARE. By target population, 15 studies were conducted in patients with primary liver cancer and one in patients with metastatic liver cancer. Five studies comparing the cost-effectiveness of TARE with transarterial chemoembolization in patients with primary liver cancer concluded that TARE was either cost-effective or likely to be cost-effective. In particular, Shirota et al.(2024) reported that TARE could be cost-effective if the reimbursement price of radioactive microspheres decreased by 2.8%. Rostambeigi et al. (2014) reported that TARE might not be cost-effective in patients with early-stage disease(Barcelona Clinic Liver Cancer stage A), but the cost could be justified in patients with advanced-stage disease(Barcelona Clinic Liver Cancer stage C). Among seven studies comparing the cost-effectiveness of TARE with systemic therapy in patients with primary liver cancer, most concluded that TARE was either cost-effective or likely to be cost-effective. However, one French study, Zarca et al. (2021), concluded that TARE was not a cost-effective option because the probability of being superior was equal between the two treatments at 50%, and TARE exceeded the willingness-to-pay threshold of €450,000 per quality-adjusted life-year. In particular, Schwander et al. (2024), based on data from two clinical studies, concluded that personalized dosimetry TARE was more cost-effective than systemic combination therapy with atezolizumab and bevacizumab. In other studies, Wu et al. (2023) reported that among three major local treatments, TARE, transarterial chemoembolization, and percutaneous ablation, percutaneous ablation was the most cost-effective bridging strategy. Rognoni et al. (2022) concluded that personalized dosimetry TARE was more cost-effective than standard dosimetry TARE. Pennington et al. (2015) concluded that TARE was more cost-effective than supportive care. One study, Brennan et al. (2020), compared the cost-effectiveness of TARE with best supportive care in patients with metastatic liver cancer originating from colorectal cancer and concluded that TARE was a cost-effective treatment.

Conclusions and Recommendations Based on the current assessment findings, the Health Technology Reassessment Subcommittee presented the following conclusions. In patients with primary liver cancer, TARE showed no significant difference in the occurrence of all or serious adverse events compared with transarterial chemoembolization. In some studies, minor adverse events, such as abdominal pain and fatigue, occurred significantly less frequently with TARE. Therefore, TARE was considered a safe health technology, with a low or very low certainty of evidence. Regarding effectiveness, TARE was significantly superior in progression-free survival and disease progression rate. However, no significant differences were observed in overall survival, objective response rate, disease control rate, or quality-of-life outcomes, with a low or very low certainty of evidence. Compared with other treatments, including systemic therapy, radiation therapy, and combination therapies, safety and effectiveness outcomes were similar, with a low or very low certainty of evidence. Combination therapy with TARE and systemic therapy showed no overall significant difference in safety compared with systemic therapy alone, but was significantly more effective. Additionally, one randomized controlled trial comparing personalized dosimetry TARE with standard dosimetry TARE, based on the dosimetry method, found no significant difference in safety. However, personalized dosimetry showed significantly better overall survival and objective response rate, while progression-free survival did not differ significantly, with low certainty of evidence. In 15 studies assessing the cost-effectiveness of TARE compared with transarterial chemoembolization, systemic therapy, or other treatments in patients with primary liver cancer, TARE was generally found to be cost-effective or likely to be cost-effective compared with these alternatives. In patients with metastatic liver cancer, TARE showed no significant difference in the occurrence of all or serious adverse events compared with transarterial chemoembolization. It was considered a safe health technology, with a very low certainty of evidence. Regarding effectiveness, no significant differences were observed in any outcomes, including survival, local treatment response, disease progression, and quality of life, with a very low certainty of evidence. Compared with other treatments, including hepatic arterial embolization and supportive care, safety and effectiveness outcomes were also similar. Combination therapy with TARE and systemic therapy showed no overall significant difference in safety or effectiveness compared with systemic therapy alone, but was significantly more effective in delaying disease progression. One study assessing the cost-effectiveness of TARE compared with best supportive care in patients with metastatic liver cancer concluded that TARE was cost-effective. The Subcommittee noted several limitations of this assessment. First, recent TARE practice has undergone a major shift from standard to personalized dosimetry, which delivers higher radiation doses and plays an important role in determining treatment effect. This development has led to improved treatment outcomes. In one randomized controlled trial included in this assessment, Garin et al. (2021, 2024) found that personalized dosimetry showed significantly better overall survival and objective response rate than standard dosimetry. However, most of the other studies included in this assessment evaluated TARE using standard dosimetry. Therefore, the assessment findings based on these studies may not fully reflect the current clinical context. Further reassessment will be needed after sufficient comparative evidence on personalized dosimetry has accumulated. Second, the comparator systemic therapies included in this assessment primarily involved agents such as sorafenib and lenvatinib. However, with recent advances in systemic therapy, combination therapy with atezolizumab and bevacizumab is now recommended as a first-line treatment. Therefore, reassessment will also be needed after sufficient comparative evidence has accumulated on personalized dosimetry and newer first-line systemic therapies. Finally, many economic evaluation studies reported that TARE was cost-effective or likely to be cost-effective compared with transarterial chemoembolization or systemic therapy. However, these findings reflect clinical settings and costs in other countries and therefore have limitations when directly applied to the Korean context. Accordingly, careful interpretation is required. At the 8th Health Technology Reassessment Committee meeting on August 8, 2025, the Committee reviewed the Subcommittee's conclusions and the opinions of the relevant division. In accordance with Article 4, Paragraph 10 of the Health Technology Reassessment Program Management Guidelines, the Committee deliberated on TARE as follows. After comprehensively considering the evidence on clinical safety and effectiveness, along with other assessment domains, the Health Technology Reassessment Committee determined that TARE is weakly recommended for patients with metastatic liver cancer in the Korean clinical context.
