인터루킨-6 [정밀면역검사]_종양이나 염증성 질환의 활동성 평가

Abstract

평가배경 인터루킨-6(interleukin-6, IL-6) [정밀면역검사]는 IL-6를 정량적으로 측정하여 감염, 패혈증의 초기 진단뿐만 아니라 종양 및 염증성 질환의 활동성 평가에 활용되는 검사로, 2001년 ‘인터루킨-6’라는 명칭으로 비급여 항목(노476)으로 등재되었다. 이후 2016년에는 본인부담률 차등적용(80%) 항목으로 전환되었고, 2018년에는 현재의 명칭과 분류체계로 변경되었으며, 2021년 12월에는 선별급여(본인부담률 90%)로 조정되었다. 본 기술은 2026년 적합성 평가가 예정된 항목으로, 수요조사와 관련 검토 절차를 통해 재평가 필요성이 인정되어 재평가 대상으로 선정되었으며, 2025년 3월 14일 개최된 제3차 의료기술재평가위원회*에서 평가계획서 및 소위원회 구성을 심의·확정하였다.

「요양기관업무포털(건강보험심사평가원)」의 ‘행위설명’에 따르면, 본 검사는 감염·패혈증의 초기 진단 또는 IL-6 상승이 동반되는 종양·염증성 질환의 활동성 평가에 활용되는 것으로 정의된다. 이에 따라 본 평가는 두 임상적 목적을 구분하여 검토하였으며, 본 보고서에는 종양·염증성 질환의 활동성 평가를 대상으로 한 평가결과를 제시하였다.

평가목적 본 평가는 IL-6 [정밀면역검사]가 종양 및 염증성 질환의 활동성 평가하는데 임상적 효과성에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가 방법 본 평가는 IL-6 검사의 적응증이 건강보험심사평가원 행위설명에 제시된 정의 및 적응증에서 ‘감염이나 패혈증의 초기 진단 또는 IL-6가 증가하는 종양이나 염증성 질환의 활동성 평가’로 기술되어 있어, 특정 질환으로 한정하기 어려운 점을 고려하였다. 이에 따라 감염, 패혈증, 종양 및 염증성 질환 전반의 연구 경향을 정해진 기간 내에 포괄적으로 파악하기 위해 체계적 문헌고찰(Systematic review, SR) 대신 체계적 문헌고찰 개관(Overview of SR) 방법을 적용하였다. 다만, 본 평가에서는 이 중 ‘종양 및 염증성 질환의 활동성 평가’에 대한 근거를 중심으로 분석하였다.

평가방법은 평가목적에 따라 IL-6 [정밀면역검사] 소위원회의 논의를 거쳐 확정하였으며, 소위원회는 감염내과 2인, 진단검사의학과 1인, 호흡기내과 1인, 소아청소년과(호흡기내과) 1인, 류마티스내과 1인, 근거기반의학 1인의 전문가 7인으로 구성하였다.

본 평가의 핵심질문은 “IL-6 정밀면역검사가 종양 또는 염증성 질환의 활동성 평가에 임상적으로 유효한가?”이다. 연구대상은 종양 환자와 전형적 염증성 질환 환자를 포함하였으며, 병태생리에 염증이 일부 관여하더라도 전형적 염증성 질환으로 분류되지 않는 경우는 제외하였다. 코로나바이러스감염증-19(Coronavirus Disease 2019, COVID-19)는 감염성 질환이지만, 실제 임상 사용 비중이 높고 평가 필요성이 반복 제기된 점을 고려하여 분석은 수행하되, 본문 결론에서는 제외하고 부록에만 제시하였다. 비교검사는 종양 환자의 경우 종양괴사인자 알파(Tumor Necrosis Factor-alpha, TNF-α), 인터루킨-2(Interleukin-2, IL-2), 용해성 인터루킨-2 수용체(Soluble Interleukin-2 Receptor, sIL-2R)를, 염증성 질환의 경우 C-반응성 단백질(C-reactive protein, CRP), 적혈구침강속도(Erythrocyte Sedimentation Rate, ESR)를 포함하였다. 참고표준검사는 각 연구에서 사용된 질환 중증도·활동성 기준을 제한 없이 수용하였으며, 의료결과는 질환 활동성 및 중증도(종양의 경우 병기 포함)와의 연관성을 중심으로 검토하였다.

연구 검색은 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 2개(KoreaMed, 한국교육학술정보원)에서 검색하였으며(최종검색일 2025. 5.1.), 최신의 연구 동향을 반영하고자 2010년 이후에 출판된 체계적 문헌고찰을 포함하였다. 연구 선정기준 및 배제기준 적용을 통한 연구선택과 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치 시 제3자 합의를 통해 조정하였다. 최종 선택된 연구의 비뚤림위험 평가는 A measurement tool to assess systematic reviews-2 (AMSTAR-2)를 사용하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 질적 검토(qualitative review)를 수행하였으며, IL-6 검사의 종양 및 염증성 질환에서의 질병활동성 평가 유용성을 통합하기 위해, 선정된 체계적 문헌고찰과 포함 원저의 질환 범위를 재확인하여 분석 단위를 재정의하고, 자가면역성 염증질환과 종양성 질환으로 구분하여 근거를 합성하였다.

본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 본 평가에 선택된 체계적 문헌고찰은 총 6편(원저 총 54편, 연구대상자 총 5,503명)으로 대상자별로 자가면역성 염증질환 4편, 종양성 질환 2편이 포함되었다. 비뚤림위험 평가 결과, 제외 문헌의 사유가 불명확하거나, 비뚤림위험 및 출판 비뚤림에 대한 고려가 미흡한 문헌이 다수 확인되었다. 이에 따라 전체적인 근거의 신뢰도는 ‘매우 낮음’으로 평가하였다.

자가면역성 염증질환과 관련된 체계적 문헌고찰은 총 4편이 포함되었으며, 각각 베체트병, 다카야수 동맥염, 전신홍반루푸스, 구강편평태선 환자를 대상으로 한 연구였다. 이들 모두 IL-6 단독 검사 결과를 보고하였으며, 비교검사와의 직접 비교는 이루어지지 않았다

베체트병 환자를 대상으로 한 체계적 문헌고찰은 총 1편(원저 총 4편, 연구대상자 총 169명)으로 질병활성도에 따른 IL-6 수치 차이를 분석한 결과, 통합 표준화된 평균차(standardized mean difference, SMD)는 1.29 (95% 신뢰구간(confidence interval, CI) 0.06, 2.53, I2 = 94.6%)으로 질병활성도가 높은 군에서 통계적으로 유의하게 높은 수치를 보였다.

다카야수 동맥염 환자를 대상으로 한 체계적 문헌고찰은 총 1편(원저 총 7편, 연구대상자 총 1,611명)으로 활성군과 비활성군의 IL-6 수치 차이를 분석한 결과, 통합 가중평균차(weighted mean difference, WMD)는 16.69 pg/mL (95%CI 5.32, 28.06)으로 활성군에서 유의하게 높은 수치를 보였다.

전신홍반루푸스 환자를 대상으로 한 체계적 문헌고찰은 총 1편 (원저 총 13편, 연구대상자 총 1,295명)에서는 질병활성도에 따른 IL-6 수치 차이는 통합 SMD가 1.10 (95% CI 0.72, 1.48, I2 = 79.2%)로 활성군에서 IL-6 수치가 통계적으로 유의하게 높았으며, IL-6 수치와 질병활성도 간에는 유의한 양의 상관관계(r = 0.36)가 확인되었다.

구강편평태선 환자를 대상으로 한 체계적 문헌고찰은 총 1편(원저 총 4편, 연구대상자 총 259명)에서는 미란형과 비미란형 간 IL-6 수치 차이는 통합 SMD가 1.37 (95% CI -0.26, 3.00, I2 = 94%)로 유의한 차이는 없었다.

종양성 질환의 활동성 평가와 관련된 체계적 문헌고찰은 총 2편이 확인되었으며, 각각 난소암과 대장직장암 환자를 대상으로 수행되었다.

난소암 환자를 대상으로 한 체계적 문헌고찰은 총 1편(원저 총 20편, 연구대상자 수 미기재)으로 국제산부인과연맹(International Federation of Gynecology and Obstetrics, FIGO) 병기 기준에 따라 조기 병기(Stage I–II)와 진행 병기(Stage III–IV)로 구분하여 IL-6 수치를 비교한 결과, 진행 병기군에서 평균 IL-6 수치가 더 높은 경향을 보였다.

대장직장암 환자를 대상으로 한 체계적 문헌고찰은 총 1편(원저 총 10편, 연구대상자 총 738명)에서 종양의 임상병리학적 특성에 따라 IL-6 수치 차이를 분석하였으며, 종양 크기가 크거나(T3-4) 원격 전이가 있거나(M1) 병기가 진행될수록(Stage III-IV) IL-6 수치가 유의하게 높았던 반면, 림프절 전이 여부(N1–2)는 유의한 차이를 보이지 않았다.

결론 및 권고결정 의료기술재평가 소위원회에서는 현재 평가결과에 근거하여 다음과 같은 의견을 제시하였다.

소위원회는 본 평가는 체계적 문헌고찰 개관 방식으로 수행되었으며, 그 결과 검색 범위와 포함 기준에 따라 일부 근거가 충분히 반영되지 않았을 가능성이 있다는 한계를 지적하였다. 이러한 점을 전제로 질환별 검토 결과가 논의되었다.

자가면역성 염증질환의 경우, IL-6 저해제가 이미 치료제로 활용되고 있어 관련 연구가 활발할 것으로 예상하였다. 그러나 이번 평가에서는 베체트병, 다카야수 동맥염, 전신홍반루푸스, 구강편평태선에 관한 근거만 확인되었고, 류마티스관절염, 대혈관 혈관염, 전신경화증과 같은 주요 질환에 대한 근거는 확인되지 않았다. 이에 따라 향후에는 이러한 질환을 대상으로 한 심층 문헌고찰을 통해 별도의 평가가 필요하다는 의견을 제시하였다.

종양성 질환의 경우, 난소암과 대장직장암을 대상으로 한 2편의 연구만 확인되었으며, 두 연구 모두 IL-6와 비교검사의 활동성 평가 결과를 제시하지 않아 그 근거가 제한적이고 해석에도 한계가 있었다. 따라서 향후에는 종양성 질환을 대상으로 활동성 평가를 확인하기 위한 별도의 심층 문헌고찰이 필요하며, 이 과정에서 IL-6와 비교검사의 결과 비교가 함께 검토될 필요가 있다는 의견을 제시하였다.

2025년 제2차 재평가전문위원회*(2025.10.17.)는 임상적 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 대상 질환에 따라 다음과 같이 권고등급을 결정하였다.

i) 자가면역성 염증질환자에서 질환의 활동성 평가를 목적으로 하는 IL-6 [정밀면역검사]의 사용은 ‘권고 보류’한다. ii) 종양성 질환자에서 질환의 활동성 평가를 목적으로 하는 IL-6 [정밀면역검사]의 사용은 ‘권고 보류’한다.

Background Interleukin-6 (IL-6) [High Quality Immunoassay] is a quantitative assay used to measure IL-6 concentrations and is utilized not only for the early diagnosis of infection and sepsis but also for the assessment of disease activity in neoplastic and inflammatory diseases. The assay was first listed as a non-reimbursable item (Code No. 476) under the name “Interleukin-6” in 2001. It was subsequently reclassified in 2016 as an item subject to differential patient cost-sharing (80%), and in 2018 its nomenclature and classification code were revised to the current designation. In December 2021, it was further reclassified as a selective benefit item with a 90% patient co-payment rate. As this technology is scheduled for suitability assessment in 2026, it was selected as a target for reassessment following a demand survey and relevant review procedures. The evaluation protocol and composition of the subcommittee were deliberated and finalized at the 3rd Health Technology Reassessment Committee, convened on March 14, 2025. According to the procedure description provided in the Health Insurance Review and Assessment Service (HIRA) Medical Practice Portal, this assay is defined as being used either for the early diagnosis of infection or sepsis or for the assessment of disease activity in neoplastic and inflammatory diseases associated with elevated IL-6 levels. Accordingly, the present reassessment examined these two clinical purposes separately, and this report presents the findings specifically for the assessment of disease activity in neoplastic and inflammatory diseases.

Objective This evaluation aimed to provide evidence on the clinical effectiveness of IL-6 [High Quality Immunoassay] for assessing disease activity in neoplastic and inflammatory diseases and to determine the health technology reassessment recommendation grade for its use.

Methods Because the indication for the IL-6 assay, as described in the HIRA procedure description, is stated as the “early diagnosis of infection or sepsis or the assessment of disease activity in neoplastic or inflammatory diseases associated with elevated IL-6,” the scope of the assessment was not considered appropriate to restrict to a single specific disease. Accordingly, instead of conducting a conventional systematic review (SR), this evaluation adopted an Overview of Systematic Reviews (Overview of SR) approach to comprehensively identify research trends across infection, sepsis, neoplastic diseases, and inflammatory diseases within a predefined period. However, the present report focused specifically on evidence related to the assessment of disease activity in neoplastic and inflammatory diseases. The evaluation methodology was finalized through discussions by the IL-6 [High Quality Immunoassay] Subcommittee, which consisted of seven experts: two infectious disease specialists, one laboratory medicine specialist, one pulmonologist, one pediatric pulmonologist, one rheumatologist, and one evidence-based medicine specialist. The key question of this evaluation was: “Is IL-6 [High Quality Immunoassay] clinically effective for assessing disease activity in neoplastic or inflammatory diseases?” The target population included patients with neoplastic diseases and patients with prototypical inflammatory diseases. Conditions in which inflammation is involved in the pathophysiology but that are not classified as prototypical inflammatory diseases were excluded. Although Coronavirus Disease 2019 (COVID-19) is an infectious disease, it was analyzed in consideration of its substantial use in clinical practice and repeated requests for evaluation; however, the findings were presented only in the appendix and were excluded from the main conclusions. For patients with neoplastic diseases, comparator tests included tumor necrosis factor-alpha (TNF-α), interleukin-2 (IL-2), and soluble interleukin-2 receptor (sIL-2R). For patients with inflammatory diseases, comparator tests included C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR). Reference standards included disease severity and activity criteria used in the individual studies without restriction, and health outcomes were primarily evaluated in relation to disease activity and severity, including tumor stage in neoplastic diseases. A literature search was conducted based on the key question in three international databases (Ovid MEDLINE, Ovid EMBASE, and the Cochrane Central Register of Controlled Trials) and two domestic databases (KoreaMed and the Korean Education and Research Information Service [RISS]), with the final search performed on May 1, 2025. To reflect recent research trends, systematic reviews published from 2010 onward were included. Study selection based on the inclusion and exclusion criteria and the risk-of-bias assessment were independently performed by two reviewers, and disagreements were resolved by consensus with a third reviewer. The risk of bias of the final included studies was assessed using A Measurement Tool to Assess Systematic Reviews 2 (AMSTAR 2). Data were extracted using a pre-specified extraction form and qualitatively reviewed. To synthesize evidence on the utility of the IL-6 assay for assessing disease activity in neoplastic and inflammatory diseases, the disease spectrum covered by the included systematic reviews and their primary studies was re-examined, the unit of analysis was redefined, and the evidence was synthesized separately for autoimmune inflammatory diseases and neoplastic diseases. Based on the findings reviewed by the subcommittee, the final health technology reassessment recommendation grade was determined by the Health Technology Reassessment Expert Committee following deliberation.

Results A total of six systematic reviews were included in this evaluation, comprising 54 primary studies and 5,503 participants overall. Of these, four systematic reviews addressed autoimmune inflammatory diseases and two addressed neoplastic diseases. Risk-of-bias assessment showed that many studies did not clearly report the reasons for excluded literature and insufficiently considered risk of bias and publication bias. Accordingly, the overall certainty of evidence was judged to be very low. Four systematic reviews related to autoimmune inflammatory diseases were included, involving patients with Behçet’s disease, Takayasu arteritis, systemic lupus erythematosus (SLE), and oral lichen planus, respectively. All of these reviews reported findings for IL-6 as a standalone test, and none included direct comparisons with comparator tests. In the systematic review of Behçet’s disease (4 primary studies; 169 participants), the pooled standardized mean difference (SMD) for IL-6 levels according to disease activity was 1.29 (95% confidence interval [CI], 0.06 to 2.53; I² = 94.6%), indicating significantly higher IL-6 levels in patients with higher disease activity. In the systematic review of Takayasu arteritis (7 primary studies; 1,611 participants), comparison of IL-6 levels between active and inactive disease yielded a pooled weighted mean difference (WMD) of 16.69 pg/mL (95% CI, 5.32 to 28.06), indicating significantly higher IL-6 levels in the active disease group. In the systematic review of SLE (13 primary studies; 1,295 participants), the pooled SMD for the difference in IL-6 levels according to disease activity was 1.10 (95% CI, 0.72 to 1.48; I² = 79.2%), showing significantly higher IL-6 levels in the active disease group. A significant positive correlation was also observed between IL-6 levels and disease activity (r = 0.36). In the systematic review of oral lichen planus (4 primary studies; 259 participants), the pooled SMD for the difference in IL-6 levels between erosive and non-erosive forms was 1.37 (95% CI, −0.26 to 3.00; I² = 94%), indicating no statistically significant difference. Two systematic reviews were identified for the assessment of disease activity in neoplastic diseases, one in ovarian cancer and the other in colorectal cancer. In the systematic review of ovarian cancer (20 primary studies; number of participants not reported), IL-6 levels were compared between early-stage disease (Stages I–II) and advanced-stage disease (Stages III–IV) according to the International Federation of Gynecology and Obstetrics (FIGO) staging system. Mean IL-6 levels tended to be higher in patients with advanced-stage disease. In the systematic review of colorectal cancer (10 primary studies; 738 participants), differences in IL-6 levels were analyzed according to clinicopathological characteristics. IL-6 levels were significantly higher in patients with larger tumor size (T3–4), distant metastasis (M1), and more advanced stage (Stage III–IV), whereas no significant difference was found according to lymph node metastasis status (N1–2).

Conclusions and Recommendation Based on the current findings, the Health Technology Reassessment Subcommittee presented the following opinions. The subcommittee noted that this evaluation was conducted as an Overview of Systematic Reviews and therefore may have been subject to the limitation that some relevant evidence was not fully captured depending on the search scope and inclusion criteria. The disease-specific findings were discussed in light of this limitation. For autoimmune inflammatory diseases, the subcommittee had anticipated that a substantial body of research would be available, given that IL-6 inhibitors are already used therapeutically in clinical practice. However, the present evaluation identified evidence only for Behçet’s disease, Takayasu arteritis, SLE, and oral lichen planus, whereas no evidence was confirmed for major diseases such as rheumatoid arthritis, large-vessel vasculitis, and systemic sclerosis. The subcommittee therefore suggested that separate in-depth literature reviews should be conducted for these diseases in future evaluations. For neoplastic diseases, only two studies involving ovarian cancer and colorectal cancer were identified. Because neither study presented comparative findings between IL-6 and comparator tests for the assessment of disease activity, the available evidence was considered limited and difficult to interpret. The subcommittee therefore suggested that a separate in-depth literature review focused on neoplastic diseases should be undertaken in the future, with comparative evaluation of IL-6 and relevant comparator tests. At the 2nd Health Technology Reassessment Expert Committee meeting held on October 17, 2025, after comprehensive consideration of the evidence on clinical effectiveness together with other evaluation domains, the recommendation grades for use in the Korean clinical setting were determined as follows according to the target disease category: i) The use of IL-6 [High Quality Immunoassay] for the assessment of disease activity in patients with autoimmune inflammatory diseases is assigned a Deferred Recommendation. ii) The use of IL-6 [High Quality Immunoassay] for the assessment of disease activity in patients with neoplastic diseases is assigned a Deferred Recommendation.
