복강내 온열 항암화학요법-결장직장암 기원 복막암종증

Abstract

평가배경 복강내 온열 항암화학요법(hyperthermic intraperitoneal chemotherapy, HIPEC)은 복막암종증 환자를 대상으로 종양감축술(cytoreductive surgery, CRS)을 시행하여 육안적 병소 제거 후, 미세병소(잔류종양)를 제거하기 위해 고온의 항암제를 일정 시간 동안 복강내에 관류한 후 배액하는 치료이다. 동 기술은 2013년 신의료기술로 인정되어 고시된 이후, 2014년 사용 대상 및 방법의 확대를 반영하여 개정 고시되었으며, 2021년 선별급여 50%로 등재되었다. 이후 선별급여 적합성 평가주기(5년)가 도래되어 유관기관의 수요조사를 통해 발굴되었다. 본 평가는 다양한 대체 의료기술이 존재하는 현시점에서 결장·직장암 기원 복막암종증 환자를 대상으로 기존기술 대비 동 기술의 안전성 및 효과성 등에 대한 최신 근거를 확인하고자, 2025년 제2차 의료기술재평가위원회(2025.2.14.)에서 의료기술의 평가계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다.

평가목적 본 평가의 목적은 결장·직장암 기원 복막암종증 환자에서 종양감축술 후 복강내 온열 항암화학요법(CRS+HIPEC)의 임상적 안전성 및 효과성 등에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 CRS+HIPEC의 안전성, 효과성 및 경제성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “복강내 온열 항암화학요법에 대한 안전성 및 효과성 평가 공동 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 산부인과 1인, 대장항문외과 1인, 위장관외과 1인, 혈액종양내과 2인, 소화기내과 1인, 근거기반의학 1인, 총 7인의 전문가로 구성하였다. 본 평가의 핵심질문은 “결장·직장암 기원 복막암종증 환자에서 CRS+HIPEC은 생존 향상 등의 목적으로 사용 시 임상적으로 안전하고 효과적인가?”이다. 소위원회는 대상환자와 관련하여 건강보험심사평가원 암질환심의위원회(2025.3.18.)에서 고시한 HIPEC 시 허가초과 항암요법의 인정 범위에 따라 5개 복막암(난소암 및 결장·직장암 기원 복막암종증, 충수점액종양 기원 복막가성점액종, 악성 복막중피종, 결합조직형성 소원형세포종양)으로 제한하여 평가하였다. 중재법은 CRS+HIPEC로 정의하였으며, 항암제 종류는 제한하지 않았다. 비교법은 CRS, 전신 항암치료, 완화/보존요법(최선지지요법, 완화수술, 2차 추시개복술)을 포함하였다. 안전성은 수술 관련 부작용 및 합병증, 30일 이내 사망 지표로, 효과성의 경우 1차 의료결과로 전체생존, 무진행생존, 무질병생존, 무재발생존 등의 생존 관련 지표로, 2차 의료결과는 재수술, 삶의 질로 설정하였다. 경제성은 점증적 비용효과비(incremental cost effectiveness ratio, ICER) 등을 지표로 설정하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, Cochrane Central Register of Controlled Trials), 국내 2개(KoreaMed, 한국교육학술정보원) 데이터베이스에서 검색하였다(최종검색일 2025.3.28.). 최종 선택된 연구의 비뚤림위험 평가는 연구설계에 따라 무작위배정 비교임상시험의 경우 Cochrane의 Risk of Bias (RoB)를, 그 외 연구는 비무작위 연구 비뚤림위험 평가도구 Risk of Bias for Nonrandomized Studies (RoBANS ver. 2.0)를 사용하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 사전에 정한 서식을 활용해 자료를 추출하고 정량적 합성이 가능한 경우 메타분석을 수행하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과, 최종 선택연구는 20편이었다. 결과유형에 따라 안전성 및 효과성을 보고한 연구는 18편, 경제성을 보고한 연구는 2편이었다. 안전성 및 효과성 선택연구의 특성(18편)은 연구유형별로 무작위 비교임상시험 2편, 비무작위 비교연구 16편(전향 코호트 연구 3편, 후향 코호트 연구 11편, 환자대조군 연구 2편)이었다. 대상환자는 결장·직장암을 원발암으로 하는 복막 전이 환자였다. 중재법과 관련하여 CRS는 적용 시점에 따라 진단 직후 1차 수술(primary cytoreductive/debulking surgery, P(C)DS), 선행항암화학요법 이후 간격 수술(interval cytoreductive/debulking surgery, I(C)DS), 초치료 이후 재발 수술(secondary cytoreductive surgery)로 다양하게 수행되었다. CRS 이후 종양 감축(completeness of cytoreduction, CC) 정도는 완전 종양감축(CC0-1 또는 R0-1) 9편, 불완전 종양감축(CC2-3 또는 R2 이상) 8편, 보고하지 않음 1편이었다. 비교법 유형(중복포함)은 CRS 9편, 전신(완화) 항암치료 9편, 그 외 최선지지요법 2편이었으며, 완화수술, 2차 추시개복술은 각 1편이었다. 비뚤림위험 평가 결과, RoB는 연구참여자에 대한 눈가림(50.0%) 영역의 비뚤림위험을 ‘높음’으로 평가하였다. RoBANS ver 2.0은 대상군 비교가능성(18.8%), 대상군 선정(31.3%), 불완전한 결과자료(12.5%) 영역의 비뚤림위험을 ‘높음’으로 평가하였다. 경제성을 보고한 연구는 2편이며, 모두 국외 수행 연구로 전신(완화) 항암치료와 비교하였다.

안전성 안전성 결과를 보고한 연구는 6편(무작위 비교임상시험 1편, 비무작위 비교연구 5편)이었다. 수술 관련 전체 이상반응을 보고한 연구는 1편이었다. CRS 단독과 비교한 비무작위 비교연구 1편으로, 중재군에서 낮은 경향을 보였으나, 군 간 유의성을 보고하지 않았다. 수술 관련 중대한 이상반응을 보고한 연구는 5편이며, 모두 CRS 단독과 비교하였다. 무작위 비교임상시험 1편이며, 술 후 30일 시점에서 중대한 이상반응은 중재군에서 높은 경향을 보였으나, 군 간 차이는 없었다. 술 후 60일 시점에서 늦은 중대한 이상반응 발생은 중재군에서 높았다. 주요 중대한 합병증 양상으로 소화관 누공, 복강내 농양 등의 복강내 합병증과 호중구 및 혈소판 감소증과 같은 혈액학적 이상 등을 보고하였다. 비무작위 비교연구는 4편이었다. 이 중 1편은 중재군에서 높았으며, 나머지 3편은 군 간 차이가 없었다. 메타분석 숲그림을 확인한 결과, 중대한 이상반응 발생 위험은 군 간 차이가 없었다. 수술 관련 30일 이내 사망을 보고한 연구는 4편이며, 모두 CRS 단독과 비교하였다. 무작위 비교임상시험은 1편이며, 30일 이내 사망은 각 군당 1.5%로 두 군에서 동일하게 발생하였다. 비무작위 비교연구는 3편이었다. 이 중 1편은 두 군에서 사망 사건이 발생하지 않았다. 다른 1편은 CRS 단독군에서 6.3% 발생하였다. 나머지 1편은 중재군에서 0.9% 발생하였고, CRS 단독군의 사망 사건은 보고하지 않았다.

효과성 효과성 결과를 보고한 연구는 18편(무작위 비교임상시험 2편, 비무작위 비교연구 16편)이었다. 중앙 전체생존기간(전체사망 위험)을 보고한 연구는 16편이었다. CRS 단독과 비교한 연구는 8편으로, 이 중 무작위 비교임상시험은 1편이며, 중앙 전체생존기간 및 전체사망 위험은 군 간 차이가 없었다. 비무작위 비교연구는 7편이며, 이 중 4편은 군 간 차이가 없었다. 나머지 3편 중 2편은 중재군에서 좋았으며, 1편은 비교군에서 좋았다. 3편을 메타분석한 결과, 전체사망 위험이 중재군에서 낮았다(hazard ratio, HR=0.63, 95% confidence interval, CI 0.45-0.86, I2=0%, p=0.004). 완화(전신) 항암치료 단독과 비교한 연구는 9편이었다. 이 중 무작위 비교임상시험은 1편이며, 중앙 전체생존기간 및 질병특이생존기간은 중재군에서 좋았다. 비무작위 비교연구는 8편이었다. 이 중 5편은 중재군에서 좋았으며, 나머지 3편은 군 간 차이가 없거나 보고하지 않았다. 2편을 메타분석한 결과, 전체사망 위험이 중재군에서 낮았다(HR=0.32, 95% CI 0.18-0.57, I2=10%, p=0.0001). 완화 수술 단독과 비교한 연구는 비무작위 비교연구 1편이었다. 중재군에서 좋은 경향을 보였으나 군 간 유의성을 보고하지 않았다. 최선지지요법 단독과 비교한 연구는 비무작위 비교연구 2편이었다. 2편 모두 중재군에서 좋은 경향을 보였으나, 1편에서만 군 간 차이가 있었다. 특정 시점에서 전체생존율을 보고한 연구는 8편이었다. CRS 단독과 비교한 연구는 5편이었다. 무작위 비교임상시험은 1편이며, 5년 전체생존율은 중재군에서 높은 경향을 보였으나, 군 간 차이를 보고하지 않았다. 비무작위 비교연구는 4편이었다. 이 중 2편은 특정 시점에서 전체생존율은 중재군에서 높은 경향을 보였으며, 나머지 2편은 CRS 단독군에서 높은 경향을 보였으나, 4편 모두 군 간 차이가 없거나 유의성을 보고하지 않았다. 5년 전체생존율을 보고한 2편을 메타분석한 결과, 5년 시점에서 사망 위험은 군 간 차이가 없었다(risk ratio, RR=1.01, 95% CI 0.14-7.56, I2=69%, p=0.95). 완화(전신) 항암치료 단독과 비교한 연구는 비무작위 비교연구 3편이었다. 3편 모두 특정 시점에서 전체생존율은 중재군에서 높은 경향을 보였으나 이 중 1편에서만 군 간 차이가 있었다. 5년 전체생존율을 보고한 2편을 메타분석한 결과, 5년 시점에서 사망 위험은 군 간 차이가 없었다(RR=0.64, 95% CI 0.04-9.68, I2=92%, p=0.28). 최선지지요법 단독과 비교한 연구는 비무작위 비교연구 1편이었다. 특정 시점에서 전체생존율은 중재군에서 높은 경향을 보였으나, 군 간 차이를 보고하지 않았다. 2차 추시개복술 단독과 비교한 연구는 비무작위 비교연구 1편이었다. 특정 시점에서 전체생존율은 2차 추시개복술 단독군에서 높은 경향을 보였으나, 군 간 차이를 보고하지 않았다. 중앙 무진행생존기간(질병진행 및 사망 위험)을 보고한 연구는 3편이었다. CRS 단독과 비교한 연구는 비무작위 비교연구 2편이었다. 이 중 1편은 중재군에서 좋은 경향을 보였으나, 군 간 차이는 없었다. 나머지 1편은 질병진행 및 사망 위험이 중재군에서 낮았다(HR=0.39, 95% CI 0.39-0.98, p=0.04). 완화 항암치료 단독과 비교한 연구는 무작위 비교임상시험 1편으로, 중앙 무진행생존기간이 중재군에서 좋았다. 특정 시점의 무질병생존율을 보고한 연구는 1편이었다. 2차 추시개복술 단독과 비교한 비무작위 비교연구 1편이었다. 3년 무질병생존율은 비교군에서 높은 경향을 보였으나, 군 간 차이는 없었다. 중앙 무재발생존기간을 보고한 연구는 2편이었다. 모든 연구는 CRS 단독과 비교하였다. 무작위 비교임상시험은 1편이며, 중앙 무재발생존기간은 중재군에서 좋은 경향을 보였으나, 군 간 차이는 없었다. 비무작위 비교연구는 1편으로 중앙 무재발생존기간은 중재군에서 좋았다. 특정 시점에서 무재발생존율을 보고한 연구는 3편이었다. 모든 연구는 CRS 단독과 비교하였다. 무작위 비교임상시험 1편이며, 특정 시점에서 무재발생존율은 중재군에서 높은 경향을 보였으나, 군 간 유의성을 보고하지 않았다. 비무작위 비교연구는 2편이며, 2편 모두 특정 시점에서 무재발생존율이 중재군에서 높았다. 재수술률을 보고한 연구는 3편이었다. 모두 CRS 단독과 비교한 비무작위 비교연구 3편이었다. 재수술률은 각 군당 18% 미만이었으며, 군 간 차이가 없거나 유사하였다. 재수술 사유로는 수술 후 합병증과 종양 재발 등을 보고하였다. 삶의 질을 보고한 연구는 없었다.

경제성 경제성을 보고한 연구는 2편이었다. 전신(완화) 항암치료 단독과 비교한 2편 중 1편은 무작위 비교임상시험을 포함한 임상연구와 Medicare/ Medicaid 비용자료를 활용하여 미국 사회적 관점에서 수행되었다. 본 연구는 Markov 모형을 적용하여 7년의 분석 기간과 3% 할인율을 사용하여 분석하였다. 분석 결과, 중재군의 총 비용은 $64,499로 전신 항암치료군 $32,637 대비 증가하였고, 추가 생존연수(life year saved, LYs)는 1.19로 전신 항암치료군 0.76 대비 0.43 증가하였다. 이에 따른 ICER는 74,098$/LYs였다. 또한 질보정생존년(quality-adjusted life year, QALY)은 중재군 0.86, 전신 항암치료군 0.51로 0.35 증가하였으며, ICER는 91,034$/QALY였다. 확률적 민감도 분석결과, CRS+HIPEC은 미국의 지불의사금액(willingness to pay) 100,000$/QALY 기준에서 87%의 확률로 비용 효과적인 치료로 보고하였다. 다른 1편은 싱가포르 국립암센터 데이터베이스(1999–2014)를 활용하여 싱가포르 보건의료체계 관점에서 수행되었다. 본 연구는 별도의 경제성 분석 모형은 적용하지 않고 실제 추적관찰 자료를 기반으로 분석하였다. 중재군의 환자 1인당 평균 비용은 S$83,680.26, 중앙 전체생존기간은 47개월이었다. 이에 따른 생존연수당 비용(cost per life year)은 21,365.19S$/LY였다. 완화 항암치료군의 평균 비용은 S$44,478.87, 중앙 전체생존기간은 9개월이었으며, 생존연수당 비용은 59,305.19S$/LY였다. 종합하면, CRS+HIPEC은 결장·직장암 복막암종증 환자 중 선별된 환자에서 생존기간을 연장하며, 기존 완화 항암요법에 비해 생존연수당 비용이 더 낮은 치료로 보고하였다.

결론 및 권고결정 의료기술재평가 소위원회에서는 현재의 평가 결과에 근거하여 다음과 같이 제언하였다. 결장·직장암 기원 복막암종증 환자에서 CRS 단독과 비교하여, CRS+HIPEC의 안전성은 중대한 합병증 발생이 4.2~42.1% 범위로 높은 편이었으나, 다수의 연구에서 군 간 차이가 없거나 유사한 경향이었다(근거수준: 낮음 또는 매우 낮음). 주요 중대한 합병증은 소화관 누공, 복강내 농양 등의 복강내 합병증과 호중구 및 혈소판 감소증과 같은 혈액학적 이상으로 보고하였다. 30일 이내 사망률은 약 6% 이하로 군 간 차이가 없거나 유사한 경향이 있었다(근거수준: 낮음 또는 매우 낮음). 효과성은 전체생존, 무진행 및 무재발생존 관련 결과에 있어 무작위 비교임상시험에서는 군 간 통계적 유의성이 일관되지 않았으며(근거수준: 낮음), 비무작위 비교연구에서는 결과의 방향성이 일관적이지 않았다(근거의 확신도: 매우 낮음). 그 외 재수술률은 군 간 유사한 경향이 있었다(근거의 확신도: 매우 낮음). 완화 항암치료 단독과 비교하여 안전성을 보고한 연구는 없었다. 효과성은 전체생존(질병특이생존) 및 무진행생존기간은 무작위 비교임상시험에서 중재군이 더 긴 경향을 보였으며, 다수의 비무작위 비교연구에서도 중재군에서 긍정적인 결과를 보였다(근거수준: 낮음 또는 매우 낮음). 그 외 5년 전체생존율은 중재군에서 높은 경향을 보였으나 군 간 유의성이 일관되지 않았다(근거수준: 매우 낮음). 경제성은 국외연구 2편에서 중재가 비용 효과적인 치료일 가능성이 높다고 보고하였다. 그 외 보존요법과 비교하여 안전성을 보고한 연구는 없었다. 효과성은 완화수술과 비교하여 전체생존기간은 중재군에서 높은 경향을 보였다(근거수준: 매우 낮음). 최선지지요법과 비교하여 전체생존기간과 5년생존율은 중재군에서 높은 경향을 보였다(근거수준: 매우 낮음). 2차 추시개복술과 비교하여 5년 생존율과 3년 무질병생존율은 군 간 차이가 없었다(근거수준: 매우 낮음). 그러나 이러한 보존요법과의 비교 결과는 포함된 연구 수가 부족하여 효과성을 판단할 만한 근거가 불충분하였다. 소위원회는 결장·직장암 기원 복막암종증 환자에서 CRS+HIPEC로 인한 중대한 합병증 발생은 비교적 높은 편에 해당한다고 보았다. 보고한 합병증은 광범위한 복부 장기 절제를 동반하는 종양감축술과 함께 고온 항암제 복강관류요법을 병행하는 고난도 치료 과정에서 기인할 수 있는 합병증으로 판단하였다. 이러한 합병증은 패혈증, 전신성 염증반응, 다장기부전 등으로 악화할 잠재적 위험이 있으나, 이를 HIPEC의 특이적 위해성으로 보기는 어렵다고 판단하였다. 따라서, 중재는 중등도 이상(grade II-IV)의 합병증 위험이 내재된 고강도 치료로서, 위험과 생존 이득 간의 균형적인 측면에서 신중한 적용이 필요하겠다. 다만, 적절한 환자 선정과 숙련된 술기, 관리 가능한 전문기관에서 시행될 경우에는 임상적으로 허용 가능한 범위 내의 안전성이 있는 치료로 평가하였다. 안전성 측면에서 전신 항암치료와의 직접 비교 결과가 제한적인 이유는 대부분의 연구가 생존 결과에 초점을 두고 수행되어, 수술과 항암치료 간 합병증 발생 양상에 대한 정보를 충분히 보고하지 않은 것으로 해석하였다. 효과성 측면에서 CRS 단독과 비교하여 생존 관련 결과의 방향성은 일관되지 않거나 군 간 차이가 없었다. 특히 대규모 다기관 무작위 비교임상시험인 PRODIGE-7 (Quenet et al., 2021)는 옥살리플라틴 기반 HIPEC을 적용하였으나, 생존 관련 유의한 이득이 관찰되지 않았다. 이는 다수의 환자가 이미 FOLFOX 요법을 포함한 옥살리플라틴 기반 전신 항암치료를 선행하여 약제 내성이 생겼을 가능성이 높아 중재 효과가 과소 평가되었을 가능성이 있는 것으로 판단하였다. 따라서 향후에는 종양부하(복막암지수), 잔존종양(절제) 정도, 환자의 전신상태 등의 예후 요인을 종합적으로 고려한 적절한 환자 선별이 필수적이며, 약제 선택 및 투여 시간, 온도 등 중재 프로토콜의 확립이 필요할 것으로 제언하였다. 완화 항암치료 단독과 비교한 경우는 과반수의 연구에서 전체생존 및 무진행생존기간이 중재군에서 더 긴 경향을 보였고, 생존 이득의 가능성이 일부 확인되나 대부분의 연구가 비무작위 비교연구로, 여전히 결과의 불확실성이 존재하는 것으로 평가하였다. 또한 경제성 측면에서 중재는 완화 항암치료와 비교하여 초기 비용 부담은 크지만 장기 생존 이득을 고려할 때, 비용 효과적인 가능성이 높은 치료로 판단하였다. 다만, 이러한 결과는 국외 연구에 근거한 것으로 국내 보건의료체계와 비용 구조의 차이를 고려하여 해석할 필요가 있다. 그 외 보존요법과의 비교 결과는 근거가 제한적이며, 향후 추가 연구가 필요하다고 제언하였다. 2025년 제3차 재평가전문위원회*(2025.11.14.)는 소위원회 결론 및 분과의견을 검토하여 다음과 같이 심의·의결하고 권고등급을 결정하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상 상황에서 결장·직장암 기원 복막암종증 환자 대상 생존 향상 등의 목적으로 복강내 온열 항암화학요법의 사용을 ‘권고 보류’로 결정하였다.

Background Hyperthermic intraperitoneal chemotherapy (HIPEC) is a treatment for patients with peritoneal carcinomatosis in which cytoreductive surgery (CRS) is first performed to remove all visible lesions, followed by the circulation of heated chemotherapeutic agents within the peritoneal cavity for a specified period to eliminate microscopic residual disease, after which the solution is drained. This technology was first recognized and notified as a new health technology in 2013. It was subsequently revised in 2014 to reflect the expansion of its indications and methods of use, and was later listed for conditional reimbursement at a 50% coverage level in 2021. As the five-year reassessment cycle for conditional reimbursement has elapsed, the technology was identified through a demand survey conducted by relevant institutions. In this reassessment, given the availability of various alternative medical technologies, we aimed to review the most recent evidence on the safety and effectiveness of HIPEC compared with existing treatments in patients with peritoneal metastases originating from colorectal cancer. The reassessment was conducted following deliberation on the evaluation plan and the composition of the subcommittee at the 2nd Healthcare Technology Reassessment Committee in 2025 (February 14, 2025).

Objective The objective of this assessment is to provide evidence on the clinical safety and effectiveness of CRS+HIPEC in patients with peritoneal metastases from colorectal cancer, and to determine the recommendation grade for the use of this technology through the healthcare technology reassessment.

Methods A systematic review was conducted to evaluate the safety, effectiveness, and cost-effectiveness of CRS+HIPEC. All methods were determined through deliberation by the Subcommittee in consideration of the assessment objectives. The Subcommittee comprised seven experts in obstetrics and gynecology, colorectal surgery, gastrointestinal surgery, hematology-oncology, gastroenterology, and evidence-based medicine. The key question was whether CRS+HIPEC is clinically safe and effective for improving survival in patients with peritoneal metastases from colorectal cancer. The target population was defined in accordance with the scope of approved off-label anticancer drug use for HIPEC, as specified by the Cancer Review Committee of the Health Insurance Review and Assessment Service (March 18, 2025). It included five types of peritoneal malignancies: peritoneal metastases from ovarian and colorectal cancers, pseudomyxoma peritonei arising from appendiceal mucinous tumors, malignant peritoneal mesothelioma, and desmoplastic small round cell tumor. The intervention was CRS+HIPEC without restriction on chemotherapeutic agents. Comparators included CRS alone, systemic chemotherapy, and palliative/supportive care. Safety outcomes included surgery-related adverse events, complications, and 30-day mortality. Effectiveness outcomes included overall survival, progression-free survival, disease-free survival, and recurrence-free survival as primary outcomes, and reoperation and quality of life as secondary outcomes. Cost-effectiveness was assessed using indicators such as the incremental cost-effectiveness ratio (ICER). Only comparative studies were included. Literature searches were conducted in Ovid-MEDLINE, Ovid-EMBASE, Cochrane CENTRAL, KoreaMed, and KERIS (last search: March 28, 2025). Risk of bias was assessed using the Cochrane RoB tool for randomized controlled trials (RCTs) and RoBANS 2.0 for nonrandomized studies (NRS). Two reviewers independently performed study selection and data extraction using predefined forms, and meta-analysis was conducted where appropriate. The level of evidence was assessed using the GRADE approach. The final recommendation grade was determined by the Healthcare Technology Reassessment Committee based on the Subcommittee’s review.

Results A total of 20 studies were included in the systematic review. Of these, 18 studies reported safety and effectiveness outcomes, while 2 studies evaluated cost-effectiveness. Among the studies assessing safety and effectiveness (n=18), 2 were RCTs and 16 were NRS, including 3 prospective cohort studies, 11 retrospective cohort studies, and 2 case–control studies. The study population consisted of patients with peritoneal metastases originating from colorectal cancer. CRS was performed at different time points depending on the treatment setting, including primary cytoreductive/debulking surgery (P(C)DS) at initial diagnosis, interval cytoreductive/debulking surgery (I(C)DS) following neoadjuvant chemotherapy, and secondary cytoreductive surgery for recurrent disease. The completeness of cytoreduction (CC) was reported as complete cytoreduction (CC0–1 or R0–1) in 9 studies, incomplete cytoreduction (CC2–3 or ≥R2) in 8 studies, and was not reported in 1 study. Comparator interventions (including overlapping categories) included CRS (n=9), systemic (palliative) chemotherapy (n=9), and best supportive care (n=2). Palliative surgery and second-look laparotomy were each reported in one study. Risk of bias assessment showed that, among RCTs, a high risk of bias was identified in the blinding of participants (50.0%). Among NRS assessed using RoBANS version 2.0, a high risk of bias was observed in comparability of groups (18.8%), selection of participants (31.3%), and incomplete outcome data (12.5%). Two studies evaluated cost-effectiveness, both conducted outside Korea, and both compared CRS+HIPEC with systemic (palliative) chemotherapy.

Safety Safety outcomes were reported in 6 studies (1 RCTs and 6 NRS). Evidence on surgery-related overall adverse events was limited. Only one NRS compared CRS+HIPEC with CRS alone, suggesting a lower incidence in the intervention group; however, this difference was not statistically significant. More data were available for serious adverse events (SAEs). Five studies, all comparing CRS+HIPEC with CRS alone, reported on this outcome. In the single RCT, the incidence of SAEs at 30 days tended to be higher in the intervention group, although no statistically significant difference was observed. By 60 days, late SAEs were more frequent in the intervention group. Reported complications included intra-abdominal events—such as gastrointestinal fistula and intra-abdominal abscess—as well as hematologic toxicities, including neutropenia and thrombocytopenia. Among the NRS (n=4), one study reported a higher incidence of SAEs in the intervention group, whereas the remaining studies found no meaningful differences. Pooled analysis similarly showed no significant difference in the overall risk of SAEs between groups. 30-day mortality was reported in four studies, all comparing CRS+HIPEC with CRS alone. In the RCT, 30-day mortality was identical between groups (1.5% each). Findings from the NRS were variable: one study reported no deaths in either group, one reported a 6.3% mortality rate in the CRS-alone group, and another reported a 0.9% mortality rate in the intervention group, with no deaths reported in the comparator group. Overall, there was no consistent evidence of a difference in short-term mortality between groups.

Effectiveness Effectiveness outcomes were reported in 18 studies (2 RCTs and 16 NRS). Median overall survival, including the risk of death, was reported in 16 studies. When compared with CRS alone (n=8), the single RCT found no difference between groups in either median overall survival or mortality risk. Among the NRS (n=7), findings were mixed: four studies reported no meaningful differences, two favored CRS+HIPEC, and one favored CRS alone. Nevertheless, pooled analysis of three studies demonstrated a significantly lower risk of death in the intervention group (hazard ratio [HR]=0.63, 95% confidence interval [CI] 0.45–0.86, I²=0%, p=0.004). When compared with systemic (palliative) chemotherapy alone (n=9), the RCT showed improved median overall survival and disease-specific survival with CRS+HIPEC. Among the NRS (n=8), five studies favored the intervention, while the remaining studies showed no difference or did not report statistical significance. Meta-analysis of two studies indicated a significantly lower mortality risk in the intervention group (HR=0.32, 95% CI 0.18–0.57, I²=10%, p=0.0001). Evidence comparing CRS+HIPEC with palliative surgery alone was limited to a single NRS, which suggested a favorable trend for the intervention without statistical significance. Similarly, two NRS comparing with best supportive care suggested improved survival with CRS+HIPEC, although only one study reported a statistically significant difference. Overall survival at predefined time points was reported in a total of 8 studies. When compared with CRS alone (n=5), the RCT reported a non-significant trend toward improved 5-year survival in the intervention group. Among the NRS (n=4), results were inconsistent, with some studies favoring CRS+HIPEC and others favoring CRS alone; however, none demonstrated statistically significant differences. Meta-analysis of two studies reporting 5-year outcomes showed no difference in mortality risk (risk ratio [RR]=1.01, 95% CI 0.14–7.56, I²=69%, p=0.95). When compared with systemic chemotherapy (n=3), all studies showed a favorable trend toward higher survival with CRS+HIPEC, although statistical significance was reported in only one study. Pooled analysis of two studies again showed no significant difference (RR=0.64, 95% CI 0.04–9.68, I²=92%, p=0.28). Evidence comparing CRS+HIPEC with best supportive care or second-look surgery was limited and did not demonstrate consistent or statistically significant differences. Median progression-free survival (PFS), including progression or death events, was reported in 2 studies. Two NRS comparing CRS+HIPEC with CRS alone showed inconsistent results, with one study suggesting a favorable trend and the other demonstrating a significantly lower risk of progression or death in the intervention group (HR=0.39, 95% CI 0.39–0.98, p=0.04). In contrast, the single RCT comparing CRS+HIPEC with systemic chemotherapy reported improved PFS with the intervention. Evidence on disease-free survival (DFS) was extremely limited. Only one NRS comparing CRS+HIPEC with second-look surgery reported 3-year DFS, showing a non-significant trend favoring the comparator. Median recurrence-free survival (RFS) was evaluated in two studies, both comparing CRS+HIPEC with CRS alone. The RCT showed a non-significant trend favoring the intervention, whereas the NRS reported improved outcomes with CRS+HIPEC. Three studies reported RFS at specific time points. The RCT and both NRS consistently suggested higher recurrence-free survival in the intervention group, although statistical significance was not consistently demonstrated. Reoperation rates were reported in three studies, all comparing CRS+HIPEC with CRS alone. Overall, reoperation rates were low (<18%) and comparable between groups. Reported indications included postoperative complications and tumor recurrence. No studies reported quality-of-life outcomes.

Cost-effectiveness Two studies evaluated the cost-effectiveness of CRS+HIPEC, both in comparison with systemic (palliative) chemotherapy alone. In a US-based analysis conducted from a societal perspective, CRS+HIPEC was associated with higher costs but also improved survival outcomes. Using a Markov model with a 7-year time horizon and a 3% discount rate, the study estimated total costs of $64,499 for the intervention group compared with $32,637 for chemotherapy alone. This corresponded to gains of 0.43 life-years (1.19 vs. 0.76), resulting in an ICER of $74,098 per life-year gained. Similarly, quality-adjusted life years (QALYs) increased by 0.35 (0.86 vs. 0.51), with an ICER of $91,034 per QALY. Probabilistic sensitivity analysis indicated that CRS+HIPEC had an 87% probability of being cost-effective at a willingness-to-pay threshold of $100,000 per QALY. In contrast, a study conducted in Singapore, based on real-world data from a national cancer center registry, suggested that CRS+HIPEC may offer greater survival benefits at a lower cost per life-year compared with chemotherapy alone. Without applying a formal economic model, the analysis reported a mean cost of S$83,680 per patient and a median overall survival of 47 months in the intervention group, corresponding to a cost of S$21,365 per life-year. By comparison, patients receiving chemotherapy alone had a lower mean cost (S$44,479) but substantially shorter survival (9 months), resulting in a higher cost per life-year (S$59,305).

Conclusion and Recommendation Based on the available evidence, the Subcommittee on Healthcare Technology Reassessment makes the following conclusions. When compared with CRS alone in patients with peritoneal metastases from colorectal cancer, CRS+HIPEC was associated with relatively high rates of serious complications (approximately 4.2%–42.1%); however, most studies reported no meaningful differences between groups (certainty of evidence: low to very low). The most frequently reported complications included intra-abdominal events, such as gastrointestinal fistula and intra-abdominal abscess, as well as hematologic toxicities, including neutropenia and thrombocytopenia. Despite these findings, short-term mortality remained low (≤6%) and was generally comparable between groups (certainty of evidence: low to very low). With regard to effectiveness, evidence comparing CRS+HIPEC with CRS alone was inconsistent. Randomized trials did not demonstrate consistent survival benefits (certainty of evidence: low), and findings from nonrandomized studies were heterogeneous and subject to substantial uncertainty (certainty of evidence: very low). Similarly, progression-free and recurrence-related outcomes did not show consistent or statistically robust differences. Reoperation rates were low and broadly comparable between groups (certainty of evidence: very low). Direct evidence on safety compared with systemic (palliative) chemotherapy alone was not available. However, effectiveness outcomes suggested a potential survival advantage with CRS+HIPEC. Randomized evidence indicated longer overall and progression-free survival with the intervention, and several nonrandomized studies showed similar trends; nevertheless, the overall certainty of evidence remains low to very low. Although 5-year survival tended to favor CRS+HIPEC, results were not consistently statistically significant. Evidence comparing CRS+HIPEC with other non-surgical or supportive strategies was limited. When compared with palliative surgery, CRS+HIPEC showed a trend toward improved survival (certainty of evidence: very low). Similarly, comparisons with best supportive care suggested a potential survival benefit, although the evidence was sparse and uncertain. In contrast, comparisons with second-look laparotomy did not demonstrate clear differences in long-term survival outcomes. Overall, the limited number of studies precludes firm conclusions regarding comparative effectiveness against these alternatives. Economic evidence from two international studies suggested that CRS+HIPEC may be a cost-effective strategy compared with systemic chemotherapy, despite higher upfront costs, primarily due to gains in survival. However, these findings should be interpreted with caution, given differences in healthcare systems and cost structures. The Subcommittee considered that the relatively high incidence of serious complications reflects the complexity of the intervention, which combines extensive cytoreductive surgery with intraperitoneal chemotherapy under hyperthermic conditions. These complications are largely attributable to the combined effects of major abdominal surgery and chemotherapy, rather than to HIPEC-specific toxicity. Although severe complications may progress to life-threatening conditions such as sepsis, systemic inflammatory response, or multi-organ failure, they were not judged to represent unique safety concerns specific to HIPEC. Accordingly, CRS+HIPEC should be regarded as an intensive treatment modality associated with a non-negligible risk of moderate-to-severe (grade II–IV) complications. Its use therefore requires careful consideration of the balance between potential risks and anticipated survival benefits. When applied in carefully selected patients, performed by experienced teams, and supported by appropriate perioperative management, the intervention may be considered clinically acceptable in terms of safety. From an effectiveness perspective, the lack of consistent benefit compared with CRS alone may, in part, reflect heterogeneity in patient selection and treatment protocols. Notably, the large multicenter randomized trial PRODIGE-7 (Quenet et al., 2021), which evaluated oxaliplatin-based HIPEC, did not demonstrate a survival advantage. This may be explained, at least in part, by prior exposure to oxaliplatin-based systemic chemotherapy (e.g., FOLFOX), potentially leading to drug resistance and underestimation of the treatment effect. Future research should therefore focus on optimizing patient selection—taking into account tumor burden (e.g., peritoneal cancer index), completeness of cytoreduction, and patient condition—as well as standardizing HIPEC protocols, including drug selection, duration, and temperature. Although comparisons with systemic chemotherapy suggest a possible survival benefit, the predominance of nonrandomized evidence limits confidence in these findings. Furthermore, evidence comparing CRS+HIPEC with other supportive strategies remains insufficient, highlighting the need for additional well-designed studies. At the 3rd Healthcare Technology Reassessment Expert Committee in 2025 (November 14, 2025), the Committee reviewed the Subcommittee’s conclusions and expert opinions and reached the following decision. After considering the overall balance of clinical safety, effectiveness, and other relevant factors, the Committee determined that the use of HIPEC for patients with peritoneal metastases from colorectal cancer should be deferred (recommendation withheld) in the current Korean clinical context.
