복강내 온열 항암화학요법-충수점액종양 기원 복막가성점액종

Abstract

평가배경 복강내 온열 항암화학요법(hyperthermic intraperitoneal chemotherapy, HIPEC)은 복막암종증 환자를 대상으로 종양감축술(cytoreductive surgery, CRS)을 시행하여 육안적 병소 제거 후, 미세병소(잔류종양)를 제거하기 위해 고온의 항암제를 일정 시간 동안 복강내에 관류한 후 배액하는 치료이다. 동 기술은 2013년 신의료기술로 인정되어 고시된 이후, 2014년 사용 대상 및 방법의 확대를 반영하여 개정 고시되었으며, 2021년 선별급여 50%로 등재되었다. 이후 선별급여 적합성 평가주기(5년)가 도래되어 유관기관의 수요조사를 통해 발굴되었다.

본 평가는 다양한 대체 의료기술이 존재하는 현시점에서 충수점액종양 기원 복막가성점액종 환자를 대상으로 기존기술 대비 동 기술의 안전성 및 효과성 등에 대한 최신 근거를 확인하고자, 2025년 제2차 의료기술재평가위원회(2025.2.14.)에서 의료기술의 평가계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다.

평가목적 본 평가의 목적은 충수점액종양 기원 복막가성점액종 환자에서 종양감축술 후 복강내 온열 항암화학요법(CRS+HIPEC)의 임상적 안전성 및 효과성 등에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 CRS+HIPEC의 안전성, 효과성 및 경제성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “복강내 온열 항암화학요법에 대한 안전성 및 효과성 평가 공동 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 산부인과 1인, 대장항문외과 1인, 위장관외과 1인, 혈액종양내과 2인, 소화기내과 1인, 근거기반의학 1인, 총 7인의 전문가로 구성하였다.

본 평가의 핵심질문은 “충수점액종양 기원 복막가성점액종 환자에서 CRS+HIPEC은 생존 향상 등의 목적으로 사용 시 임상적으로 안전하고 효과적인가?”이다. 대상환자는 건강보험심사평가원 암질환심의위원회(2025.3.18.)에서 고시한 HIPEC 시 허가초과 항암요법의 인정 범위에 따라 5개 복막암(난소암 및 결장·직장암 기원 복막암종증, 충수점액종양 기원 복막가성점액종, 악성 복막중피종, 결합조직형성 소원형세포종양)으로 한정하여 평가하였다. 중재법은 CRS+HIPEC로 정의하였으며, 항암제 종류는 제한하지 않았다. 비교법은 CRS, 전신 항암치료, 완화/보존요법(최선지지요법, 완화수술, 2차 추시개복술)을 포함하였다. 안전성은 수술 관련 부작용 및 합병증, 30일 이내 사망 지표로, 효과성의 경우 1차 의료결과로 전체생존, 무진행생존, 무질병생존, 무재발생존 등의 생존 관련 지표로, 2차 의료결과는 재수술, 삶의 질로 설정하였다. 경제성은 점증적 비용효과비(incremental cost effectiveness ratio) 등을 지표로 설정하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하였다.

연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 2개(KoreaMed, 한국교육학술정보원) 데이터베이스에서 검색하였다(최종검색일: 2025.3.28.). 최종 선택된 연구의 비뚤림위험 평가는 연구설계에 따라 무작위배정 비교임상시험의 경우 Cochrane의 Risk of Bias (RoB)를, 그 외 연구는 비무작위 연구 비뚤림위험 평가도구 Risk of Bias for Nonrandomized Studies (RoBANS ver. 2.0)를 사용하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며, 사전에 정한 서식을 활용해 자료를 추출하고 정량적 합성이 가능한 경우 메타분석을 수행하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하였다.

본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과

체계적 문헌고찰 결과, 최종 선택연구는 2편이었다. 결과유형에 따라 안전성 및 효과성을 보고한 연구는 2편, 경제성을 보고한 연구는 없었다. 선택연구의 특성은 2편 모두 후향 코호트 연구였다. 대상환자는 충수점액종양이 원발암인 복막가성점액종 환자로, 저·고등급 또는 인환세포 동반 고등급 점액성 복막암종증을 포함하였다. 복막암지수(peritoneal cancer index, 0-39점)는 중앙값 기준 20점 이상이었으며, 중재 직전의 질환 상태는 동일 연구 내에서 종양학적 특성이나 치료반응에 따라 일부 환자(약 30~50%)에서만 선행항암치료가 시행되었다. 중재법과 관련하여 CRS 이후 종양감축(completeness of cytoreduction, CC) 정도는 완전 종양감축(CC 0-1 또는 R0-1) 비율이 전체 대상자의 50%로 보고한 연구 1편, 불완전 종양감축(CC 2-3 또는 R2 이상) 환자를 대상으로 한 연구가 1편이었다. 비교법은 모두 CRS 단독이었으며, 수술 전후 시점으로 전신 항암치료가 병용되었다. 비뚤림위험 평가 결과, 대상군 비교가능성(50.0%) 영역을 ‘높음’으로 평가하였다.

안전성 안전성 결과를 보고한 연구는 후향 코호트 연구 1편이었다. 수술 관련 전체 이상반응을 보고한 연구는 없었다. 수술 관련 중대한 이상반응을 보고한 연구는 1편이며, CRS 단독과 비교하였다. 중대한 이상반응은 중재군에서 높은 경향을 보였으나, 군 간 차이는 없었다(odds ratio, OR=0.94, 95% confidence interval, CI 0.65-1.37). 수술 관련 30일 이내 사망을 보고한 연구는 1편으로, CRS 단독과 비교하였고, 군 간 차이가 없었다(OR=0.38, 95% CI 0.11-1.28).

효과성 효과성 결과를 보고한 연구는 후향 코호트 연구 2편이었다. 중앙 전체생존기간(전체사망 위험)을 보고한 연구는 1편이었다. CRS 단독과 비교하여, 사망 위험은 중재군에서 낮았다(hazard ratio, HR=0.83, 95% CI 0.72-0.95).

특정 시점에서 전체생존율을 보고한 연구는 2편이며, 모두 CRS 단독과 비교하였다. 2편 모두 특정 시점에서 전체생존율은 중재군에서 더 높았으며, 이 중 1편은 10년 전체생존율이 중재군에서 더 높았다. 2편을 메타분석한 결과, 5년 시점에서 전체사망 위험은 중재군에서 낮았다(risk ratio, RR=0.79, 95% CI 0.66-0.95, I2=0%).

중앙 무진행생존 관련 결과를 보고한 연구는 1편이었다. CRS 단독과 비교하여, 중앙 무진행생존기간과 3년 무진행생존율은 중재군에서 더 좋았다. 그 외 생존 관련 결과를 보고한 연구는 없었다.

재수술 및 삶의 질을 보고한 연구는 없었다.

경제성 충수점액종양 기원 복막가성점액종 환자에서 경제성을 보고한 연구는 없었다.

결론 및 권고결정 의료기술재평가 소위원회에서는 현재의 평가 결과에 근거하여 다음과 같이 제언하였다.

충수점액종양 기원 복막가성점액종 환자에서 CRS+HIPEC은 CRS 단독과 비교하여, 안전성 측면에서는 중대한 이상반응 발생 및 30일 이내 사망률이 유사하였다(근거수준: 매우 낮음). 효과성 측면에서 전체사망 위험과 생존 관련 결과는 중재군에서 더 좋은 경향을 보였다(근거수준: 매우 낮음).

소위원회는 안전성과 관련하여, 중대한 합병증 발생이 CRS 단독과 비교하여 유의한 차이가 없더라도 중재가 중등도 이상의 합병증 위험을 내재한 고강도 치료인 점과 전반적인 합병증 발생 수준이 다른 복막 전이암과 유사한 점을 감안할 때, 적절한 환자 선정과 숙련된 술기, 관리 가능한 전문기관에서 시행될 경우에는 임상적으로 허용 가능한 범위 내의 안전한 치료로 평가하였다.

효과성 측면에서 충수점액종양 기원 복막가성점액종은 복막암 중에서도 발생 빈도가 매우 드문 희귀질환으로 환자 모집의 어려움으로 인해 단일기관 또는 단기간 연구설계는 근본적인 한계가 있는 것으로 보았다. 이러한 질환의 특성을 고려할 때, 평가에 포함된 연구는 국제 복막표면종양학회(PSOGI) 레지스트리 데이터(다국적, 다기관, 전향적 수집)를 활용한 연구와 중국 단일기관에서 10년간 수행된 후향 코호트 연구로, 일관된 장기 생존 이득을 보고하였다. 또한 진료지침을 대상으로 수행된 체계적 문헌고찰 및 권고강도 분석연구(2025)에서도 중재를 표준치료로 권고(GRADE 등급: I)하고 있어, 중재는 임상적 유용성이 있는 치료로 판단하였다. 결론적으로, 점액종양의 특수한 병리학적 특성상 전신 항암치료만으로 효과가 제한적인 점, 중재의 남용 가능성이 없는 점들을 종합적으로 고려할 때, 종양부하(복막암지수), 잔존종양(절제) 정도, 전신상태 등을 고려하여 적절히 선별된 환자에서는 중재가 치료 대안으로 고려될 수 있다고 평가하였다. 다만 비교연구 수가 부족하여 향후 근거 강화를 위해 추가 연구가 필요할 것으로 제언하였다.

2025년 제3차 재평가전문위원회*(2025.11.14.)는 소위원회 결론 및 분과의견을 검토하여 다음과 같이 심의·의결하고 권고등급을 결정하였다.

재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 충수점액종양 기원 복막가성점액종 환자 대상 생존 향상 등의 목적으로 복강내 온열 항암화학요법의 사용을 ‘약하게 권고함’로 결정하였다.

Background Hyperthermic intraperitoneal chemotherapy (HIPEC) is a treatment for patients with peritoneal carcinomatosis in which cytoreductive surgery (CRS) is first performed to remove all visible lesions, followed by the circulation of heated chemotherapeutic agents within the peritoneal cavity for a specified period to eliminate microscopic residual disease, after which the solution is drained. This technology was first recognized and notified as a new health technology in 2013. It was subsequently revised in 2014 to reflect the expansion of its indications and methods of use, and was later listed for conditional reimbursement at a 50% coverage level in 2021. As the five-year reassessment cycle for conditional reimbursement has elapsed, the technology was identified through a demand survey conducted by relevant institutions. In this reassessment, given the availability of various alternative medical technologies, we aimed to review the most recent evidence on the safety and effectiveness of HIPEC compared with existing treatments in patients with pseudomyxoma peritonei from appendiceal mucinous tumors. The reassessment was conducted following deliberation on the evaluation plan and the composition of the subcommittee at the 2nd Healthcare Technology Reassessment Committee in 2025 (February 14, 2025).

Objective The objective of this assessment is to provide evidence on the clinical safety and effectiveness of CRS+HIPEC in patients with pseudomyxoma peritonei from appendiceal mucinous tumors, and to determine the recommendation grade for the use of this technology through the healthcare technology reassessment.

Methods A systematic review was conducted to evaluate the safety, effectiveness, and cost-effectiveness of CRS+HIPEC. All methods were determined through deliberation by the Subcommittee in consideration of the assessment objectives. The Subcommittee comprised seven experts in obstetrics and gynecology, colorectal surgery, gastrointestinal surgery, hematology-oncology, gastroenterology, and evidence-based medicine. The key question was whether CRS+HIPEC is clinically safe and effective for improving survival in patients with pseudomyxoma peritonei from appendiceal mucinous tumors. The target population was defined in accordance with the scope of approved off-label anticancer drug use for HIPEC, as specified by the Cancer Review Committee of the Health Insurance Review and Assessment Service (March 18, 2025). It included five types of peritoneal malignancies: peritoneal metastases from ovarian and colorectal cancers, pseudomyxoma peritonei arising from appendiceal mucinous tumors, malignant peritoneal mesothelioma, and desmoplastic small round cell tumor. The intervention was CRS+HIPEC without restriction on chemotherapeutic agents. Comparators included CRS alone, systemic chemotherapy, and palliative/supportive care. Safety outcomes included surgery-related adverse events, complications, and 30-day mortality. Effectiveness outcomes included overall survival, progression-free survival, disease-free survival, and recurrence-free survival as primary outcomes, and reoperation and quality of life as secondary outcomes. Cost-effectiveness was assessed using indicators such as the incremental cost-effectiveness ratio (ICER). Only comparative studies were included. Literature searches were conducted in Ovid-MEDLINE, Ovid-EMBASE, Cochrane CENTRAL, KoreaMed, and KERIS (last search: March 28, 2025). Risk of bias was assessed using the Cochrane RoB tool for randomized controlled trials (RCTs) and RoBANS 2.0 for nonrandomized studies (NRS). Two reviewers independently performed study selection and data extraction using predefined forms, and meta-analysis was conducted where appropriate. The level of evidence was assessed using the GRADE approach. The final recommendation grade was determined by the Healthcare Technology Reassessment Committee based on the Subcommittee’s review.

Results A total of two studies were included in the systematic review, both reporting safety and effectiveness outcomes; no studies evaluated cost-effectiveness. Both were retrospective cohort studies. The study population consisted of patients with pseudomyxoma peritonei originating from appendiceal mucinous tumors, including both low- and high-grade disease as well as high-grade mucinous peritoneal carcinomatosis with signet ring cell features. Tumor burden was generally high, with median peritoneal cancer index scores exceeding 20. Preoperative systemic chemotherapy was administered in a subset of patients (approximately 30–50%), depending on tumor characteristics and treatment response. With respect to the intervention, completeness of cytoreduction (CC) varied across studies. One study reported that approximately 50% of patients achieved complete cytoreduction (CC0–1 or R0–1), whereas the other focused on patients with incomplete cytoreduction (CC2–3 or ≥ R2). In both studies, CRS alone served as the comparator, with systemic chemotherapy administered before and/or after surgery. Risk of bias assessment indicated a high risk of bias in comparability of groups (50%).

Safety Evidence on safety was limited to a single retrospective cohort study. No studies reported surgery-related overall adverse events. The available study assessed serious postoperative complications and found a higher incidence in the CRS+HIPEC group compared with CRS alone; however, this difference was not statistically significant (odds ratio [OR]=0.94, 95% confidence interval [CI] 0.65–1.37). Thirty-day mortality was also reported in this study and did not differ significantly between groups (OR=0.38, 95% CI 0.11–1.28).

Effectiveness Effectiveness outcomes were reported in 2 retrospective cohort studies. Median overall survival was reported in one study, which demonstrated a lower risk of death in the CRS+HIPEC group compared with CRS alone (hazard ratio [HR]=0.83, 95% CI 0.72–0.95). Overall survival at specific time points was reported in both studies, each showing higher survival rates in the intervention group. One study reported improved 10-year survival with CRS+HIPEC. Pooled analysis of the two studies demonstrated a reduced risk of death at 5 years in the intervention group (risk ratio [RR]=0.79, 95% CI 0.66–0.95, I²=0%). Only one study reported progression-related outcomes, showing both longer median progression-free survival and higher 3-year progression-free survival rates with CRS+HIPEC compared with CRS alone. No studies reported outcomes related to reoperation or quality of life.

Conclusion and Recommendation Based on the available evidence, the Subcommittee on Healthcare Technology Reassessment provides the following conclusions. In patients with pseudomyxoma peritonei originating from appendiceal mucinous tumors, CRS+HIPEC demonstrated a safety profile comparable to that of CRS alone, with similar rates of serious adverse events and 30-day mortality (certainty of evidence: very low). From an effectiveness perspective, the intervention showed a consistent trend toward improved survival outcomes, including reduced mortality risk (certainty of evidence: very low). The Subcommittee considered that, although no statistically significant differences in serious complications were observed, CRS+HIPEC remains an intensive treatment modality inherently associated with a risk of moderate-to-severe complications. Given that the overall complication profile appears comparable to that observed in other peritoneal malignancies, the intervention may be regarded as clinically acceptable in terms of safety when performed in carefully selected patients, by experienced surgical teams, and within adequately resourced specialized centers. With respect to effectiveness, pseudomyxoma peritonei is an extremely rare disease, which limits the feasibility of large-scale or long-term prospective studies. The available evidence—derived from a multinational, multicenter registry of the Peritoneal Surface Oncology Group International (PSOGI) and a single-center retrospective cohort study with long-term follow-up—consistently suggested a survival benefit associated with CRS+HIPEC. Furthermore, a recent systematic review and guideline-based analysis (2025) recommended CRS+HIPEC as a standard treatment (GRADE I), supporting its clinical utility. Taken together, given the unique biological characteristics of mucinous tumors and the limited effectiveness of systemic chemotherapy alone, CRS+HIPEC may represent a reasonable therapeutic option in appropriately selected patients. Key considerations for patient selection include tumor burden (e.g., peritoneal cancer index), completeness of cytoreduction, and overall patient condition. Nevertheless, the limited number of comparative studies underscores the need for further research to strengthen the evidence base. At the 3rd Healthcare Technology Reassessment Expert Committee in 2025 (November 14, 2025), the Committee reviewed the Subcommittee’s conclusions and expert opinions and reached the following decision. After considering the overall balance of clinical safety, effectiveness, and other relevant factors, the Committee issued a weak recommendation for the use of HIPEC in patients with pseudomyxoma peritonei of appendiceal origin to improve survival in the Korean clinical context.
