언어전반진단검사-언어장애(실어증, 발달성 언어장애)

Abstract

평가배경 언어전반진단검사는 여러 질환으로 언어장애가 의심될 때, 발음장애, 언어 학습장애 여부 및 장애의 유형을 감별진단하고, 손상의 정도를 판별하고 언어치료의 계획 수립과 언어구사 능력 회복 여부에 대한 예후를 예측하기 위해 사용되는 검사이다. 본 검사는 신의료기술평가 제도 도입 이전인 2005년에 비급여로 등재되었다. 이후 비급여 보고제도 항목으로 수요조사에서 유관기관으로부터 제안된 후 관련 선정 절차를 거쳐 최종 의료기술재평가 대상으로 선정되었다. 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서는 본 기술을 신속검토 방법으로 평가하기로 결정하고, 소위원회를 구성하여 의료기술재평가를 진행하는 것으로 심의하였다.

본 평가는 언어전반진단검사의 적용 대상 및 활용되는 평가도구에 대한 근거를 검토하여, 임상 현장에서의 적정한 사용에 참고가 될 수 있는 정보를 제공하는 것을 목적으로 수행하였다. 특히, 언어장애 중 실어증 및 발달성 언어장애를 주요 대상으로 포함하여 관련 근거를 중심으로 검토를 진행하였다.

평가목적 본 평가는 언어전반진단검사의 사용 대상과 평가도구에 대한 근거를 검토하되, 특히 언어장애(실어증, 발달성 언어장애)에 초점을 맞추어 임상 현장에서의 활용에 도움이 될 수 있는 관련 정보를 제공하는 것을 목적으로 한다.

평가방법 본 평가는 언어전반진단검사의 임상적 유용성을 검토하기 위해 문헌근거, 진단 관련 법령·지침, 임상전문가 의견을 종합하는 단계적 접근을 적용하였다. 먼저, 국내 의학·재활 관련 교과서와 정신질환 진단 및 통계 편람 제5판(Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, DSM-5), 국제질병분류 제10판(International Classification of Diseases, 10th Revision, ICD-10), 한국표준질병·사인분류(Korean Standard Classification of Diseases, KCD) 등 진단 분류체계 및 관련 법령을 검토하여 본 검사 적용이 가능한 주요 질환군을 범주화하였다. 이어, 언어재활 및 재활의학 분야의 전문서적과 연구문헌을 통해 질환별로 활용 가능한 평가도구의 종류와 구성요소, 적용범위를 확인하였으며, 장애 판정 및 언어장애 등록과 관련된 제도·지침을 함께 검토하여 국내 임상 현장에서의 실제 사용 양상을 파악하였다.

또한, 평가도구의 신뢰도·타당도 등 과학적 근거를 확인하기 위해 국내 연구문헌을 우선적으로 검토하였고, 근거가 제한적인 경우 최신 해외 연구를 추가로 참고하여 질환별 평가도구의 중증도 관련성 및 평가대상자 선정기준의 활용 여부를 함께 살펴보았다. 마지막으로, 이러한 일련의 검토 결과와 법적·제도적 활용근거 및 임상전문가 의견을 종합하여 언어전반진단검사의 실제 임상적 적용 가능성과 유용성을 평가하였다.

평가결과 본 평가는 언어전반진단검사의 임상적 적용 대상을 규명하기 위하여, 관련 교과서 및 전문서적 9편과 더불어 DSM-5, ICD-10, KCD 등 주요 질환 분류체계, 보건복지부 고시 「장애정도판정기준」, 「장애인 등에 대한 특수교육법 시행령」의 특수교육대상자 선정기준을 포함한 제도적 근거를 종합적으로 검토하였다. 또한 국내 임상현황을 분석한 연구와 실어증 및 발달성 언어장애에서 사용되는 평가도구의 신뢰도·타당도 및 활용 근거를 확인한 문헌 11편을 추가로 검토하였다.

검토 결과, 언어전반진단검사가 적용되는 주요 언어장애 범주에는 실어증과 발달성 언어장애가 포함되며, 두 질환은 모두 「장애정도판정기준」에서 언어장애 유형으로 명시되어 있음을 확인하였다. 실어증은 뇌졸중, 외상성 뇌손상, 원발성 진행성 실어증 등으로 인해 발생하는 후천적 의사소통 장애로, 언어전반진단검사에는 실어증 감별을 위한 핵심 평가도구가 포함되어 있다. 국내에서 활용되는 표준화 검사는 선별 검사로 한국어판 간편 실어증 선별 검사(Frenchay Aphasia Screening Test, K-FAST), 신경언어장애 선별 검사(Screening Test for Aphasia and Neurologic-Communication Disorders, STAND), 의미유창성 검사(Semantic Verbal Fluency, SVF), 미니 실어증 선별 검사(Mississippi Aphasia Screening Test, MAST)가 있으며, 정밀 검사로는 한국어판 웨스턴 실어증 검사(Korean version of the Western Aphasia Battery, K-WAB)와 한국어판 보스턴 이름대기 검사(Boston Naming Test, K-BNT)가 널리 사용되고 있다. 이 중 K-WAB, K-BNT, K-FAST, STAND는 「장애정도심사규정」에서 실어증 장애 등록을 위한 심사 검사로 규정되어 있으며, K-WAB와 K-BNT는 건강보험 급여 항목에도 포함되어 있어 임상적 활용성이 높다.

발달성 언어장애는 아동의 발달 과정에서 말과 언어 기능 전반에 영향을 미치는 발달장애를 포괄하는 개념으로, 언어전반진단검사에는 그림어휘력 검사와 언어놀이진단 등이 포함되어 있다. 「장애정도판정기준」에서는 발달성 언어장애의 장애 정도 판정 시 학령 전 아동 수용·표현 언어발달 검사(Preschool Receptive-Expressive Language Scale, PRES)와 영유아 언어발달 검사(Sequenced Language Scale for Infants, SELSI)의 활용을 명시하고 있으며, 국민연금공단의 장애등록 심사에서도 PRES 또는 SELSI 검사결과 제출이 필수로 규정되어 있다. 임상에서는 연령대에 따라 다양한 표준화 검사가 활용되는데, 영유아기에는 한국판 영아·유아기 의사소통 발달평가(Korean MacArthur-Bates Communicative Development Inventories, K M-B CDI)와 한국판 조기 사회·의사소통 행동발달 검사(Communication and Symbolic Behavior Scales Developmental Profile, K-CSBS DP)가 사용되며, 학령전기에는 수용·표현 어휘력 검사(Receptive & Expressive Vocabulary Test, REVT), 구문문장이해 검사, 언어문제해결 검사, 한국어판 구문처리 능력 검사(Korean Oral Language Assessment of Comprehension, KOPLAC), 한국어판 구문능력 검사(Korean Oral Narrative Assessment, KONA)가 활용된다. 학령기에는 학령기 언어기능 검사(Language for School-Age Children, LSSC)가 대표적으로 사용되고 있다.

종합하면, 언어전반진단검사는 실어증과 발달성 언어장애 환자에서 감별진단, 손상 정도 평가, 치료 계획 수립 및 예후 관련 평가에 사용되고 있으며, 이러한 목적을 위해 다양한 표준화된 검사도구가 임상에서 적용되고 있다. 본 평가에서는 이러한 언어장애 범주를 중심으로 언어전반진단검사의 적용 현황과 활용 근거를 제시하였다.

결론 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같은 의견을 제시하였다.

언어전반진단검사는 언어 지연이나 기능 손상이 있는 환자에게 임상적으로 필수적인 검사로, 임상적 의사결정 전반에서 핵심적인 역할을 수행한다. 특히 언어장애 여부를 감별할 수 있어 전반적 발달지연, 지적장애, 자폐스펙트럼장애, 언어장애 등 다양한 임상 상황에서 중요한 진단 도구로 활용된다는 점이 강조되었다.

이번 평가는 주요 대상 질환 중 언어장애에 초점을 두었으며, 그 범주에는 실어증과 발달성 언어장애가 포함된다. 실어증에는 K-WAB, K-BNT, K-FAST 등 표준화된 검사도구가 사용되고 있으며, 발달성 언어장애에는 SELSI, PRES, REVT, LSSC 등 연령과 발달 단계에 따라 다양한 도구가 활용되고 있다. 이러한 도구들은 진단, 치료계획 수립, 예후 예측, 장애 정도 판정에 이르기까지 임상 전반에서 중요한 근거자료로 활용되고 있다.

다만, 실제 임상에서는 연령과 질환군에 따라 검사 목적과 사용되는 도구가 달라지며, 일부 항목은 다른 급여 검사와의 중복 문제가 존재하는 것으로 파악되었다. 이에 따라 연령별·질환군별 특성을 반영한 수가체계의 합리적 재분류 및 세분화, 그리고 이를 뒷받침할 표준화된 평가지침 마련의 필요성이 제기되었다.

대상 질환과 관련하여 난독증(dyslexia)을 포함한 학습장애의 포함 여부에 대한 논의가 있었다. 난독증은 언어기능 평가가 필요한 경우가 있었으나, DSM-5 및 ICD 분류에서는 특정학습장애(또는 특정 발달성 학습기술장애)로 규정되어 있으며 의사소통장애(또는 특정 발달성 언어 및 말장애)와 구분되고 있어 본 언어전반진단검사의 주요 대상 질환에는 포함하지 않았다. 다만, 난독증 환자들이 언어적 어려움을 동반하는 경우가 많고, 언어 전반을 진단할 수 있는 검사도구가 현재 건강보험급여·비급여목록에서는 언어전반진단검사에 한정되어 있다는 점을 고려할 때, 난독증을 포함한 학습장애도 언어전반진단검사의 대상 질환으로 활용될 수 있다는 의견이 있었다.

2025년 제11차 재평가전문위원회*(2025.11.14.)는 소위원회 결론안 중 난독증 등 학습장애 포함 여부 및 수가체계 재분류 관련 의견은 본 평가의 범위를 다소 벗어나는 것으로 판단되어, 소위원회에서 제시한 결론안을 검토·수정한 후, 아래와 같이 심의 의결하였다.

언어전반진단검사는 언어 지연 또는 기능 손상이 있는 환자에서 임상적으로 필수적인 검사로, 진단부터 치료계획 수립, 예후 예측까지 임상적 의사결정 전반에서 핵심적인 역할을 수행한다는 점이 강조되었다. 특히 동 검사는 언어장애 여부의 감별에 유용하여 전반적 발달지연, 지적장애, 자폐스펙트럼장애, 언어장애 등 다양한 임상 상황에서 중요한 진단 도구로 활용되고 있음을 확인하였다.

이번 평가는 주요 대상 질환 중 언어장애에 초점을 두어 검토하였으며, 그 범주에는 실어증과 발달성 언어장애가 포함되었다. 실어증 평가에서는 K-WAB, K-BNT, K-FAST 등 국내 표준화된 검사도구가 사용되고 있으며, 발달성 언어장애 평가에서는 SELSI, PRES, REVT, LSSC 등 연령과 발달 수준에 따라 적절히 선택될 수 있는 다양한 검사도구가 활용되고 있음을 확인하였다. 이러한 검사도구들은 진단, 치료계획 수립, 예후 예측, 장애 정도 판정 등 임상 전반에서 신뢰할 수 있는 근거자료로 기능하고 있다고 판단하였다.

Background The comprehensive language assessment is used when a language disorder is suspected in association with various conditions. It is used to differentiate speech sound disorders and language disorders, identify disorder subtypes, determine the severity of impairment, establish language therapy plans, and predict prognosis for the recovery of language function. The assessment was registered as a non-reimbursed procedure in 2005, prior to the introduction of the New Health Technology Assessment (nHTA) system. It was subsequently proposed by relevant agencies through the non-reimbursed service reporting and demand survey process and, following the applicable selection procedures, was ultimately designated as a subject for health technology reassessment. At the 5th Health Technology Reassessment Committee meeting held on May 16, 2025, it was decided that this technology would be evaluated using a rapid review approach and that a subcommittee would be formed to conduct the reassessment. This assessment was conducted to review the evidence regarding the target populations and assessment instruments used in the comprehensive language assessment and to provide information that may serve as a reference for its appropriate use in clinical practice. In particular, aphasia and developmental language disorder were included as the primary target conditions, and the review was conducted with a focus on the relevant evidence.

Objective The objective of this assessment is to review the evidence regarding the target populations and assessment instruments used in the comprehensive language assessment, with a particular focus on language disorders, including aphasia and developmental language disorder, and to provide relevant information that may assist clinical practice.

Methods A stepwise approach was applied to evaluate the clinical utility of the comprehensive language assessment by synthesizing evidence from the literature, diagnostic classification systems, relevant laws and institutional guidelines, and clinical expert opinion. First, domestic medical and rehabilitation textbooks, major diagnostic classification systems, including the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), the International Classification of Diseases, 10th Revision (ICD-10), and the Korean Standard Classification of Diseases (KCD), as well as relevant legislation, were reviewed to categorize the major disease groups to which the comprehensive language assessment may be applied. Subsequently, specialist textbooks and research literature in the fields of speech-language pathology and rehabilitation medicine were examined to identify the types, components, and scope of application of assessment instruments available for each condition. Institutional guidelines and policies related to disability rating and language disorder registration were also reviewed to understand patterns of actual use in domestic clinical settings. To verify the scientific basis of the assessment instruments, including their reliability and validity, domestic research literature was reviewed as a priority. Where the available evidence was limited, recent international studies were additionally consulted to examine the association between individual assessment instruments and disease severity, as well as their use in establishing eligibility criteria. Finally, the findings from these reviews were integrated with the legal and institutional basis and clinical expert opinion to evaluate the feasibility and utility of the comprehensive language assessment in actual clinical practice.

Results To define the target population for the comprehensive language assessment, nine relevant textbooks and specialist references were comprehensively reviewed along with institutional evidence, including the major disease classification systems (DSM-5, ICD-10, and KCD), the Ministry of Health and Welfare notification on the Criteria for Disability Rating, and the eligibility criteria for special education recipients stipulated in the Enforcement Decree of the Act on Special Education for Persons with Disabilities. In addition, 11 studies were reviewed to examine the reliability, validity, and evidence base of assessment instruments used in aphasia and developmental language disorder.

The review confirmed that aphasia and developmental language disorder are included among the major categories of language disorders to which the comprehensive language assessment applies and that both conditions are explicitly designated as language disorder subtypes in the Criteria for Disability Rating. Aphasia is an acquired communication disorder caused by conditions such as stroke, traumatic brain injury, and primary progressive aphasia, and it affects multiple domains of language function, including speaking, comprehension, reading, and writing. The comprehensive language assessment includes core instruments used to identify and differentiate aphasia. Standardized instruments currently used in Korea include the following screening tools: the Korean version of the Frenchay Aphasia Screening Test (K-FAST), the Screening Test for Aphasia and Neurologic-Communication Disorders (STAND), the Semantic Verbal Fluency test (SVF), and the Korean version of the Mississippi Aphasia Screening Test (MAST). The Korean version of the Western Aphasia Battery (K-WAB) and the Korean version of the Boston Naming Test (K-BNT) are widely used as comprehensive diagnostic instruments. Among these, K-WAB, K-BNT, K-FAST, and STAND are designated as required assessment instruments for disability registration in aphasia under the relevant disability review criteria, and K-WAB and K-BNT are also included in the National Health Insurance covered item list, reflecting their high clinical utility.

Developmental language disorder is a broad concept that encompasses developmental conditions affecting speech and language functions during childhood. The comprehensive language assessment includes tools such as the Picture Vocabulary Test and language play-based assessment tools. The Criteria for Disability Rating stipulates the use of the Preschool Receptive-Expressive Language Scale (PRES) and the Sequenced Language Scale for Infants (SELSI) for disability severity rating in developmental language disorder, and the National Pension Service also requires the submission of PRES or SELSI results as mandatory documentation for disability registration review. In clinical practice, various standardized assessment instruments are used according to age. In infancy, the Korean MacArthur-Bates Communicative Development Inventories (K M-B CDI) and the Korean Communication and Symbolic Behavior Scales Developmental Profile (K-CSBS DP) are used. In the preschool period, the Receptive and Expressive Vocabulary Test (REVT), the Korean Sentence Comprehension Test (KOSECT), the Language Problem Solving Test (TOPS), the Korean meta-pragmatic language assessment for children (KOPLAC), and the Korean Oral Narrative Assessment (KONA) are used. In school-age children, the Language Scale for School-Age Children (LSSC) is commonly used.

In summary, the comprehensive language assessment is used in patients with aphasia and developmental language disorder for differential diagnosis, assessment of impairment severity, treatment planning, and prognostic evaluation, and various standardized assessment instruments are applied in clinical practice for these purposes. This assessment presents the current utilization status and evidence base of the comprehensive language assessment, with a focus on these language disorder categories. Conclusion Based on the current assessment findings, the Health Technology Reassessment Subcommittee presented the following

conclusions. The comprehensive language assessment is a clinically indispensable evaluation for patients with language delay or functional impairment and plays a pivotal role throughout clinical decision-making. In particular, it was emphasized that the comprehensive language assessment is useful in determining whether a patient has a language disorder and therefore serves as an important diagnostic tool across a wide range of clinical situations, including global developmental delay, intellectual disability, autism spectrum disorder, and language disorder. This assessment focused on language disorders among the major target conditions, encompassing aphasia and developmental language disorder. For aphasia, standardized instruments such as K-WAB, K-BNT, and K-FAST are used. For developmental language disorder, a range of instruments, including SELSI, PRES, REVT, and LSSC, are used according to age and developmental level. These instruments provide important information for clinical decision-making throughout clinical practice, including diagnosis, treatment planning, prognostic prediction, and disability severity rating. However, it was noted that, in actual clinical practice, the purpose of assessment and the instruments used vary according to patient age and disease category and that some items overlap with other covered diagnostic tests. Accordingly, the need was raised for a rational reclassification and further stratification of the fee schedule to reflect age-specific and disease-specific characteristics, together with the development of standardized assessment guidelines to support such a system. With regard to the scope of target conditions, there was discussion as to whether learning disabilities, including dyslexia, should be included. Although assessment of language function may be required in some cases of dyslexia, it is classified as a specific learning disorder (or specific developmental disorder of scholastic skills) under DSM-5 and ICD and is distinguished from communication disorders (or specific developmental disorders of speech and language). Accordingly, dyslexia was not included among the primary target conditions of the comprehensive language assessment. Nevertheless, given that patients with dyslexia frequently present with concomitant language difficulties and that the comprehensive language assessment is currently the only instrument capable of comprehensive language diagnosis listed in the National Health Insurance covered and non-covered item list, the opinion was raised that learning disabilities, including dyslexia, may also be considered target conditions for the comprehensive language assessment. The 11th Health Technology Reassessment Expert Committee* meeting, held on November 14, 2025, determined that the subcommittee’s recommendations regarding the inclusion of learning disabilities such as dyslexia and the reclassification of the fee schedule somewhat exceeded the scope of this assessment. After reviewing and revising the subcommittee’s proposed conclusions, the Committee deliberated and resolved as follows. The comprehensive language assessment is a clinically indispensable evaluation for patients with language delay or functional impairment, and it was emphasized that this assessment plays a pivotal role throughout clinical decision-making, from diagnosis to treatment planning and prognostic prediction. It was also confirmed that the comprehensive language assessment is useful in determining whether a patient has a language disorder and is being used as an important diagnostic tool in diverse clinical situations, including global developmental delay, intellectual disability, autism spectrum disorder, and language disorder. This assessment reviewed the major target conditions with a focus on language disorders, encompassing aphasia and developmental language disorder. It was confirmed that standardized domestic assessment instruments such as K-WAB, K-BNT, and K-FAST are used for the evaluation of aphasia and that a variety of instruments, including SELSI, PRES, REVT, and LSSC, are used for the evaluation of developmental language disorder, with selection based on age and developmental level. These instruments were judged to serve as reliable assessment tools in clinical practice, supporting diagnosis, treatment planning, prognostic prediction, and disability severity rating.
