전립선 온열요법 - 전립선 비대증

Abstract

평가 배경 전립선 온열요법(Prostate Hyperthermia)은 전립선 조직에 열을 가하여 응고·괴사시키는 방법이다. 동 기술은 신의료기술평가 제도가 도입되기 이전에 급여로 등재되어 현재까지 사용되고 있다. 전립선 비대증에 적용할 수 있는 다양한 의료기술이 존재하는 현 시점에서 동 의료기술의 상대적 안전성 및 효과성을 파악하고자 내부 모니터링을 통해 재평가 대상 항목으로 발굴하였다. 2022년 제8차 의료기술재평가위원회(2022.8.12.)에서 의료기술재평가 안건으로 선정되어 재평가를 수행하였다.

평가 방법 전립선 비대증으로 인한 배뇨장애 환자에서 전립선 온열요법의 임상적 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “전립선 온열요법 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 현재 전립선 온열요법으로 임상에서 사용하고 있는 기술은 경요도 극초단파 온열요법(Transurethral microwave thermotherapy, TUMT)과 경요도 침소작술(Transurethral needle ablation, TUNA)이라는 소위원회 의견을 반영하여 전립선 온열요법의 중재를 두 가지로 한정하였다. 모든 평가방법은 평가목적을 고려하여 “경요도 극초단파 온열요법은 전립선 비대증으로 인한 배뇨장애 환자에서 안전하고 효과적인가?”와 “경요도 침소작술은 전립선 비대증으로 인한 배뇨장애 환자에서 안전하고 효과적인가?”이었고, 안전성은 ‘부작용 및 합병증’ 지표로, 효과성은 ‘증상점수, 삶의 질, 만족도, 요역동학적 효과, 전립선 용적, 전립선 특이항원’ 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였고, Cochrane의 Risk of Bias (RoB)를 사용하여 문헌의 비뚤림위험을 평가하였다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis)과 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 전립선 비대증으로 인한 배뇨장애 환자에서 전립선 온열요법의 안전성, 효과성, 경제성은 총 30편에 근거하여 평가하였다. 총 30편 중 안전성·효과성에 대한 문헌은 20편으로 이 중 경요도 극초단파 온열요법 15편(8개 연구), 경요도 침소작술 5편(3개 연구)이었다. 경제성에 대한 문헌은 13편으로 경요도 극초단파 온열요법(TUMT) 9편, 경요도 침소작술(TUNA) 4편이었다. 만성 전립선 비대증에 대한 전립선 온열요법의 결과는 연구단위로 기술하였다. 경요도 극초단파 온열요법(TUMT) 안전성·효과성에 대한 연구로 선택된 8개 연구에서의 비교시술은 모두 경요도적 전립선 절제술이었고, 본 평가에서 비교시술로 고려한 광선택적 전립선 기화술, 홀뮴레이저를 이용한 전립선 광적출술, 전립선 동맥 색전술, 일시적 요도 스텐트 삽입술, 이식형 결찰사를 이용한 전립선 결찰술과 비교한 무작위배정 비교임상시험(Randomized controlled trial, RCT)은 확인되지 않았다. 선택문헌의 종합적인 비뚤림위험은 중등도로 평가되었다. 안전성 경요도적 전립선 절제술과 비교한 경요도 극초단파 온열요법의 안전성은 전체 부작용 및 합병증, 출혈관련 합병증, 감염, 배뇨관련 합병증, 부작용 및 합병증으로 인한 추가 카테터 사용, 성기능 장애, 기타 부작용 및 합병증이 확인되었다. 전체 부작용 및 합병증은 1개 연구에서 수술 후 12개월 시점에 중재군 2.0% (2/99명), 대조군 3.7% (8/46명)에서 발생하였다고 보고하였다. 6개 연구에서 출혈관련 합병증으로 출혈·혈전, 수혈, 혈뇨, 뇌출혈을 보고하였다. 1개 연구는 중재군에서 출혈·혈전은 발생하지 않았고, 대조군에서 7.5% (3/40명) 발생하였다. 2개 연구에서 중재군에서 수혈이 대조군에 비해 유의하게 적게 발생하였다(Risk ratio (RR) 0.10, 95% CI 0.01~0.83, p=0.03, I2=0%). 혈뇨(2개 연구)는 두 군간 유의한 차이가 없었고(RR 0.43, 95% CI 0.16~1.13, p=0.09, I2=16%), 뇌출혈을 보고한 1개 연구에서 중재군에서는 발생하지 않았고, 대조군에서 1.7% (1/59명) 발생하였다. 6개의 연구에서 감염관련 합병증으로 요로감염, 부고환염, 척수염을 보고하였다. 요로감염(6개 연구; RR 1.16, 95% CI 0.72~1.88, p=0.09, I2=25%)과 부고환염(2개 연구; RR 2.11, 95% CI 0.22 ~19.87, p=0.51, I2=0%)은 두 군간 유의한 차이가 없었다. 1개 연구에서 척수염이 중재군 0% (0/80명), 대조군 2.6% (1/39명)에서 발생하였다고 보고하였다. 배뇨관련 합병증은 6개 연구에서 협착, 요실금, 절박뇨를 보고하였다. 중재군에서 요도협착(RR 0.15, 95% CI 0.03~0.69, p=0.02, I2=0%)과 요실금(RR 0.24, 95% CI 0.09~0.63, p=0.04, I2=0%)이 대조군보다 적게 발생하였고, 방광협착(RR 0.47, 95% CI 0.08~2.63, p=0.39, I2=0%), 절박뇨(RR 1.68, 95% CI 1.01~2.79, p=0.05, I2=0%)는 두 군간 차이가 없었다. 부작용 및 합병증으로 인한 추가 카테터 사용을 보고한 연구는 총 3개 연구 중 2개의 연구에서 추가 카테터를 사용한 환자수를 제시하였는데 두 군간 유의한 차이가 없었다(RR 0.81, 95% CI 0.13~5.08, p=0.83, I2=24%). 다른 한편에서 보고한 추가 카테터 사용기간은 중재군, 대조군 각각 평균 12.7일, 평균 4.1일이었다. 성기능 장애로 5개 연구에서 사정장애(4개 연구), 발기부전(3개 연구), 성생활 유지여부(2개 연구)를 보고하였다. 사정장애 4개, 발기부전은 3개, 성생활 유지여부는 2개 연구에서 보고하였다. 이를 각각 합성한 결과 사정장애(RR 0.50, 95% CI 0.23~0.08, p=0.08, I2=46%), 발기부전(RR 0.50, 95% CI 0.25~1.02, p=0.06, I2=0%)은 두 군간 유의한 차이가 없었다. 1개 연구에서 시술 후 24개월 시점에 두 군 모두 성생활을 그대로 유지하였고, 다른 1개에서는 중재군 82% (46/56명), 대조군 86% (42/49명)가 시술 후 성생활을 유지한다고 하였다. 기타 부작용 및 합병증으로 사망, 전립선암, 방광암이 보고되었지만, 1개 연구에서는 해당 사건들이 시술과 관련이 없다고 하였고, 이외 다른 연구에서는 관련성을 언급하지 않았다. 효과성 경요도적 전립선 절제술(TURP) 대비 경요도 극초단파 온열요법의 효과성은 증상점수, 삶의 질, 만족도, 요역동학적 효과(최고요속, 잔뇨량, 배뇨근압, 배뇨량), 전립선 용적, 전립선 특이항원, TURP로 전환으로 평가하였다. 각 연구들에서 시술 후 6개월부터 60개월까지 다양한 시점에서 결과를 보고하였고, 소위원회 논의를 통해 이 중 12개월 시점과 최종 시점을 중심으로 평가결과를 제시하였다. 8개 연구에서 보고한 증상점수가 12개월 시점에서 중재군에서 대조군보다 높았고(Standard mean difference (SMD) 0.67, 95% CI 0.25~1.09, p=0.002, I2=76%), 최종 시점에서도 중재군에서 높았다(SMD 0.57, 95% CI 0.21~0.94, p=0.002, I2=76%). 삶의 질(4개 연구)은 12개월 시점(MD 0.56, 95% CI 0.68~1.81, p=0.38, I2=95%)과 최종 시점(MD 0.70, 95% CI –0.23~1.64, p=0.14, I2=91%)에서 모두 두 군간 유의한 차이가 없었다. 1개 연구에서 6개월 시점에 만족도가 중재군에서 대조군에 비해 유의하게 높았다(p=0.004). 요역동학적 효과 중 7개 연구에서 보고한 최고요속은 12개월 시점에 중재군보다 대조군에서 유의하게 높았고(MD -5.71ml/s, 95% CI –8.50~-2.93, p<0.0001, I2=68%), 최종 시점에서도 중재군보다 대조군에서 유의하게 높은 것으로 나타났다(MD -6.25ml/s, 95% CI –8.94~-3.56, p<0.0001, I2=63%). 7개 연구에서 보고한 잔뇨량은 12개월 시점(5개 연구; MD 28.87ml, 95% CI 12.95~44.78, p<0.0004, I2=45%), 최종 시점(MD 29.63, 95% CI 18.69~40.57, p<0.0001, I2=22%)에서 모두 중재군보다 대조군에서 유의하게 높았다. 배뇨근압을 보고한 3개 연구 중 2개의 연구를 합성한 결과 중재군보다 대조군에서 유의하게 낮았고(MD 9.72 cmH2O, 95% CI 1.53~17.90, p=0.02, I2=31%), 배뇨량(3개 연구)은 12개월 시점(MD 6.14 ml, 95% CI –46.37~58.65, p=0.82, I2=0%)과 최종 시점(MD 0.34 ml, 95% CI –43.85~44.34, p=0.99, I2=0%)에서 모두 두 군간 유의한 차이가 없었다. 전립선 용적을 보고한 5개 연구를 합성한 결과 12개월 시점(MD 9.98 ml, 95% CI 5.19~14.77, p<0.0001, I2=23%)과 최종 시점(MD 13.73 ml, 95% CI –10.08~17.38, p<0.0001, I2=0%)에서 모두 중재군보다 대조군에서 전립선 용적이 유의하게 더 적었다. 1개의 연구에서 전립선 특이항원이 증가한 환자수를 중재군 6.3% (5/80명), 대조군 0% (0/39명)로 보고하였지만 통계적 유의성에 대한 언급은 없었다. 경제성 경요도적 전립선 절제술 대비 경요도 극초단파 온열요법의 경제성을 보고한 연구는 총 9편이었다. 이 중 4편에서는 각 나라의 비용-효과성 판단기준(threshold)을 고려했을 때, 중재군이 대조군보다 비용은 낮고 더 효과적인 우월한(dominant) 치료대안으로 보고하였고, 이 외 1편에서 비용은 중재군이 대조군보다 낮으나, 효과에 대한 불확실성이 있어 경요도 극초단파 온열요법의 비용을 낮추어야 한다고 하였다. 다른 4편에서는 두 기술의 비용을 비교하였고, 이 중 3편에서는 중재군의 소요비용이 대조군보다 낮다고 보고하였으나, 다른 1편에서는 중재군에서의 6개월 이후 기대비용이 대조군보다 높다고 하였다. 경요도 침소작술(TUNA) 안전성·효과성에 대한 연구로 선택된 3개 연구에서의 비교시술은 모두 경요도적 전립선 절제술이었고, 광선택적 전립선 기화술, 홀뮴레이저를 이용한 전립선 광적출술, 전립선 동맥 색전술, 일시적 요도 스텐트 삽입술, 이식형 결찰사를 이용한 전립선 결찰술과 비교한 RCT는 확인되지 않았다. 선택문헌의 종합적인 비뚤림위험은 중등도로 평가되었다. 안전성 전립선 비대증에서 경요도적 전립선 절제술 대비 경요도 침소작술의 안전성은 출혈관련 합병증, 감염, 배뇨관련 합병증, 성기능 장애가 확인되었다. 출혈관련 합병증은 총 3개 연구에서 보고하였다. 수혈·혈전은 1개 연구에서 두 군간 차이가 없었고, 출혈을 보고한 2개의 연구 중 1개에서 중재군 32.3% (21/65명), 대조군 100% (56/56명)가 발생하였고, 다른 1개에서는 각각 0% (0/26명), 100% (33/33명)로 발생하였다. 수혈과 출혈을 양적 합성한 결과 두 군간 차이는 없었다(RR 0.12, 95% CI 0.01~1.24, p=0.08, I2=74%). 2개의 연구에서 각각 6개월 12개월 시점에 요로감염을 보고하였고, 양적 합성결과 두 군간 유의한 차이가 없었다(RR 0.79, 95% CI 0.35~1.08, p=0.58, I2=0%). 4개의 연구에서 배뇨관련 합병증으로 협착, 요실금, 배뇨곤란을 보고하였다. 협착(2개 연구)은 두 군간 유의한 차이는 없었고(RR 0.23, 95% CI 0.04~1.31, p=0.10, I2=0%), 다른 1개에서는 중재군에서는 발생하지 않았고, 대조군에서 방광협착이 1.8% (1/58명) 발생하여 방광경부를 절개하였다. 요실금(3개 연구)은 중재군에서 대조군보다 더 많이 발생하였고(RR 0.17, 95% CI 0.05~0.65, p=0.009, I2=0%), 배뇨곤란(배뇨불안정, 긴박뇨, 배뇨시도 실패)은 1개 연구에서 중재군 55% (11/20명), 대조군 27.3% (6/22명)에서 발생하였다고 보고하였지만 통계적 유의성에 대한 언급은 없었다. 3개 연구에서 성기능 장애로 역행성 사정장애, 발기장애를 보고하였다. 역행성 사정장애(RR 0.03, 95% CI 0.00~0.19, p=0.0003, I2=0%)와 발기장애(RR 0.04, 95% CI 0.04~0.52, p=0.003, I2=0%) 모두 중재군에서 대조군에 비해 더 적게 발생하였다 기타 부작용 및 합병증으로 사망과 전립선암을 보고하였으나, 시술 관련에 대한 언급은 없었다. 효과성 전립선 비대증으로 인한 배뇨장애 환자에서 경요도적 전립선 절제술 대비 경요도 침소작술의 효과성은 증상점수, 삶의 질, 요역동학적 효과(최고요속, 잔뇨량, 배뇨근압), 전립선 용적으로 평가하였다. 각 연구들에서 시술 후 6개월부터 60개월까지 다양한 시점에서 결과를 보고하였는데, 소위원회 논의를 통해 이 중 12개월 시점과 개별 연구의 최종 시점을 중심으로 평가결과를 제시하였다. 증상점수를 보고한 3개 연구 중 2개의 연구를 합성한 결과, 12개월 시점 증상점수가 중재군이 대조군보다 유의하게 높았고(SMD 2.96, 95% CI 0.83~5.09, p=0.006, I2=94%), 최종 시점에서는 두 군간 유의한 차이가 없었다(SMD 0.87, 95% CI –0.66~2.39, p=0.27, I2=94%). 3개 연구에서 보고한 삶의 질을 합성한 결과 12개월 시점 중재군의 삶의 질이 대조군보다 유의하게 높았지만(MD 0.91, 95% CI 0.55~1.26, p<0.0001, I2=0%), 최종 시점에서는 두 군간 유의한 차이가 없었다(MD 0.38, 95% CI –0.49~1.24, p=0.39, I2=83%). 요역동학적 효과 중 최고요속은 3개 연구에서 보고하였다. 이 중 2개 연구를 합성한 결과 12개월 시점 중재군의 최고요속보다 대조군에서 더 높았고(MD –7.46 ml/s, 95% CI –10.20~-4.72, p<0.0001, I2=63%), 최종 시점에서도 대조군에서 더 높았다(MD –2.06ml/s, 95% CI –3.23~-0.88, p=0.0006, I2=84%). 배뇨후 잔뇨량(2개의 연구)은 12개월 시점(MD 51.17ml/s, 95% CI –4.30~98.05, p=0.003, I2=72%), 최종 시점(MD 28.49ml/s, 95% CI 11.20~45.77, p=0.001, I2=72%)에서 모두 중재군보다 대조군에서 적었다. 1개 연구에서 24개월 시점에 대조군(평균 36±8 cmH2O)보다 중재군(평균 71±36 cmH2O)에서 최대요속 시 배뇨근압이 더 높게 보고되었지만 나타났지만았지만 통계적인 유의성을 보고하지 않았다. 전립선 용적을 보고한 2개 연구를 양적 합성결과 중재군 전립선 용적보다 대조군에서 더 작았다(MD 10.51, 95% CI 7.18~13.84, p<0.0001, I2=46%). 경제성 경요도 침소작술의 경제성을 분석한 연구는 총 4편으로 1편에서 경요도 침조작술, 경요도적 전립선 절제술, 이식형 결찰사를 이용한 전립선 결찰술 3개 기술의 비용을 비교하였고, 다른 3편은 경요도 침조작술과 경요도적 전립선절제술의 비용을 비교하였다. 1편에서 해당 국가의 비용-효과성 판단기준(threshold)을 고려했을 때 경요도적 전립선 절제술과 비교하여 경요도 침조작술의 비용이 낮고, 효과는 높은 우월한(dominant) 치료대안으로 보고하였다. 이 외 3편에서는 두 기술의 비용을 비교하였는고 경요도 침조작술의 소요비용이 낮았다.

결론 및 제언 소위원회에서는 전립선 비대증으로 인한 배뇨장애 환자에서 전립선 온열요법에 대해 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 전립선 비대증으로 인한 배뇨장애 환자에서 전립선 온열요법의 부작용 및 합병증은 경요도적 전립선 절제술과 유사하거나 적게 발생하여 소위원회는 동 기술을 비교적 안전한 기술로 평가하였다. 증상점수, 만족도를 기준으로 볼 때, 경요도적 전립선 절제술에 비해 전립선 온열요법의 치료효과가 낮거나 유사하고, 삶의 질에서는 두 군간 차이가 없어 소위원회는 경요도적 전립선 절제술에 비해 전립선 온열요법의 효과가 낮은 것으로 평가하였다. 본 평가 결과를 바탕으로 소위원회에서는 전립선 비대증으로 인한 배뇨장애가 있는 환자에서 ‘전립선 온열요법’이 안전하나, 경요도적 전립선 절제술에 비해 효과는 열등한 기술로 판단하였다. 2023년 제7차 의료기술재평가위원회(2023.07.14.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “전립선 온열요법-전립선 비대증”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 전립선 비대증으로 인한 배뇨장애가 있는 환자에서 전립선 온열요법이 안전하나, 경요도적 전립선 절제술에 비해 효과는 열등한 기술로 ‘권고하지 않음’으로 심의하였다.

Prostate Hyperthermia Therapy-Benign Prostatic Hyperplasia

Background Prostate hyperthermia therapy, which induces coagulation and necrosis of prostate tissue through heat application, was listed as a reimbursable procedure before the implementation of the new health technology assessment system, and it remains covered by insurance. Given the range of available treatments for benign prostatic hyperplasia (BPH), this technique was identified for reassessment through internal monitoring to evaluate its relative safety and effectiveness. It was selected for reassessment at the 8th Health Technology Reassessment Committee meeting (August 12, 2022).

Method A systematic literature review was conducted to assess the clinical safety and effectiveness of prostate hyperthermia therapy in patients with urinary dysfunction secondary to BPH. All assessment methods were finalized after deliberations by the Subcommittee for Prostate Hyperthermia Therapy (hereinafter referred to as the "Subcommittee"). The Subcommittee limited the interventions for prostate hyperthermia therapy to two types currently used in clinical practice: transurethral microwave thermotherapy (TUMT) and transurethral needle ablation (TUNA). In consideration of the objectives of the assessment, the key questions were: "Is TUMT safe and effective for patients with urinary dysfunction secondary to BPH?" and "Is TUNA safe and effective for patients with urinary dysfunction secondary to BPH?" Safety was assessed based on adverse events and complications, and effectiveness was assessed based on symptom scores, pain, quality of life, satisfaction, urodynamic effects, prostate volume, and prostate-specific antigen. Based on the key questions, literature searches of three international databases and five Korean databases were conducted. Two reviewers independently selected studies based on inclusion and exclusion criteria. The risk of bias was assessed independently by two reviewers using the Cochrane's Risk of Bias (RoB) tool. Data extraction was conducted independently by two reviewers using predefined extraction forms, and disagreements were resolved through discussion with a third reviewer. Data were analyzed using quantitative analysis and qualitative review. The level of evidence from the systematic literature review was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach.

Results The safety, effectiveness, and cost-effectiveness of prostate hyperthermia therapy for patients with urinary dysfunction secondary to BPH were assessed based on 30 articles. Among these, 20 studies reported data on the safety and effectiveness of the technique, with 15 articles (8 studies) on TUMT, and 5 articles (3 studies) on TUNA. In total, 13 studies reported data on cost-effectiveness, with 9 reporting on TUMT and 4 reporting on TUNA. The results of prostate hyperthermia therapy for chronic BPH were presented on a per-study basis.

TUMT All 8 studies reporting on the safety and effectiveness of TUMT compared the technique with transurethral resection of the prostate (TURP). No RCTs comparing TUMT to other treatments such as photoselective vaporization of the prostate, holmium laser enucleation of the prostate, prostate artery embolization, temporary prostatic stent insertion, or prostatic urethral lift were identified. The overall risk of bias in the selected studies was moderate.

Safety The safety of TUMT compared with TURP was assessed based on overall adverse events (AEs) and complications, bleeding-related complications, infection, urinary complications, additional catheter use due to AEs and complications, sexual dysfunction, and other AEs and complications. The incidence of overall AEs and complications (1 study) at 12 months postoperatively was 20% in the intervention group (2/99 patients) and 3.7% in the control group (8/46 patients). A total of 6 studies reported bleeding-related complications such as bleeding, thrombosis, blood transfusion, hematuria, and cerebral hemorrhage. In 1 study, there were no cases of bleeding or thrombosis in the intervention group, with bleeding or thrombosis occurring in 7.5% of controls (3/40 patients). Additionally, 2 studies revealed that blood transfusions were significantly less frequent in the intervention group compared to the control group (risk ratio [RR] 0.10, 95% confidence interval [CI] 0.01–0.83, p=0.03, I²=0%). For hematuria (2 studies), there was no significant difference between the groups (RR 0.43, 95% CI 0.16–1.13, p=0.09, I²=16%). In one study, cerebral hemorrhage occurred in 0% of patients in the intervention group, while the incidence was 1.7% (1/59 patients) in the control group. Furthermore, 6 studies reported infection-related complications, including urinary tract infections, epididymitis, and myelitis. There was no significant difference in the incidence of urinary tract infections (RR 1.16, 95% CI 0.72–1.88, p=0.09, I²=25%) and epididymitis (RR 2.11, 95% CI 0.22–19.87, p=0.51, I²=0%) between the groups. Another study reported the occurrence of myelitis in 2.6% (1/39) of controls with no cases observed in the intervention group. Additionally, 6 studies reported urinary-related complications, including stricture, incontinence, and urgency. The intervention group had significantly fewer cases of urethral stricture (RR 0.15, 95% CI 0.03–0.69, p=0.02, I²=0%) and incontinence (RR 0.24, 95% CI 0.09–0.63, p=0.04, I²=0%) compared to the control group. There were no significant differences in bladder neck stricture (RR 0.47, 95% CI 0.08–2.63, p=0.39, I²=0%) or urgency (RR 1.68, 95% CI 1.01–2.79, p=0.05, I²=0%) between the two groups. Only 3 studies reported on additional catheter use due to AEs and complications, with 2 out of the 3 studies finding no significant difference between the groups in the number of patients requiring additional catheter use (RR 0.81, 95% CI 0.13–5.08, p=0.83, I²=24%). The other study reported a mean duration of additional catheter use of 12.7 days in the intervention group and 4.1 days in the control group. Furthermore, 5 studies reported data on sexual dysfunction, including ejaculation disorders (4 studies), erectile dysfunction (3 studies), and preservation of sex life (2 studies). When the data were synthesized, no significant differences were observed in ejaculation disorders (RR 0.50, 95% CI 0.23–1.08, p=0.08, I²=46%) and erectile dysfunction (RR 0.50, 95% CI 0.25–1.02, p=0.06, I²=0%) between the groups. Additionally, 1 study found that both groups reported a normal sex life at 24 months post-procedure, while another study found that 82% (46/56) of patients in the intervention group and 86% (42/49) of controls reported a normal sex life post-procedure. Other AEs and complications included death, prostate cancer, and bladder cancer; 1 study found that these events were not associated with the procedure, while other studies did not mention the relationship.

Effectiveness The effectiveness of TUMT compared to TURP was assessed based on symptom scores, quality of life, patient satisfaction, urodynamic outcomes (peak flow rate, post-void residual volume, detrusor pressure, voided volume), prostate volume, prostate-specific antigen levels, and conversion to TURP. The included studies reported outcomes at various time points, from 6-60 months. The Subcommittee decided to present results for the 12 month and final follow-up. In 8 studies, symptom scores at 12 months were significantly higher in the intervention group than those in the control group (standard mean difference [SMD] 0.67, 95% CI 0.25–1.09, p=0.002, I²=76%). The score at the final follow-up was also higher in the intervention group (SMD 0.57, 95% CI 0.21–0.94, p=0.002, I²=76%). A total of 4 studies assessed quality of life. At 12 months, there was no significant difference between the groups (mean difference [MD] 0.56, 95% CI -0.68–1.81, p=0.38, I²=95%). Again at the final follow-up, there was no significant difference (MD 0.70, 95% CI -0.23–1.64, p=0.14, I²=91%) between the groups. Only 1 study reported significantly higher patient satisfaction in the intervention group than that observed in the control group at six months (p=0.004). For urodynamic effects, the peak flow rate (7 studies) was significantly higher in the control group at 12 months than that in the intervention group (MD -5.71 mL/s, 95% CI -8.50 to -2.93, p<0.0001, I²=68%) and remained higher at the final follow-up (MD -6.25 mL/s, 95% CI -8.94 to -3.56, p<0.0001, I²=63%). Post-void residual volume (7 studies) was significantly lower in the intervention group than that in the control group at both 12 months (MD 28.87 mL, 95% CI 12.95–44.78, p=0.0004, I²=45%) and the final follow-up (MD 29.63 mL, 95% CI 18.69–40.57, p=0.0001, I²=22%). Detrusor pressure was reported in 3 studies. In 2 of these studies, it was significantly lower in the control group (MD 9.72 cmH₂O, 95% CI 1.53–17.90, p=0.02, I²=31%), while voided volume did not significantly differ between the groups at either the 12 month or final follow-up. Data for prostate volume reported by 5 studies were synthesized. Prostate volume was significantly lower in the control group than that in the intervention group at both 12 months (MD 9.98 mL, 95% CI 5.19–14.77, p<0.0001, I²=23%) and final follow-up (MD 13.73 mL, 95% CI -10.08–17.38, p<0.0001, I²=0%). Furthermore, 1 study reported an increase in prostate-specific antigen levels in 6.3% (5/80) of patients in the intervention group, compared to 0% (0/39) of controls, but the significance of this difference was not reported.

Cost-effectiveness A total of 9 studies reported on the cost-effectiveness of TUMT compared to TURP. Among these, 4 concluded that TUMT is the dominant treatment alternative, as it is both less costly and more effective according to the cost-effectiveness thresholds of the respective countries. Another study indicated that while TUMT is less costly than TURP, there is uncertainty about its effectiveness, suggesting the need to reduce the costs associated with TUMT. The remaining 4 studies compared the costs of the two techniques. Of these studies, 3 reported that TUMT incurred lower costs than TURP. However, 1 study found that the expected costs associated with TUMT were higher than those for TURP beyond six months post-treatment.

TUNA All 3 studies reporting on the safety and effectiveness of TUNA compared the technique with TURP. No RCTs comparing TUNA to other treatments such as photoselective vaporization of the prostate, holmium laser enucleation of the prostate, prostate artery embolization, temporary prostatic stent insertion, or prostatic urethral lift were identified. The overall risk of bias in the selected studies was moderate.

Safety The AEs and complications of TUNA included bleeding-related complications, urinary complications, and sexual dysfunction. Bleeding-related complications were reported in 3 studies. There were no significant differences in the incidence of blood transfusion and thrombosis between the groups in one study. Of 2 studies reporting data on bleeding, 1 study reported that bleeding occurred in 32.3% (21/65) of patients in the intervention group and 100% (56/56) of controls. In the other study, bleeding occurred in 0% (0/26) of patients in the intervention group and 100% (33/33) of the controls. A quantitative synthesis of transfusion and bleeding data revealed that there is no significant difference between the groups (RR 0.12, 95% CI 0.01–1.24, p=0.08, I²=74%). In 2 studies that reported urinary tract infections at 6- and 12-months post-procedure, the quantitative synthesis showed no significant difference between the groups (RR 0.79, 95% CI 0.35–1.08, p=0.58, I²=0%). Furthermore, 4 studies reported urinary-related complications, including stricture, incontinence, and difficulty urinating. Strictures (2 studies) did not significantly differ between the groups (RR 0.23, 95% CI 0.04–1.31, p=0.10, I²=0%). In another study, strictures did not occur in the intervention group, while 1.8% (1/58) of the controls developed bladder neck stenosis and underwent an incision. Urinary incontinence (3 studies) was more common in the intervention group than in the control group (RR 0.17, 95% CI 0.05–0.65, p=0.009, I²=0%). In 1 study, urinary difficulty (unstable urination, urgency, and failed voiding attempts) occurred in 55% (11/20) of patients in the intervention group and 27.3% (6/22) of controls, but statistical significance was not mentioned. Additionally, 3 studies reported sexual dysfunction, including retrograde ejaculation and erectile dysfunction. Both retrograde ejaculation (RR 0.03, 95% CI 0.00–0.19, p=0.0003, I²=0%) and erectile dysfunction (RR 0.04, 95% CI 0.04–0.52, p=0.003, I²=0%) were less common in the intervention group than in the control group. Other AEs and complications included death and prostate cancer, but the studies did not specify whether these were related to the procedure.

Effectiveness The effectiveness of TUNA compared to TURP in patients with urinary dysfunction secondary to BPH was assessed based on symptom scores, quality of life, satisfaction, urodynamic outcomes (peak flow rate, post-void residual volume, detrusor pressure), and prostate volume. The included studies reported outcomes at various time points, from 6-60 months, but the Subcommittee decided to present results for the 12 month and final follow-up. Data for symptom scores were reported in 3 studies, and results from 2 of these studies were synthesized. The symptom score was significantly higher in the intervention group than control group at 12 months (SMD 2.96, 95% CI 0.83–5.09, p=0.006, I²=94%), but there was no significant difference at the final follow-up (SMD 0.87, 95% CI –0.66–2.39, p=0.27, I²=94%). Data for quality of life reported by 3 studies were synthesized. The intervention group had a significantly higher quality of life than the control group (MD 0.91, 95% CI 0.55–1.26, p<0.0001, I²=0%) at 12 months, but there was no significant difference between the groups (MD 0.38, 95% CI –0.49–1.24, p=0.39, I²=83%) at the final follow-up. For urodynamic outcomes, peak flow rate was reported in 3 studies. Results from 2 of these studies were synthesized. At 12 months, the control group had a significantly higher peak flow rate than the intervention group (MD –7.46 mL/s, 95% CI –10.20–4.72, p<0.0001, I²=63%), and this difference persisted at the final follow-up (MD –2.06 mL/s, 95% CI –3.23–0.88, p=0.0006, I²=84%). Two studies reported post-void residual volume, with the control group having significantly lower volumes than the intervention group at 12 months (MD 51.17 mL, 95% CI –4.30–98.05, p=0.003, I²=72%) and at the final follow-up (MD 28.49 mL, 95% CI 11.20–45.77, p=0.001, I²=72%). One study reported that at 24 months, the intervention group had a higher detrusor pressure at peak flow than the control group (mean 71±36 cmH₂O vs. 36±8 cmH₂O), but statistical significance was not reported. Additionally, 2 studies assessed prostate volume. A meta-analysis showed that the control group had significantly smaller prostate volumes than the intervention group at the final follow-up (MD 10.51 ml, 95% CI 7.18–13.84, p<0.0001, I²=46%).

Cost-effectiveness In total, 4 studies analyzed the cost-effectiveness of TUNA; 1 study compared the costs of TUNA, TURP, and prostatic urethral lift (PUL). The remaining 3 studies compared the costs of TUNA and TURP. Considering the cost-effectiveness threshold of the respective country, one study reported that TUNA is the dominant treatment modality compared to TURP, meaning it has lower costs and higher effectiveness. The remaining three studies compared the costs of both techniques and reported that TUNA is less costly than TURP.

Conclusion and Recommendations Based on the current assessment results, the Subcommittee provided the following recommendations regarding prostate hyperthermia therapy for patients with urinary dysfunction secondary to BPH: The Subcommittee determined that prostate hyperthermia therapy is relatively safe, as the incidence of AEs and complications was similar to or lower than those reported for TURP. In terms of symptom scores and patient satisfaction, prostate hyperthermia therapy was found to be either less effective or comparable to TURP. There was no significant difference in quality of life between the two treatment options. Therefore, the Subcommittee determined that prostate hyperthermia therapy is less effective than TURP. Based on these findings, the Subcommittee determined that while prostate hyperthermia therapy is safe for patients with urinary dysfunction secondary to BPH, it is inferior to TURP in terms of effectiveness. Based on the Subcommittee assessment and Article 4-10 of the Health Technology Reassessment Project Management Guidelines, the following conclusions were drawn for prostate hyperthermia therapy for benign prostatic hyperplasia at the 7th 2023 Health Technology Reassessment Committee (July 14, 2023). Based on the evidence for the clinical safety and effectiveness of the technology and other evaluation criteria, the Health Technology Reassessment Committee determined that prostate hyperthermia therapy is safe but less effective than TURP, thus is “not recommended” for patients with urinary dysfunction secondary to BPH.
