만성질환에 대한 의료보장 사각지대 연구 - 잠재적 빈곤가구의 미충족의료 및 과부담의료비 실증분석 -

Abstract

현재 우리나라 의료의 보장성 강화정책은 중증질환을 중심으로 이루어지고 있는데, 그에비해 만성질환가구의 의료이용 및 가구경제의 위험성은 크게 주목받지 못하고 있는 실정이다. 이러한 문제의식 하에, 본 연구는 만성질환을 보유한 잠재적 빈곤가구(차상위와 차차상위계층)의 미충족 의료 및 과부담의료비의 발생현황을 분석함으로써 이들 가구가 경험할수 있는 의료불평등을 탐색하고자 하였다. 분석결과, 잠재적 빈곤가구는 모든 역치수준에서 일반가구(건강보험 가입가구), 의료급여수급가구와 비교하여 과부담의료비 발생가능성이 더 높아지는 것으로 나타났고, 미충족의료 역시 건강보험 가입자에 비해 약 4배 높은 것으로 나타났다. 이러한 사실은 만성질환을보유한 잠재적 빈곤가구가 최저생계비의 100%를 약간 상회함에도 불구하고 충분한 의료제도의 혜택을 받지 못하여 의료사각지대가 발생하고 경제적 부담이 가중되는 것이라 할수 있다. 이는 의료수급가구와는 또 다른 측면에서 만성질환을 보유한 잠재적 빈곤층이 의료빈곤화에 노출되기 쉽다는 점을 의미한다. 따라서 질병과 빈곤의 이중고를 겪고 있는 잠재적 빈곤층에게도 기본적인 의료권의 보장이필요하며, 이를 통해 잠재적 빈곤층과 제도권 내의 빈곤계층 및 일반계층 간의 의료불평등을 개선해야 할 것이다.

Access to comprehensive, quality health care services is important for the achievement of health equity and for increasing the quality of a healthy life for everyone. But the remaining barriers to accessing health services such as ‘lack of availability, high cost, lack of insurance coverage’ can accelerate transition to poverty especially near poor with chronic diseases. On the basis of this situation on health care, the main purpose of this study is to analyze the inequality that can result on the near poor families with chronic diseases. The study has found out that the treatment of chronic diseases causes financial burden and insufficient medical care, which especially in the near poor sector. Therefore to prevent catastrophic health expenditure and unmet health needs, the improvement of medical security system and relating policy is required.
