정신분열병을 가진 사람의 환청에 대한 사회복지사 개입

Abstract

환청은 정신분열병을 가진 사람의 대표적 증상 가운데 하나로 현재 항정신병 약물치료를 위주로 심리사회적 개입이 병행되고 있으나, 클라이언트 다수가 지속적으로 환청을 경험함으로써 치료 및 재활의 진척에 지장을 받는 것은 물론 서비스 이용자로서 자신의 주관적 경험과 그 의미를 이해받지 못하는데 따른 불만을 갖고 있다. 이 연구에서는 클라이언트의 욕구에 부응하는 동시에 재활성과에 기여할 수 있는 심리사회적 개입을 좀 더 다양화하고 활성화할 이 같은 필요성에 따라, 관련 분야의 동향을 소개하고 국내 기관의 환청개입 현황을 파악하여 이들 접근을 국내 상황에 접목할 수 있는 방안을 제시하고자 했다. 정신보건 사회복지 실무자들에 대한 심층면접을 통해 현장에서 이루어지는 환청개입, 환청개입의 훈련방식, 환청개입에 대한 클라이언트의 반응을 조사한 결과, 클라이언트의 환청개입 욕구가 다양하며 이에 보다 적절하게 반응하기 위해서는 대처기술 위주의 행동적 접근에 더하여 경험과 의미를 다루어주는 인지행동 및 내러티브 접근이 유용할 수 있음이 시사되었다.

Auditory Hallucination(AH) is one of the symptomatic features of schizophrenia and it is approached mostly by antipsychotic pharmacotherapy and ancillary psychosocial interventions. Many clients not only experience persistent AH that interferes with the progress expected during treatment and rehabilitation, but also present dissatisfaction as a service user that their subjective experience and the related meanings are not sufficiently validated. This points toward the need for an increased use of diverse psychosocial approaches that can meet such client needs and contribute to positive rehabilitation outcomes as well. This study thus reviews recent developments in this area, obtains a broad understanding of social work practice in AH interventions based on interview data, and presents suggestions to adapt such developments to current settings. The authors conducted an in-depth interview with social work practitioners and collected information on interventions, training, and related client responses. The result showed that clients had diverse needs in the way AHs are approached. Suggestions were made that use of cognitive and narrative approaches that address personal experience and meanings be increased, in addition to current emphasis on behavioral strategies.
