미국의 거동불편 칩거노인들을 위한 재택의료 서비스

Abstract

재택의료는 만성복합질환을 앓고 있는 거동불편 노인들이 가지는복합적인 요구를 충족하기에 적합한 의료 서비스 제공모델이다. 1990년대 말 한국사회에서 왕진은 그 자취를 감추었으나, 미국의 재택의료는 의료 서비스 제공 수단의 한 방법으로 꾸준히 존재해왔으며, 지난 20여 년에 걸쳐 괄목한 만한 성장을 보이고 있다. 미국 재택의료의 성장의 배경에는 노인인구 증가에 따른 수요 증대 이외에도 재택의가 가지는 임상적 효과성 및 비용절감과 같은 긍정적인 요인이 작용하였다. 미국 재택의료 성장의 추동력이 되었던 지점들을확인하는 것은 2022년 재택의료센터 시범사업을 앞둔 시점에서 유의미한 작업이 될 것이다

Home-based medical care (HBMC) aims to meet the complex medical and functional needs of the homebound elderly with multimorbidity and mobility impairment by providing comprehensive and longitudinal primary care. Unlike South Korea where home visit service by a physician has completely vanished since the late 1990s, the HBMC in the U.S. has continued to expand and evolve as a new demand for HBMC has increased over the years. The favorable clinical and functional impact and cost-saving effect of the home visit service have been attributed to the resurgence of HBMC in the U.S. The article describes the crucial components of HBMC and driving forces for its growth in the U.S., and these qualities may need to be incorporated into newly implemented HBMC initiatives in South Korea.
