보건의료서비스가 강화된 통합돌봄 혁신모형의 제안

Abstract

본 연구에서는 지역사회 통합돌봄 선도사업 추진과정에서 확인된 문제점을 비판적으로 검토하고, 서로 다른 재원과 전달체계를 가진 보건의료와 사회서비스를 어떻게 사람중심 관점을 반영한 통합돌봄 모형으로 발전시킬 수 있을지를 제안하고자 하였다. 국내외 문헌고찰, 다학제 전문가 포럼 자료의 내용분석을 통해 기존체계 운영구조분석, 노인돌봄체계의 한계점을 정리하였고, 반복적 추론을 활용하여 국내 현실에 적합한 새로운 통합돌봄모형을 제안하였다. 돌봄의 목표, 대상자, 중재 및 성과관리지표의 통합성, 장기요양예방계정 신설과 성과기반 총액계약제 도입, 사례관리 및 방문의료 전담기구의 신설, 보건복지위원회와 통합재가컨소시움 구축, 통합재가 연구개발지원사업과 간호사·사회복지사 신규 채용 확대, 빅데이터 기반 고위험군 분류시스템과 모바일 전자돌봄기록 플랫폼 구축 등 총 18가지 권고사항을 제안하였다. 본 연구를 통해 도출된 모형은 지역보건의료 및 사회복지 서비스를 연계·통합하는데 도움을 줄 수 있으며, 궁극적으로 지역사회 노인의 장기요양 진입을 예방하고 시설입소를 방지하는 성과를 가져올 수 있을 것으로 기대한다.

In this study, we critically review the problems identified in the process of implementing a community care pilot project, and examine how health care and social services with different financial resources and delivery systems can be developed into an integrated care model that reflects a person-centered perspective. Therefore, we researched and reviewed the limitations of the existing elderly care system that appeared in the process of implementing the integrated care project, and suggest how the innovative model of integrated care is suitable for the situation of South Korea through domestic and foreign literature reviews, analysis of the structure of the existing system, and the 1st to 7th expert forums. As a result of the study, a total of 18 recommendations were proposed in the areas of goals and subjects, finance, service delivery, leadership and governance, workforce, information and communication technology and research. The model derived from this study can help to link and integrate local health and social care services in South Korea, and it is expected that it would ultimately be able to prevent the elderly from entering long-term care in the local community and keeping them from residential care.
