- Abstract
- The survival of adipocytes in autologous fat transplantation depends on early revascularization, which is observed at the fourth day after transplantation. Revascularization is an important step for survival of adipose tissue graft. This study was designed to investigate the efficacy of endothelial cell growth factor (ECGF;3750㎍/ml) with heparin (25unit/ml) in gelatin sponge to expedite the neovascularization and to increase the ultimate viability of adipose tissue.
Fat grafts of equal volume were harvested from the groin fat pad of white rabbit using surgical excision. The excised fat was then divided into small pieces and placed into a syringe to obtain 2ml for placement into its recipient ear pocket. The fragmented fat was mixed with gelstin sponge soaked ECGF-heparin solution. The fat fragment was then injected into the designed subcutaneous pocket in both ears of the same rabbit through the 3mm opening. The right ear was used as the experimental group which was treated with ECGF-heparin in gelatin sponge and the left ear was used as the control group which was treated with normal saline in gelatin sponge.
The viablity of adipose tissue was documented with evaluation of the changes in volume, weight, and histopathologic analysis of adipose tissue after 2,4,8, and 24 weeks.
The results are as follows :
1. In control and experimental groups, autotransplanted adipose tissue was all survived.
2. After 24 weeks, analysis of volume maintenance(0.78ml vs 0.93ml) revealed statistical significance(p<0.05, paired t-test) when comparing the volume of ECGF treated fat to saline treated control.
3. After 24 weeks, analysis of weight maintenance(0.79gm vs 0.92gm) revealed statistical significance(p<0.05, paired t-test) when comparing the weight of ECGF treated fat to saline treated control.
4. In histopathologic examination, the experimental group showed more adipose tissue in subcutaneous layer of the ear after 8 weeks. After 24 weeks, more adipocytes were observed in the experimental group. And the experimental group showed less fibrosis than the control group.
With the above results, the author concluded that the angiogenic agent, endothelial cell gowth factor, is likely to enhance survival of autotransplanted adipose tissue in rabbits.
