Abstract
The Tokyo Guidelines were developed with international scope to diagnose acute cholecystitis. These guidelines, however, were insufficient in emergency settings. Thus, we analyzed acute cholecystitis patients in order to establish a more precise tool for evaluating the severity of acute cholecystitis. In our study, a total of 135 acute cholecystitis patients were recruited. These patients were divided into three groups: Group I contained 98 patients (72%), Group II contained 33 patients (25%), and Group III contained 4 patients (3%). We compared age, sex, body mass index (BMI), heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), body temperature (BT), white blood cells (WBC), platelets, C-reactive protein (CRP), blood urea nitrogen (BUN), creatinine (Cr), total bilirubin (TB), prothrombin time (PT), aspartate transaminase (AST), alanine transaminase (ALT), alkaline phosphatase (ALP), and gamma-glutamyl transferase (GGT) between Groups I with Group II or Group III. Our results showed that there were significant differences in the past medical history of hypertension, age, SBP, duration of hospitalization, WBC, CRP, BUN, Cr, albumin, bile culture, blood culture and pancreatitis. In conclusion, when patients with acute cholecystitis enter the emergency department, abnormalities in WBC, CRP, BUN, Cr and albumin should be assessed. We can easily divide patients by severity, enabling for better treatment.
