칸디다 맥락망막염에 합병된 맥락막신생혈관에서 유리체강내 베바시주맙 주입술 치료

Abstract

목적: 칸디다 맥락망막염 환자에서 발생한 맥락막신생혈관을 유리체강내 베바시주맙 주입술을 초기치료로 사용하여 효과적으로 치유된 증례에 대해 보고하고자 한다. 증례요약: 50세의 여성이 5일 전부터 발생한 좌안의 시력저하와 변시증을 주소로 내원하였다. 환자는 2달 전 양안의 내인성 칸디다맥락망막염으로 항진균제 치료를 받은 과거력이 있었다. 형광안저촬영과 빛간섭단층촬영에서 좌안의 중심와곁 맥락막신생혈관이 확인되었다. 3회의 유리체강내 베바시주맙 주입술을 시행하였고 시각증상이 호전되었으며 빛간섭단층촬영상 맥락막신생혈관의 퇴행이관찰되었다. 6개월간의 경과관찰 동안 재발과 합병증은 관찰되지 않았다. 결론: 칸디다 맥락망막염과 관련된 맥락막신생혈관에서 유리체강내 베바시주맙 주입술은 효과적으로 신생혈관의 퇴행을 유도하며 시각증상의 호전을 가져올 수 있다.

Purpose: To report a case of choroidal neovascularization (CNV) secondary to candida chorioretinitis initially treated with an intravitrealbevacizumab injection. Case summary: A 50-year-old female presented at our clinic with decreased vision and metamorphopsia in her left eye of 5 daysduration. She received an anti-fungal treatment 2 months prior due to the presence of endogenous candida choroiditis in botheyes. Fluorescein angiography and optical coherence tomography (OCT) revealed juxtafoveal CNV in her left eye. Three monthlyintravitreal injections of bevacizumab were administered as the initial loading dosage. Her visual symptoms improved andCNV regression was observed on OCT. No recurrence or complications were observed during the 6 month follow-up. Conclusions: Based on the present study results we suggest that intravitreal bevacizumab injection can be used to effectivelytreat CNV and improve visual symptoms during the treatment of juxtafoveal CNV associated with candida choroiditis.
